We initially inspected Units 6 and 8 and it was agreed by all that these units were not in acceptable condition. Your construction personnel proposed to come back in to do the necessary construction to bring these Units up to acceptable condition. Unit 5 is not in acceptable condition. Until we receive a dedicated easement to the river and the off fall canal is constructed, I cannot recommend that these units be accepted. Yours very, truly, LLOYD AND ASSOCIATES a Robert F. Lloyd, P. F. Encl. W 0 SEBASTIAN HIGHLANDS STREET INSPECTION UNITS 5, 6, and 8 This is an inspection of Sebastian Highlands Units No. 6 and 8. Inspection was made on Tuesday, October 4, 1977. Excess asphalt laying on the shoulder along Clearmont needs to be picked up and disposed of. Strait Avenue is satisfactory to the Engineer and to the Mayor. As a general statement, the asphalt that had to be removed should be picked up along the various streets. Where new asphalt has been laid at Clearmont, the Engineer requests that the shoulder be pulled up a little bit to prevent any drop off between the asphalt and the shoulder. Clearmont is acceptable through Units 6 and 8, except for the items noted above. Mr. Flip Lloyd requests that the Report be followed about certifying to positive drainage. Shoulders on the east and west of Laconia where it ties into Fellsmere Road need to be built up and maintained as requested in Flip Lloyds original Report, on page 32. No headwalls on either one of the culverts on Lansdowne, south of Vine Avenue on Lansdowne; back slopes on Noah off Lansdowne need to be mowed. Fore slopes have been mowed, but back slopes have not. This also holds true on Lansdowne. On Lansdowne, just north of Noah, check the distance from the culvert to the edge of the pavement and see that they comply with the plans as prepared by Bristol. At Summer and Lansdowne, on the north side, headwall pipe has to be extended out approximately three (3) feet to meet the specifications and plans. On Lansdowne by Topsail, cross culvert the grates need to be lowered and also need headwalls on the culverts. Wilson Terrace is acceptable. General statement for the entire project is that "we should check the plumbness of the Street Signs. " Sebastian Highlands Street Inspection October 4, 1977 Units 5, 6, and 8 Page 2 Plans call for riprap on the swale inlets to the cross culverts in the field. At Persian and Rosebush, the culvert crossing asphalt needs to be sawed out and replaced. The culvert between Seamist and Topsail on Rosebush is short. On Rose Arbor between Tunison and Watercrest, there is a cross culvert with no headwall on the north end of it, and the swale entrances into the cross culvert need to riprapped (one side only, the other side has been done. ) Rose Arbor west of Melrose cross culvert - no headwalls on either side. Crossing at Rose Arbor and Autumn, diagonal culvert crossing cut out and replace with asphalt paving. At intersection of Dyke Street and Rose Arbor - short culvert needs to be checked. Plans will have to be resolved on Dominican as to the width of the pavement. There is eighteen feet (181) on the ground. Plans call for twenty feet (201). Refer to Page 36 of the Report. At the intersection of Bywood and Melrose, ditch on the east side will need to be maintained and reworked a little bit. No headwalls on the cross drain at Melrose between Lansdowne and Rosa Arbor. Seagrass east of Melrose needs some fill to bring the shoulder up to the pavement elevation on the south side. At Crown and Melrose, we have erosion in the shoulder and into the swale. Needs some dressing up and some material. On Barber just north of Lansdowne, a back lot ditch needs some checking over and rework of headwalls on both sides. On Lansdowne just west of Barber, check the thickness of the asphalt. Sebastian Highlands Street Inspection October 4, 1977 Units 5, 6, and 8 Page 3 At intersection of Quarry and Seaside, we have a short culvert and some erosion around the culvert and the side lot ditch. On Melrose and Jetty Terrace, there are a couple places where motor oil has been drained out of carson the edge of the road. Engineer requests we should fix those. ************* We are now doing the inspection at Unit No. 5. We are at the culvert between Lots 19 and 20, in Block 164: the headwall of the culvert on the east side needs some additional fill. We are getting some site washout. Riprap bottom needs to be repaired. The cross culvert on Blossom, Lot 172 - the west side needs some reworking. In Blocks 169, 170, 171 work has been accomplished, but there is some minor maintenance work to be done on these culverts. In Block 150 there is water still standing along the entire ditch section. Dolphin Road just west of Roseland, the first culvert headwall needs to be repaired on the south side. Inspection made October 4, 1977 Those in attendance: Eugene Cragg, City of Sebastian Robert F.
