As of 4:10 PM this date the following action is being taken at Sebastian Muni to resolve the concrete failure:
• Joe Griffin is standing by at the airport to close Runway 8-26 upon the arrival of West Construction Co
crews (ETA 6:00 PM).
• West will remove the two cans and concrete located on Runway 8-26 and fill to within 6" of finished
grade w/limerock base.
• Base will be compacted and 6" cold mix asphalt patch applied and compacted to finished grade +,- %".
• Joe Griffin will re -open Runway for operations as soon as he has completed FOD check tonight.
• Runway 8-26 will remain open throughout the weekend, barring further problems.
• E.S. from KSM will be arriving on site approximately 6:30 PM tonight as QA to insure contractor
performance of above initiatives.
• West Construction will provide early next week a plan for the permanent repair as soon as practical.
• I will be in close contact with all parties affected to insure adequacy of permanent repairs.

Your immediate remediation plan is approved and the necessary co-ordination with the Airport Manager, Joe
Griffin, is underway to close the runway for tonight's work.

This email is in response to your correspondence below. We have been in communication today to resolve this
issue with your firm
LPA and also with H.L. Pruitt our subcontractor responsible for the concrete and detail for the area in question
since we were notified by your firm. The resolution for the correction is as follows:
H.L.Pruitt will install rock base material compacted and finish with 6" of compacted cold patch asphalt. This
remedial work
Will take place tonight and the crew is mobilizing as we correspond. The replacement measures will be
scheduled early next week
And we will be contacting the approved mix bag material supplier for the investigation as to why the material
did not perform as per manufacturer product data, then we can evaluate if this material is to be used for the
finish product or an alternate mix be recommended. Please provide a report from your inspector and testing QC
firm that was onsite when this pour took place, I will
Need this information for our evaluation on the material, since he was present and advising also on the
application.

This email is in reference to your concrete pour of Wednesday, December 30, 2009, at 8:00 P.M. at the
Sebastian Municipal Airport. As you know, the requirement for the concrete around the runway edge lights
(Rnwy 4-22) crossing the open runway (Rnwy 8-26) was that it must reach a compressive strength of 2500 psi in
5 hours to allow safe operation of the active runway.
As of 12:00 P.M. today (16 hours after the pour), the sample concrete cylinders have yielded very low
compressive strengths with the highest break being 1090 psi. This is far below the strength necessary to safely
support the wheel loading of the heaviest aircraft currently based at Sebastian. In addition, not knowing the
type itinerant aircraft that may use the airport this weekend, we have recommended to the Airport Manager
that Runway 8-26 be closed until such time as sufficient compressive strength can be demonstrated or a
remedial action is performed. Closing Runway 8-26 will result in the closing of the entire airfield. You may
anticipate that the airport will seek to recover lost revenues due to this unexpected closure. Please bear in mind
that as a consequence of the failure of the concrete to meet the required structural strength, cracking, shifting,
and spalling may produce depressions and/or FOD on the runway and should be monitored closely.
I request at this time that you send us a plan for correction of this deficiency as soon as possible.
