Agenda Item Title: Golf Storage Building Change Order (CO) #2
Recommendation: Approve CO #2; $22,830 to Kerns Construction.
Background: On 16 December 2015, Council approved the contract to Kerns
Construction for $507,988.53 for construction of the Golf
Course Storage Building. At that meeting, there was consensus
from Council to add a breakroom inside the building. Kerns
agreed to look into this request and provide design and a
proposal. Change Order #2 is for the construction of the added
breakroom to the storage building.

Total Cost: $507,988.53 + CO#2 = $530,818.53

At the request of the City Council, we have explored options for revising the floor plan to better accommodate the
proposed utilization of the building. The City was e-mailed two options "A" and "B" on January 13' and
subsequently selected to pursue option "B". The foregoing change order request is intended to quantify the costs
for modifying the PEMB structure and interior partitions to the proposed configuration:

Please note that this proposal excludes all mechanical, electrical and plumbing in the newly created conference /

proper execution of this document, the Contractor is hereby notified to commence work. All changes stated
below are hereby incorporated and made a part of the Contract identified above, and all the terms and
conditions of said Contract are enjoined and in full force while executing the change(s) stipulated as follows:
