CONSULTANT SERVICE AGREEMENT (CSA)

Project Identification: Working Waterfront Park Phase 2A
IT IS AGREED to undertake the following work in accordance with the provisions of the Hoyle Tanner
& Associates Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated October 15, 2015.
Description of Assignment:
Holy Tanner & Associates will prepare professional engineering services for the topographic surveying,
permitting and engineering design of the boardwalk, restroom and shoreline improvements at the
Working Waterfront.
See attachment A for the scope of services and fee
Deliverable:
The design plans and permits
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA 1 will be performed for a fee not to
exceed the amount of $33,250.00.

ATTACHMENT A
SCOPE OF SERVICES
FOR
THE CITY OF SEBASTIAN WATERFRONT PARK RESTROOM FACILITY
The following parties are referred to hereafter: CITY (Owner — City of Sebastian); ENGINEER (Owner's
Engineer - Hoyle, Tanner & Associates, Inc.).
The engineering services for this project will consist of the site planning, architectural design for a public
park restroom facility, development review permitting and seawall permitting.
A. PROJECT DESCRIPTION
The services can be generally described as the site design for a new park restroom facility,
automobile turnout, parking and permitting, herein referred to as the PROJECT.
B. ENGINEERING PHASES
- Topographic Survey - Phase 1
- Site Design - Phase 2
- Architectural Design - Phase 3
- Permitting - Phase 4
- Landscape/Irrigation - Phase 5
- Coordination - Phase b
- Geotechnical Investigation - Phase 7
C. BASIC SERVICES
1. Design Survey
Employ the services of a licensed surveyor to collect data including but not limited
to: elevations, utility locations, structure locations, edge of pavements, etc. Final
deliverable will be in an electronic format drawing plus signed and sealed hard
copies.
2. Site Design
a. Revise preliminary conceptual layout of the site and present to the City for
approval.
b. Define pavement design parameters.
C. Prepare site plans to include geometrical layouts, typical sections, retention ponds
(as necessary), MOT details, marking plans, erosion control and various details.
3. Architectural Design
a. Building Components
Prepare drawings to include all architectural, structural, electrical, ventilation and
plumbing to obtain a permit from the City of Sebastian and the Indian River
County Utilities. Facility will be handicap accessible. Facility will be designed
with a minimum 4' lockable storage space in between the men's and women's
restrooms. The structure will be a masonry (CBS) building with pre-engineered
wood trusses and a metal roof.
b. Meetings
Attend meetings as necessary to coordinate with the City staff.
C. Signage
Design a masonry monument sign that blends in with the adjacent environment.
d. Not Included in Project Scope
• Fire sprinkler design
• Engineering of pre-engineered wood roof trusses
• Considerations for protection of sea turtles
4. Permitting
a. Site Permitting
Perform site permitting and prepare a permit modification application through
FDEP for additional impervious surface. The application fees are to be paid by the
City of Sebastian.
b. Seawall Permitting
Provide permitting for a new seawall to be constructed on the east side of the
existing structure known as Crab -E -Bills Seafood Market and Eatery and placed to
the pre-existing condition from 1992. The design of the seawall is to be completed
under a separate project scope. No environmental assessment such as seagrass
surveys and wetlands delineation will be performed.
C. Utility Permitting
Provide permit applications for the connection of a new water service and sewer
service. Permit applications will be completed with Indian River County Utilities
as well as the FDEP. The application fees are to be paid by the City of Sebastian.
d. Development Review Permitting
Submit the necessary permit application to the City Community Development
Board to attain approval to construct the project scope. Application fees to be paid
by the City of Sebastian.
5. Landscape and Irrigation
Provide a landscape and irrigation plan for plantings on the site and around the
new monument sign.
6. Coordination
Coordinate the project with the various agencies, City departments, etc.
7. Geotechnical Investigation
Employ the services of a licensed geotechnical firm to collect data including but
not limited to: soil samples, seasonal high water table, recommendations for soils
compactions, boring logs, etc.
D. ADDITIONAL SERVICES
Any Services not identified herein will serve as additional services. The Engineer shall not
provide any such Additional Services without written approval from the Cly. Specifically
the following services are not included:
1. Services in connection with permitting, delineation or mitigation of wetlands.
2. Environmental Assessments.
3. Economic Impact Statements.
4. Construction Phase Services.
E. QUANTITIES AND PROBABLE COST ESTIMATES
1. The ENGINEER shall develop estimates of quantities of the various items required for the
construction of the PROJECT.
2. The ENGINEER shall prepare an estimate of probable cost. Since the ENGINEER has no
control over the cost of labor, materials, equipment or services furnished by others, or over
the contractor(s) methods of determining prices, or over competitive bidding or market
conditions, ENGINEER's opinions of probable construction costs provided for herein are to
be made on the basis of ENGINEER's experience and qualifications and represent
ENGINEER's best judgement as an experienced and qualified professional engineer,
familiar with the construction industry; but ENGINEER cannot and does not guarantee that
proposals, bids or actual construction costs will not vary from opinions of probable
construction cost prepared by ENGINEER.
