OFFICERS' RETIRF. MENT TRUST FUND ~
THURSDAY, JANUARY 12, 1995 IN THE
CITY MANAGER'S CONFERENCE ROOM
This meeting was called to order by Chairpcmm Gore at 2:00 p.m.. Roll called and those ~ in
attendance were Gregory Gore,. Judy Robim~.Eugene Ewen, Lany Hardison, and Yartlyn Waldis. Also
in attendance as an advisor to the board was City Altorney Clifton McClelland and citizon.l)deres
McCalthy.
Reading of the Minu~ and Treasurer's Report were postponed to a later meeting.
OLD BUSINESS: Disability claim for Gary ~. Disability claim procedures discussed with City
Attorney McClelland. Board needs to desi~m~ a physician or group af physicians to evalua~ claimant.

Chairperson Gore asked.f~r a motion to have City Attorney McClelland contact Dr. 'Direc~ as to his
interest in handling this disability claim and to review his fee schedule. E. Ewert made the motion and the
motion was seconded by L. Hardison. Unanimous vote to approve.
A motion was made by L. Hardison to authorized Chairperson Gore to approve physkilm fee schedule for
the'Board upon re/xmummdation of AlWmey McClclland and to begin medical review ~claimant
NEW BUSINESS: Motion made by L. Hardison to authorized Chair to contract with Pmdenlial Bache
for a 1994 Professional Evaluagon, with a m~imum f~ of $500.00. J. Robinson seco~ed ired motion
E. Ewen made a motion to authorize Chair to purohase ~ stationary, costs not exc~md $100.00. L.
Hardison seconded. Unanimous approval.
Marry Miller's letter to the Board requesting withdrawal ~f his portion of conUibufi~ ~o Police Officers'
Retirement Trust due to his resignation as a Seba~i~n Police ~. Motion made by N, Ewert to approve
request of M, Miller to withdraw funds, seconded by L. Hardison, passed by unanimous vote.
Motion made to adjourn by L. Hautisoa and E. Ewert seconded,
