Dear Mr. Votapka:
The District is in receipt of your Environmental Resource Stormwater Permit application
under the status of standard general permit application. Upon preliminary review of the
proposed project, the following technical information is required to sufficiently review the
possible impacts the project may have on the surrounding area. This information is
being requested pursuant to the authority vested in the St. Johns River Water
Management District under chapter 373, Florida Statutes, and sections 40C-42.023 and
40C-42.024, Florida Administrative Code.
In order to expedite the review of your application, please use the application number
referenced above on all correspondence, and submit three (3) copies of all requested
information unless otherwise indicated by a specific information request.
Please provide the required treatment volume of 1.25 inches over the impervious
area plus 0.5 inches over the drainage area for basin #2. [40C -42.026(1)(a), F.A.C.]
2. Please provide the land surface elevation at each boring location and demonstrate
how the wet season water table elevation was calculated. It appears from the site
plan submitted that the land surface elevations at B-1 and B-2 is 24.6 feet. This
would put the wet season water table for B-1 at approximately 23.1 feet and for B-2
at 22.1 feet. [40C -42.026(1)(c), F.A.C.]
3. Please provide stage/area/storage relationships for both retention basins. [40C -
42.026(1)(a), F.A.C.]
4. The bottom contour at retention basin #1 is shown at 23.0 feet, however the bottom
elevation listed is 22.5 feet. Please correct this discrepancy. [40C-42.026, F.A.C.]

5. The plans list the area at the bottom of retention basin #1 as 7700 square feet,
however the area shown on the plans (using the scale of 1 "=30') at the top of bank
is 7700 square feet. Please correct the calculations to show the correct areas and
storage volume for the basin. (Making a stage/area/storage table will aid in
correcting the problem). [40C -42.026(1)(a), F.A.C.]
6. The recovery analysis for basin #2 shows F(y) is equal to zero. When F(y) is equal
to zero, F(x) is equal to zero, as shown on Figure 26-7 in the Stormwater Applicant's
Handbook. If F(x) is equal to zero, then the recovery time is infinity and the design
does not meet the District's recovery requirements. Please redesign the retention
basin with the bottom elevation higher than the wet season water table elevation.

7. The recovery analysis for basin #1 shows the remaining treatment volume to be
recovered under saturated lateral flow is 6717 cubic feet. The variable, h(2), is the
height of the treatment volume at the start of saturated lateral flow. The value
calculated is incorrect. Please use the stage/area/storage table required above in
determining the elevation at which 6717 cubic feet is provided in the retention basin
and recalculate the time to recover the remaining treatment volume. [40C -

If the applicant desires to dispute the necessity for any information requested on an
application form or in a letter requesting additional information, pursuant to section 40C-
1.605(5), F.A.C., he or she may request an administrative hearing in accordance with
section 120.57, F.S. Any petition for administrative hearing must comply with sections
40C-1.511 and 40C-1.521, F.A.C., must be filed within fourteen (14) days of receipt of
the request for additional information, and must be filed with the District Clerk, in
Palatka.
Please be advised, pursuant to subsection 40C-1.605(5),F.A.C., any application which
has not been technically completed within ninety (90) days from the date of receipt of a
request for additional information by the District, will be prepared for an Intent to Deny
at the next timely Governing Board meeting. If you require more than the allotted ninety
(90) days, please indicate this to the staff.
In addition, no construction shall begin on the proposed project until a permit is issued
by the St. Johns River Water Management District. This is pursuant to subsection
40C-40.042(2), F.A.C., which states, "No construction, operation, maintenance,
alteration, abandonment or removal shall be commenced until the permittee receives a
written authorization to proceed from the District'; and subsection 40C-42.024(1),
F.A.C, which states in relevant part, " No construction, alteration, removal, operation,
maintenance, or abandonment of a stormwater management system shall be
undertaken without a valid standard general or individual environmental resource
stormwater permit as required pursuant to this section."
