Lauderdale, FI 33309-3421 RE: Pending Permit for US #1 to move pump and well Dear Mr. Schwab: Please be advised that we are in need of your assistance in obtaining a permit to relocate a well, pump and other utilities to the right of way of US Highway #1 from private property which the City has sold and the new owners want it moved. This equipment is used for the irrigation system along the US #1 median corridor for which we already have a maintenance agreement. Gerry McSween has advised us that we will need to bury everything a minimum of thirty inches underground. It will be extremely difficult to bury a pump facility underground to accommodate her request and we are looking to you to assist us with alternate possibilities. This equipment will be a minimum of 14 feet from the white line and the housing for the pump will be approximately a 3 foot cube. We would greatly appreciate your help in this matter as we are under a tight time constraint and would like to remedy this situation as quickly as possible. (H) One or more typical cross sections to adequately reflect the underground location of the utility facility. (1) All know involved utilities in the proposed installation area shall be shown. However, if only aerial facilities requiring no addition poles are involved, then only aerial facilities need be shown on the permit drawing. (J) If overhead or underground facilities involve only one side of the rights-of- way, then only involved utilities on that side of the rights-of-way need to be shown on the permit drawing. (K) In cases, the Permittee shall list all know rights-of-way users in the installation area on the permit form and notify each of them by copy of the permit drawing, whether they are involved or not. (L) The minimum vertical clearance above or below the pavement shall be shown. (M) The approximate distance and direction to either the nearest town, major road intersection, bridges, or railroad crossings. (N) Other significant physical features such as vegetation, wetlands or bodies of water shall be indicated on the plans. The District Manager may be contacted for assistance to determine any potential to FDOT vegetation. (0) A sample key map showing the location of this proposed facility should be included. (P) When the proposed utility work requires MOT, the permit application package must include a TCP. (Q) In order to document existing conditions of the work area prior to any Utility work, a minimum of one and maximum of six pictures, based on the complexity of the project, must be submitted with the application as a remedy for claims or final approval concerns.
