The Owner/Developer or his duly authorized representative, the Engineer -of -Record and the construction contractor shall have a pre -construction meeting with Indian River County Department of Utility Services (IRCDUS) a minimum of five working days before beginning construction. 3. The contractor shall notify Customer Service, 1RCDUS, at (772) 567-8000 a minimum of 48 hours prior to beginning construction or performing any system tests. 4. All water and sewer utility locations are to be coordinated with other utilities such as, but not limited to, electric, cable, telephone, irrigation, etc. Minimum setback requirements from water and sewer utilities, as outlined below, must be adhered to prior to acceptance of the water and sewer utilities. Type of Structure _Min. Horizontal Separation between Utility Water/Sanitary lines and other utilities& structures. a) Aboveground permanent structures (i.e. Walls, Trees, Transformer pads, etc.. Depth of the pipe plus diameter of the pipe b) Underground utility lines (i.e., telephone, ower, drainage, etc.) Four (4) feet C) Surface water body [op of bank (i.e. Lakes, Ponds, Canals, etc). Two times the depth of the pipe plus the diameter of the pipe FINAL ACCEPTANCE OF WATER AND SEWER IS CONTINGENT UPON A FINAL INSPECTION BY THE UTILITIES DEPARTMENT AFTER ALL OTHER ONSITE UTIIJ= HAVE BEEN INSTALLED SO THAT MINIMUM SEPARATION REQUIREMENTS, AS OUTLINED ABOVE, CAN BE VERIFIED, 5. This Utility Construction Permit does not eliminate the necessity to obtain a right-of-way permit from Indian River County Public Works" Department or other permits that are required by the Florida Department of Environmental Protection or any other county, state, or federal agencies. 6. No construction shall begin until all required easements have been acquired. 7. All applicable permits allowing utilities construction inside any right-of-way shall be submitted prior to commencement of construction. The Land Development Permit must be obtained, if applicable, prior to commencement of any utility construction. 8. Capacity Charges must be paid in full prior to commitment of capacity or issuance of building permit whichever comes fust. THERE IS NO GUARANTEE THAT CAPACITY WILL BE AVAILABLE AT TIME OF REQUEST. 9. County Inspection Services: The County's hours of Operation for this project is limited to between the hours of 8:00 a.m. and 4:00 p.m., Monday through Friday, excluding holidays. The DEVELOPER shall coordinate needed inspection services between these hours of Operation. Should the DEVELOPER require County inspection services beyond the designated hours then the DEVELOPER shall pay the inspectors' hourly prevailing rate times an overtime dueet multiplier of 1.5 times the hourly rate. The DEVELOPER shall pay for a minimum of three (3) hours of overtime for each occurrence of 'inspection services needed beyond the designated hours of Operation. The County's average prevailing wage rate plus overtime for inspection is $38.64 per hour (FY -2005 rate) The DEVELOPER shall not have the right to declare this Agreement in default because it disagrees with the fees and charges imposed for the extended use of the County Inspectors. 10. All equipment, materials, and workmanship shall meet or exceed current Indian River County Water and Wastewater Utility Standards and shall be subject to the unconditional inspection and approval of the Indian River County Department of Utility Services. ATTACHMENT A - PAGE 1 ATTACHMENT A UTH.TTY CONSTRUCTION PERMIT — SPECIAL PROVISIONS 11. Only IRCDUS approved appurtenances shall be used in construction. 12. Shop drawings shall be provided and reviewed by IRCDUS prior to construction. 13. The Engineer -of -Record (EOR) shall have an on-site representative (inspector) whom shall witness and document all materials used, installation procedures, problems encountered and all tests specified by the Operation Permit Checklist Daily construction reports shall be submitted not less than monthly to IRCDUS. The daily reports shall be signed and sealed by the EOR. The daily construction reports shall be submitted to IRCDUS no later than seven days after completion of that portion of construction requiring clearance. Indian River County has unconditional rights to inspect the construction and materials at any time. 14. All connections to the IRCDUS system and operation of utility system valves and equipment shall be made under the direct observation of personnel from IRCDUS. Where loss of utility service will occur, a minimum of a 48-hour notice to IRCDUS and the public is required. A 48-hour notice is required for access to private property. 15. No testing of potable water or sanitary sewer system shall commence until as -built drawings have been submitted, approved and accepted by the Department of Utility Services. 16. Upon completion of construction, utility system shall be flushed, disinfected and tested in accordance with the current IRCDUS Specifications. 17. Project Closeout: At the time of final completion an inspection shall be held by the COUNTY in the presence of the property owner, DEVELOPER, Contractor and Engineer -of -Record. At this time, the DEVELOPER shall provide all necessary documentation as required by the Utility Construction Pefmit and regulatory agencies, such as the FDEP. At the time of completion of all utility work, a final inspection shall be held. The DEVELOPER shall make arrangements with the Owner, Contractor, Engineer -of Record and the COUNTY for a joint follow-up inspection and shall send a written notice to said parties to inform them of the date and time of the inspection. After the inspection, the COUNTY, through the Engineer -of -Record, shall inform the DEVELOPER of any corrections required. 18. The one-year maintenance period shall not commence until a final certificate final Certification -of - Construction (100% completion) has been prepared and approved by FDEP and a Memo of Acceptance has been issue by the COUNTY. 19. Partial Utilization: The COUNTY Utilities Department shall have the right to utilize or place into service any utility equipment pursuant to FDEP Certificate -of -Construction (DEP form 62-555.900) or other usable portion of the work prior to completion of the work. In such case, the COUNTY, identifying the specific portion or portions of the work to be so utilized or otherwise placed into service, will notify the DEVELOPER in writing. The DEVELOPER shall understand that until such written notification is issued, all responsibility for ownership, care and maintenance of the work shall be borne by the DEVELOPER. Upon issuance of said written notice of partial utilization, the DEVELOPER accept full responsibility for the protection and maintenance of all such items or portions of the work described in the written notice until final acceptance by the COUNTY. The DEVELOPER shall retain full responsibility for satisfactory completion of the work, regardless of whether a portion thereof has been partially utilized by the COUNTY and the DEVELOPER'S one-year correction period shall commence only after the date of Substantial Completion for the work. DEVELOPER shall be further responsible for submitting a final Certification -of - Construction (100% completion) to FDEP for any outstanding portion of the work- 20. ork20. Any deviation from the approved construction drawings or County specifications must be specifically identified and justified by the Engineer. 8. Copy of a clearance letter from the Florida Department of Environmental Protection (FDEP), authorizing the water distribution system to be placed into service. 9. Dedication of the water distribution system and accompanying easements. The dedication is to include an itemized list of all materials along with total materials, construction and engineering costs. 10. Where the water distribution system is located in established easements or road rights-of-way, the attached bill of sale is to be executed along with an itemized list of all materials to include materials and construction.costs. 11. Complete on-site inspection by a County utility inspector with confirmation that the water distribution system appears acceptable. 12. $150.00 review fee payment. 13. Part I Subpart (5)(A) items 42, 43, 61, 63, and 64 of the application were "Xed" and an explanations of N/A was provided in Part 11 Subpart (5)(B). Please be advised that an explanation of "Not Applicable" is inadequate. In the space provided, or an additional attached sheet, give a brief (but specific) explanation for each of these items and resubmit. 2. Revise Part II Subpart (5)(A)(45) of the permit application to indicate the design complies with 62-604.400(2)(b), F.A.C. by initialing. In review of the submitted drawings, the lightning arrester detailed on sheet CD -2 satisfies the requirement. The Engineer of Record initialed Part II (5)(A)(48 & 78) indicating the design meets the required 2 feet per second scouring velocity; however based on the flows proposed, the minimum 2.0 feet per second scouring velocity will not be met in the 2 -inch and 4 -inch force mains. In accordance with Recommended Standards for Wastewater Facilities (RSWF) Chapters 42.38 and 48.1, at design pumping rates, a cleansing velocity of at least 2.0 feet per second (0.60 m/s) should be maintained. Revise the lift station design, lift station calculation (1.25 -inch diameter force main was used in the velocity calculation provided), and appropriate documentation to ensure compliance with this standard or revise Part If (5)(A)(48) of the permit application to indicate the design is not in accordance with RSWF 42.38 and 48.1 by placing an "X" in the space provided, and include an explanation in Part II (5)(B). Additionally, if a minimum scouring velocity of 2.0 ft/s is not provided, include a letter from the Utility or responsible partying stating that they will provide any additional maintenance needed in the applicable force mains, due to the lower scouring velocity. Due to the significantly lower scouring velocity proposed in the 4 -inch force main, the Department may require additional assurances if additional flows are not proposed (i.e. maintenance schedule, dates when additional flows will be added, etc). 4. Part 11 Subpart (5)(A)(50) of the permit application has been "Xed" and an explanation provided that does not address each of the seven (7) parts if item 50. Suitable shutoff and check valves shall be placed on the discharge line of each pump. The check valve shall be located between the shutoff valve and the pump. Check valves shall be suitable for the material being handled and shall be placed in the horizontal portion discharge piping except for ball checks, which may be placed in the vertical run. Valves shall be capable of withstanding normal pressure and water hammer. All shutoff and check valves shall be operable from the floor level and accessible for maintenance. Valves required under paragraph 42.5 shall be located in a separate valve pit or can be located above ground. There appears to be a discrepancy between Part 11 Subparts (5)(A)(53 & 54) of the permit application and the lift station detail in the engineering drawings. The items were "Xed" indicating that the design does not comply with RSWF 42.64 and 42.71; however, Sheet CD -2 of the proposed engineering drawings shows a vent on the lift station which satisfies this requirement. Please revise by initialing each of these items to indicate that they meet the requirement and resubmit. 6. Review the flow meter requirements presented in Part II Subpart (5)(A)(61). It should be noted that suitable devices for measuring flow are required for all lift stations. If it is the design engineer's professional opinion that a flow measuring device such as an elapsed time meter would not serve a useful purpose then the design engineer is required to present adequate engineering justification in Part 11 Subpart (5)(B). 7. Revise Part II Subpart (5)(A)(65) of the permit application to indicate the design complies with Recommended Standards of the Wastewater Facilities (RSWF) paragraph 44.1. Submersible pumps and motors shall be designed specifically for raw wastewater use, including totally submerged operation during a portion of each pumping cycle and shall meet the requirements of the National Electrical Code for such units. An effective method to detect shaft seal failure or potential seal failure shall be provided. 8. Revise Part 11 Subpart (5)(A)(68) of the permit application to indicate the design complies with Recommended Standards for Wastewater Facilities (RSWF) paragraph 44.32. The motor control center shall be located outside the wet well, be readily accessible, and be protected by a conduit seal or other appropriate measures meeting the requirements of the National Electric Code, to prevent the atmosphere of the wet well from gaining access to the control center. The seal shall be located so located that the motor may be removed and electrically disconnected without disturbing the seal. When such equipment is exposed to weather, it shall meet the requirements of weather proof equipment NEMA 3R or 4. 9. Part 11 Subpart (5)(A)(70) of the permit application is initialed indicating the design complies with RSWF paragraph 44.4. Valves required under paragraph 42.5 shall be located in a separate valve pit or located above ground. Provisions shall be made to remove or drain accumulated water from the valve pit. The valve pit may be dewatered to the wet well through a drain line with a gas and water tight valve. Check valves that are integral to the pump need not be located in a separate valve pit provided the valve can be removed from the wet well in accordance with paragraph 44.2. Provide the valve pit detail in the proposed engineering drawings and resubmit. 10. Revise Part Il Subparts (5)(A)(71, 72, and 73) of the permit application and the engineering drawings to indicate the design complies with 62-604.400(2)(a)(l and 2), F.A.C., and RSWF 45, 46.423, and 46.433. At a minimum, alarm systems, emergency pumping capability, and an emergency generator receptacle shall be provided for all pumping stations. Where portable pumping and/or generating equipment or manual transfer is used, the design must include sufficient storage capacity with an alarm system to allow time for detection of pump station failure and transportation and connection of emergency equipment. 11. Revise Part II Subpart (5)(A)(76) of the permit application to indicate the design complies with RSWF 46.422. Please revise the permit application to indicate the design meets the requirement or revise the explanation provided to indicate there are no permanently installed or portable engine -driven pumps used for the project design. 12. Revise the engineering drawings to include the following items and resubmit signed and sealed drawings for the proposed lift station. a. lockable fence or otherwise designed locking mechanisms on the wet well, control panel, and valve vault, b. elapsed time meters, c. complete detail of the valve pit, including ,check and shutoff valves for each pump, along with a detail of provisions to remove or drain accumulated water from the valve pit, if it is a totally enclosed valve pit, c. an emergency pump connection in the discharge lines of the proposed lift station, d. concrete ballast weight and/or dimensions, and e. the following separation requirements. The separation notes shall include the minimum vertical and horiwntal separation between potable water and sanitary sewer mains to comply with Rules 62-604.400(2)(g) - (i), F.A.C. and 62- 604.400 (3), F.A.C. as noted below. CITY OF SEBASTIAN OCD -DW -11-0329 PAGE 3 New or relocated, underground water mains shall be laid to provide a horizontal distance of at least six feet, and preferably ten feet, between the outside of the water main and the outside of any existing or proposed gravity- or pressure-type sanitary sewer, wastewater force main, or pipeline conveying reclaimed water not regulated under Part III of Chapter 62-610, F.A.C. The minimum horizontal separation distance between water mains and gravity -type sanitary servers shall be reduced to three feet where the bottom of the water main is laid at least six inches above tite top of the sewer. New or relocated, underground water mains crossing any existing or proposed gravity- or vacuum -type sanitary sewer shall be laid so the outside of the water main is at least six inches, and preferably 12 inches, above or at least 12 inches below the outside of the other pipeline. However, it is preferable to lay the water main above the other pipeline. New or relocated, underground water mains crossing any existing or proposed pressure-type sanitary sewer, wastewater or storm water force main, or pipeline conveying reclaimed water shall be laid so the outside of the Coater main is at least 12 inches above or below the outside of the other pipeline. However, it is preferable to lay the water main above the other pipeline. At the utility crossings described above, one fill length of water main pipe shall be centered above or below the other pipeline so the water main joints will be as far as possible from the other pipeline. Alternatively, at such crossings, the pipes shall be arranged so that all water main joints are at least three feet from all joints in vacuum -type sanitary sewers, or pipelines conveying reclaimed water regulated under Part III of Chapter 62-610, F.A.C., and at least six feet from all joints in gravity -or pressure-type sanitary sewers, wastewater force mains, or pipelines conveying reclaimed water not regulated under Part III of Chapter 62-610, F.A.C.
