OBSERVATION AND DOCUMENTATION
OF RECENTLY COMPLETED EXISTING CONSTRUCTION
"ALSO KNOWN IN THE CONSTRUCTION INDUSTRY AS A PUNCH -LIST"
Holeman Suman Architects, Inc. (HSA) has prepared this report to provide an opinion of
the aesthetics of the existing buildings located at Hardee Park, Barber Street Sports
Complex North and the Yacht Club, Sebastian, Florida 32958.
As requested, HSA performed an on-site observation on 2/12/02 at 1:00pm.
Observation was performed by Wayne Albritton, Site Representative with HSA. The
assessment is based on visual observation of the buildings, building systems, and
review of available construction documents. No sampling or testing was employed.
OBJECTIVE:
Observe the completed construction of the roof of the buildings, flashing details, soffits,
exterior masonry walls, interior finishes, floors, doors, lighting, bathroom fixtures and
accessory items and render an opinion as to the quality of workmanship performed by
others as compared to that of industry standards.
Observations: Exterior and Interior of Buildings.
A detailed observation revealed many visual, material and workmanship imperfections.
These include the installation of the fascia/soffit system, flashing details, split -face
masonry wall construction, doors/frames and hardware, toilet partitions and doors,
bathroom fixtures and accessory items, painting, floor surfaces and floor drains, ceilings
and general carpentry.
A review of the attached photos will substantiate all of the above comments.

Please review photos nos. 1-125 as part of the report.
CONCLUSION
It is concluded that many of the defects and imperfections that were observed were
caused at the time of original construction.
HSA is available to assist with the satisfactory resolution to the building's problems.

Dear Mr. Fisher:
Flinn Construction performed an on-site inspection of the restrooms located at Hardee
Park, Barber Street Sports complex and the Sebastian Yacht Club, based on the deficiency
report submitted by Holeman Suman Architects, dated February 20, 2002. Based on
discussions with your staff, a large portion of the report deals with cosmetic problems
which will be repaired "in house". We propose to furnish materials, equipment and labor
to repair the major problems as follows:
Yacht Club:
1. Chip and grind concrete floors to achieve a proper pitch towards drain in
both women's and men's rooms; prepare floor for new file surface; supply and install new
ceramic tile; reset floor drain. (Color of the to be selected from samples provided.)
2. Remove existing hardware, install new push plates and reinstall hardware
on entrance doors.
3. Replace shorted -out electric lock on women's room entry door
4. Clean paint from handicap bars.
5. Patch holes in stall doors.
Total cost estimated to be less than $5.000.00
