Subject: City of Sebastian Phase II Municipal Separate Storm Sewer System (MS4) NPDES Permit ID Number FLR04E124 (Cycle 3) Request for Additional Information: Notice of Intent Dear Mr. Minner: The Florida Department of Environmental Protection has received the Notice of Intent to Use Generic Permit for Discharge of Stormwater from Phase II Municipal Separate Storm Sewer Systems (NOI) and permit application fees for the Phase II operator above. In order to fully review your application for renewal of permit coverage, the Department is requesting you incorporate our comments in the attached document. Please revise your NOI to address the comments in the attached marked up NOT. Submit your signed revised NOI electronically within 30 days from the receipt of this letter. You will NOT be covered under the permit until these items are addressed and the revised NOI is processed. Please note that failure to provide the revised NOI by the deadline may result in denial of coverage.
