DAMAGE DESCRIPTION AND DIMENSIONS:

During the incident peood of Monday, October 3, 2016 through Wednesday, October 19, 2016, Hurricane Matthew produced severe storms,
tomadoes, high sustained winds, and prolonged Wade of rainfall, resulting in widespread goading and wind damage, power outages. and
transportations difficulties affected The Applicant the City of Sebastian located m Indian River County, Florida. Damages Include me
I. Crab E Bills Pier located at 1540 Indian River Drive, Sebastian FI. 32958 (GPS:27.821361-60.47000) surge titles and strong wave action
damaged the pier with wood decking and limber plies 5 ft. x 146 ff.
2. Fisherman Landing Pier/Dock located at 1532 Indian River Drive, Sebastian. Ft. 329M (GPS:27.8216W-80.46972) surge tides and

action damaged a section 2,943 at. of ere pier with wood decking l ighting undemeath), joist, waterline, mil lighting, concrete piles and
ladders on the Iwo ringer piers
4. Yacht Club Pier located at 820 Indian River Drive, Sebastian F1.32958 (GP5:27.8111 V-80 46416) surge tides and strong wave action
damaged ate ADA accessible pier specifically; 1 section 8 8. x 3601f. of mod decking, joist and concrete piles; 1 center section 8 ft. x 120
If, of wood decking, joist and wnchate piles and a T section 10 fl. x 30 ff. of this 3601f. pier. The water lines, lighting, two (2) light poles and

The Applicant contracted the services of MBV Engineering, Inc, to perform a stmctuml engineering review at a cost of $7,500.00 and

contracted the services of Pinson Contractor Services, I.I.C. To perform a Seagrass Survey at a coat of $2,600.00

To Bring beck the facilities to pre -disaster design and function the Applicant awarded the bid to Brother's Construction, Inc. the contract to
rebuild the following 4 (four) piers.
Site 1: Crab E Bills Pier located at 1540 Indian River Drive, Sebastian FI. 32958 (GPS:27.82138/-80.47000) the contractor will perform the
following work:
•DemolNon of 100% of existing pier (deck and piles)
-Replace 5 ft. x 146 If. pier with timber plies, decking and railing.
-Per Florida Building Cade (FBC) install electrical lighting

-Demolition of W% of existing pier (deck and plies)

-Replace 78 damaged piles
-Per FBC install 210 electrical lighting
-install a Weaken light pole

following work:
-Demolition of 75% of existing pier (deck and piles)
-Replace 2,943 an. (2 in. x 8 In) decking, joist and rails
-Replace fib concrete piles
-install waterline, lighting and 20 rail lights
-install 2 (Me) 531f x 8 ft. (2 In, x 8 In.) deck and piles
-install ladders on two finger piers

-Demolition of 100% of existing pier (deck and piles)
•1 (one) section 8 ft. x 3601f. of decking, joist and plies
•1 (ore) center section 8 ft. x 1201f. of decking. joist and alis
•1 (ore) T supper 10 ft. x W H. of Me 3601f. pier.
-install water linea, lighting, 28 rails lights and 2 pole light
-Replace 501f. x 8 ft. (2 in. x 61n.) southern tleck, joist and rails.
-install 105 concrete piles
-Install ADA rails S ladders on two finger piers

Project Nates:
The Original bid submitted by Brother Construction, Inc., included rebuild to the Boudrot Pier (North Main Street Pier). The PS has deducted
Me cost of repair to Boudrot Wer in the amount of $93,327.00. A PW for an alternate project will be written to capture Boudrot Pier.
The public worts department is responsible for routine maintenance and deaning of the City's public docks, piers and boat ramp facilities.
The department does not keep a maintenance record but has provided historical invoices for maintenance performed in line past. (Attached)
Crab E Bills was constructed in 1986
Fisherman Lending Pier was constructed in 1978
The Main street pier was reconstructed in 1998
Yacht Club pier was constructed in 1969
The Applicant does not keep maintenance records; however they have provided historical Invoices for repalm FYI 1. FYI 3. FY14 and FYI
(Attached)
Standard Comments:
DIRECT ADMINISTRATIVE COSTS: The Subgrantee chooses not to claim costs to manage and administer this project as pad of the Public
Assistance program's grant award. Declining such costs does not exempt the Subgrantee from maintaining records adequately and
documenting the source and application of funds as required In 2 CFR 200.413.

HAZARD MITIGATION PROPOSAL: Hazard Miligadon under section 406 has been considered for this project and due to the type of work
or project, effective mitigation is not feasible within the requirements of 44 CFR 206 226(c).
PROCUREMENT: The Applicant was advised by FEMA PAC and/or Project Specialist that in the seeking of proposals and letting of
contracts for eligible work, the Applicant most comply with its Local, State and/or Federal procurement laws, regulations, and procedures as
required by 2 CFR 317-326.
RECORD RETENTION: As described is 2 CFR 200.33 Subgrantee must maintain all work-related reports for a period of three (3) years
form Subgrantee ctosure (final payment), all records relative this project worksheet are subject to examination and audit by the State, FEMA
and the Comptroller General of the United States and must reflect work related to disaster specific costs.
PERMITS: Federal Funding is contingent upon acquiring all necessary Fetleral, State and Local permits. Noncompliance with this
requirement may jeopardize the receipt of federal funds. The applicant is responsible for obtaining all required permits prior to the
commencement of work.
ENVIRONMENTAL AND HISTORIC PRESERVATION Applicant must comply with all applicable environmental and historic preservation
laws. Federal funding Is contingent upon acquiring all necessary Federal, State and Local permits. Noncompliance with this requirement
may jeopardize lite receipt of federal funds.
CHANGES TO SCOPE OF WORK DESCRIBED IN THIS PW/SA (SUBGRANT APPLICATION): The applicant shall comply with all
applicable codes and standards in the completion of eligible work to repair or replace damaged public facilities. Any change to the approved
scope of work on a Project Worksheet (PW/SA) must be reported and approved before work begins. Failure to report changes may
jeopardize Federal and State funding. In the case of a change In scope of work, the applicant shall notify the Florida Division of Emergency
Management program representative prior to starting work.
Donated Resources: Volunteer Labor Hours when provided by the applicant and determined to be eligible by FEMA to offset the 25% local
match (non -Federal share) cannot exceed the maximum credit allowed for donated resources for a project. The amount of donated
resources credit that can be applied to a protect is capped at the non -Federal share of emergency work so that the Federal share will not
exceed the Applicant's actual out-of-pocket costs per the Public Assistance Program and Policy Guide (PAPPG), pp 3536.
INSURANCE REVIEW: The applicant is aware that all projects are subject to an Insurance review as stated In 44 C.F.R. Sections 206.252
and 206.253.lf applicable, an insurance determination will be made either as anticipated proceeds or actual proceeds in accordance with
the applicant's insurance policy which may affect the total amount of the project. Approval of this project may result in an obtar/malntaln
insurance requirement. The Subgrantee must comply with Insurance reviewer terms and conditions upon receipt of sub -grant from the
State.
COST BASIS FOR LABOR, EQUIPMENT AND MATERIALS: Costs used to formulate this project were based on. Applicant actual or
recent contract costs.
AUDIT STATEMENT: All documentation related to this project worksheet is subject to audit and must reflect tlisaster — relatetl work and
project— specific seat. The applicant has been advised of responsibility to maintain supporting documentation (records). The type of records
to be maintained is specified In FEMA policy 2 CFR Subpart F, Audit Requirements. Records must be maintained for three 3 years from the
date the last project was completed or from the date final payment was received, whichever is later.
75% FEDERAL FUNDING: In accordance with 44 CFR 201 and current disaster declaration determinations. this project worksheet will
be funded with the Fetleral Cost share at 75% of all eligible costs.
By accepting this grant the Applicant to the best of their ability acknowledges that all damages descrbed within this Sub -grant Application
and all associated costs being claimed were a direct result of the declared event. and in connection with the Incident period for DR4283 or
DR4280 with the exception of requests for alternate or Improved projects.
BACKUP DOCUMENTATION: Backup Documentation has been reviewed and a 100% sampling has been included with this project.
WORK TO BE COMPLETED: Upon completion, this site will be returned to Its original design, function, and capacity within the anginal
footprint. Acquiring all necessary Federal, State, and local permits is required for Federal Funding. Noncompliance with this requirement
may jeopardize the receipt of Federal funds
LARGE PROJECT ADJUSTMENT, ANY CATEGORY: When Subgranl Applications for Assistance (Project Worksheets) are written as
Large Projects ($121,801 or greater) not participating in the SRIA Alternative Procedures Pilot Program for Permanent Work— fixed
estimated cost, an adjustment must be made during the closeout process In order to match the actual eligible dollars spent. This will require
a Change (version) to be submitted In EMMIE to capture the over-runlunder-run.
