SURVEY NOTES
1. SURVEY INFORMATION IS BASED ON A TOPOGRAPHIC SURVEY BY MCMILLEN SURVEYING, INC., AND AS-BUILT
SURVEY BY WILLIAM MOTT LAND SURVEYING, INC. DATED 06/02/2016 AND 04/18/18 RESPECTIVELY.
2. BATHYMETRIC AND TOPOGRAPHIC CONTOURS REPRESENT GENERAL CONDITIONS EXISTING ON THE DATE OF
SURVEY AS SHOWN HEREON.
3. IF THE CONTRACTOR FINDS A DIFFERENCE BETWEEN THESE DRAWINGS AND EXISTING CONDITIONS, OR
OTHER CONDITIONS WHICH PROHIBIT EXECUTION OF THE WORK AS DIRECTED IN THESE DRAWINGS, THE
CONTRACTOR SHALL NOTIFY THE OWNER'S REPRESENTATIVE IMMEDIATELY.
4. CONTRACTOR SHALL CONFIRM THE LOCATIONS OF ALL UTILITIES PRIOR TO COMMENCEMENT OF WORK.
THESE PLANS DO NOT WARRANT THAT UTILITIES ARE SHOWN ACCURATELY OR THAT ALL UTILITIES ARE
SHOWN. THE CONTRACTOR SHALL BE RESPONSIBLE FOR DETERMINING THE EXACT LOCATION OF ALL
UTILITIES PRIOR TO CONSTRUCTION OPERATIONS. ANY UTILITIES DAMAGED OR DISTURBED DURING
CONSTRUCTION SHALL BE RESTORED TO THEIR ORIGINAL CONDITION AT THE CONTRACTOR'S EXPENSE.
5. CONTRACTOR SHALL RETAIN A LICENSED PROFESSIONAL SURVEYOR FOR SETTING OUT AND VERIFYING
LOCATIONS AND ELEVATIONS OF THE WORK.
6. THE CONTRACTOR SHALL PROVIDE AN AS-BUILT SURVEY SEALED BY A LICENSED PROFESSIONAL ENGINEER
OR SURVEYOR UPON COMPLETION OF THE PROJECT AND PRIOR TO SUBMITTING FINAL APPLICATION FOR
PAYMENT. AS-BUILT SURVEY SHALL MEET REQUIREMENTS OF THE PROJECT PERMITS.
GENERAL NOTES
1.ALL WORK SHALL CONFORM TO THE REQUIREMENTS OF THESE PLANS AND BOTH THE PROJECT NOTES AND
FDOT STANDARD SPECIFICATIONS. FDOT STANDARD SPECIFICATIONS ARE REFERENCED FOR MATERIAL
SPECIFICATIONS AND CONSTRUCTION METHODS UNDER DIVISION II “CONSTRUCTION DETAILS”. DIVISION I
“GENERAL REQUIREMENTS AND COVENANTS” OF THE FDOT STANDARD SPECIFICATION DOES NOT APPLY. FDOT
STANDARD SPECIFICATIONS REFER TO THE FLORIDA DEPARTMENT OF TRANSPORTATION “STANDARD
SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION”, JULY 2018 EDITION AND ALL SUPPLEMENTAL
SPECIFICATIONS.THE CONTRACT WITH OWNER SHALL GOVERN OVER FDOT SPECIFICATIONS. MEANS OF
MEASUREMENT AND PAYMENT SHALL BE AS STIPULATED BY THE OWNER AND THE CONTRACT.
2.CONTRACTOR SHALL STRICTLY COMPLY WITH ALL APPLICABLE SAFETY REGULATIONS. MEANS, METHODS, AND
TECHNIQUES OF CONSTRUCTION ARE THE RESPONSIBILITY OF THE CONTRACTOR.
3.CONTRACTOR SHALL ABIDE BY ALL APPLICABLE ENVIRONMENTAL PROTECTION LAWS, STANDARDS, AND
REGULATIONS. THIS INCLUDES ADHERING TO ALL PERMIT CONDITIONS AND REPORTING REQUIREMENTS.
4.THE CONTRACTOR SHALL INCORPORATE BEST MANAGEMENT PRACTICES TO PREVENT CONSTRUCTION
DEBRIS, DUST, AND OTHER MATERIALS FROM LEAVING THE IMMEDIATE WORK AREA AND ENTERING LOCAL
WATERS. THIS INCLUDES TURBIDITY CONTROLS AND MONITORING FOR IN-WATER WORK AND OTHER
LAND-DISTURBING ACTIVITIES. TURBIDITY CONTROLS AND MONITORING SHALL BE IN ACCORDANCE WITH
PROJECT PERMITS.
5. ADDITIONAL WORK PERFORMED BY OTHER CONTRACTORS MAY BE OCCURRING AT THE PROJECT SITE AT THE
SAME TIME AS THE WORK DEFINED IN THESE DOCUMENTS. CONTRACTOR IS RESPONSIBLE FOR COORDINATION
OF THESE OTHER PROJECT CONTRACTORS AND ELEMENTS INTO THEIR WORK.
6.CONTRACTOR SHALL BE RESPONSIBLE FOR COORDINATING SITE LAY DOWN AND STORAGE AREAS WITH
OWNER'S REPRESENTATIVE PRIOR TO CONSTRUCTION. NO STORAGE OF CONSTRUCTION EQUIPMENT OR
DEBRIS SHALL OCCUR IN ANY LOCATION ON SITE OTHER THAN THE AREA APPROVED BY THE OWNER'S
REPRESENTATIVE. THE CONTRACTOR SHALL REMOVE ALL SURPLUS MATERIAL, SUPPLIES, AND DEBRIS AT THE
COMPLETION OF THE PROJECT AND PRIOR TO SUBMITTING THE FINAL APPLICATION FOR PAYMENT.
7.T HE CONTRACTOR SHALL REMOVE ANY DEBRIS FROM THE SITE ON A DAILY BASIS. DISPOSAL OF THE MATERIAL
IS THE RESPONSIBILITY OF THE CONTRACTOR.
8.THE CONTRACTOR SHALL REMOVE ALL BARGES, WORK BOATS, STAGING EQUIPMENT AND MATERIAL, AND ANY
OTHER TEMPORARY PLATFORMS OR CONSTRUCTION AT THE COMPLETION OF THE PROJECT AND PRIOR TO
SUBMITTING THE FINAL APPLICATION FOR PAYMENT. ALL DEBRIS SHALL BE DISPOSED OF IN A PERMITTED
SANITARY LANDFILL
9.THESE DRAWINGS ARE INTENDED TO BE USED WITH SITE-CIVIL AND STRUCTURAL DRAWINGS AND
SPECIFICATIONS BY OTHERS. CONTRACTOR IS RESPONSIBLE FOR COORDINATION OF THESE OTHER PROJECT
ELEMENTS INTO THEIR WORK.
10.SITE VERIFICATION: PRIOR TO BEGINNING CONSTRUCTION THE CONTRACTOR SHALL CHECK THE DRAWINGS
AGAINST THE SITE AND NOTIFY THE OWNER AND ENGINEER IN WRITING OF ANY DISCREPANCIES IN
DIMENSIONS OR SITE CONDITIONS. THE CONTRACTOR SHALL NOT BEGIN CONSTRUCTION IN ANY SUCH
AFFECTED AREA UNTIL THE DISCREPANCY HAS BEEN RESOLVED BY THE CONTRACTOR AND APPROVED BY THE
ENGINEER.
11.ALL SUBAQUEOUS DEBRIS AND EXISTING FOUNDATIONS UNCOVERED DURING CONSTRUCTION SHALL
IMMEDIATELY BE BROUGHT TO THE ATTENTION OF THE ENGINEER.
12.THE CONTRACTOR SHALL BE RESPONSIBLE FOR BRINGING ALL ELEMENTS OF THE PROJECT IN CONFORMANCE
WITH THESE PLANS AND SPECIFICATIONS. IF ANY MODIFICATIONS ARE REQUIRED IN ANY ELEMENT, THE
CONTRACTOR SHALL SUBMIT PROPOSED CHANGES TO THE ENGINEER FOR APPROVAL.
13.ANY DAMAGE TO STATE, COUNTY, OR LOCAL ROADS, OR PUBLIC OR PRIVATE WATERWAY STRUCTURES
CAUSED BY THE CONSTRUCTION ACTIVITIES RELATED TO THIS PROJECT SHALL BE REPAIRED BY THE
CONTRACTOR TO THE SATISFACTION OF THE OWNER.
14.THE ENGINEER SHALL FIELD-APPROVE CONTRACTOR'S LAYOUT STAKING PRIOR TO COMMENCEMENT OF
CONSTRUCTION.
15.CONTRACTOR SHALL PROVIDE THE FOLLOWING AS PART OF THE BID SUBMITTAL:
·A PRELIMINARY WORK PLAN TO INCLUDE GENERAL METHODOLOGIES PROPOSED TO ACHIEVE THE WORK
DEPICTED IN THESE DRAWINGS AND IN CONFORMANCE WITH THE TECHNICAL SPECIFICATIONS AND ALL
PERMIT REQUIREMENTS. THIS PRELIMINARY WORK PLAN SHALL INCLUDE:
·A GENERAL CONSTRUCTION SCHEDULE.
·ITEMIZATION AND DISPOSITION OF MATERIALS
·CLEAN UP PLAN
·EQUIPMENT INVENTORY
16. A DETAILED SCHEDULE OF CONSTRUCTION OPERATIONS SHALL BE SUBMITTED TO THE OWNER'S
REPRESENTATIVE IMMEDIATELY UPON RECEIVING THE NOTICE TO PROCEED.
17.ALL EARTHWORK OPERATIONS AND SITE PREPARATION SHALL BE IN ACCORDANCE WITH THE GEOTECHNICAL
INVESTIGATION REPORT BY KELLER, SCHLEICHER & MACWILLIAM ENGINEERING AND TESTING, INC.
18. NOTES SHOWN ON THE DRAWINGS ARE NOT INTENDED TO REPLACE SPECIFICATIONS. SEE WRITTEN
TECHNICAL SPECIFICATIONS FOR REQUIREMENTS IN ADDITION TO THE DRAWING NOTES.
19.CONTRACTOR(S) SHALL HAVE A MINIMUM OF 5 YEARS OF SUCCESSFUL EXPERIENCE WITH MARINE

TURBIDITY BARRIER APPLICATIONS
1. THE CONTRACTOR SHALL FURNISH, INSTALL, AND MAINTAIN TURBIDITY CONTROL
BARRIERS AROUND THE WORK AREA AS NECESSARY. THE PURPOSE OF THE
BARRIERS IS TO PREVENT ANY DEBRIS AND SUSPENDED SEDIMENT FROM
WORK AREA. THE CONTRACTOR HAS THE OPTION TO ENCLOSE THE ENTIRE
WATERSIDE OF THE DISTURBANCE LIMITS, OR TO INSTALL AND MOVE
THE BARRIER IN STAGES. THE CONTRACTOR SHALL MAINTAIN SUCH BARRIERS
AT ALL TIMES IN THE AREAS WHERE WORK IS IN PROGRESS.
2. TURBIDITY BARRIERS SHALL BE TYPE II IN ACCORDANCE WITH FDOT STANDARD
SPECIFICATIONS SECTION 104 AND STATE OF FLORIDA EROSION AND SEDIMENT
SHALL BE ANCHORED AT MAX. 100 FT INTERVALS. PROVIDE A BUOY AT EACH
ANCHOR LOCATION.
3. DURING CONSTRUCTION AND DEMOLITION, THE CONTRACTOR SHALL MONITOR
TURBIDITY LEVELS TO ENSURE THAT THE LOCAL WATER QUALITY STANDARDS ARE
MAINTAINED AND CONSTRUCTION METHODS ARE IN ACCORDANCE WITH THE
PERMITS.
4. NAVIGATION MAY REQUIRE SEGMENTING TURBIDITY BARRIER DURING CONSTRUCTION
OPERATIONS.

EXISTING COVERED AND OPEN DECK STRUCTURES.
2.OPEN DECK AND UTILITIES MAY BE TEMPORARILY DISASSEMBLED AS NECESSARY

REVETMENT NOTES:

2.SMALLER STONES SHALL BE UTILIZED FOR CHINKING TO ACHIEVE UNIFORM
INSTALLATION CONFORMITY TO LINES AND GRADES.
3.ROCK SIZE SHOWN IS BASED ON A MINIMUM DRY UNIT WEIGHT OF 135 PCD.
4.EXISTING GRADE SHOWN AT TIME OF SURVEY.
5.QUANTITY OF EXISTING EXCAVATION AND CONCRETE RUBBLE TO BE REMOVED
WAS ESTIMATED BASED ON EXISTING CONDITIONS SURVEY. CONTRACTOR
SHALL VERIFY ALL QUANTITIES.
6.GEOTEXTILE SHALL HAVE A MINIMUM OVERLAP OF TWO FEET AND BE SECURED

7.TEMPORARY EXCAVATION OF SAND FOR CONSTRUCTION OF PROPOSED WORK
SHALL BE STOCKPILED AND REGRADED INTO EXISTING BEACH PROFILE AFTER
REVETMENT COMPLETION. ANY CONCRETE RUBBLE, ROCK, GEOTEXTILE, OR
OTHER DEBRIS SHALL BE REMOVED FROM THE SITE AND DISPOSED OF BY
CONTRACTOR.
8.TEMPORARILY EXCAVATED SAND MAY BE USED TO FILL AREAS REQUIRED FOR
REVETEMENT INSTALLATION.
9.CONTRACTOR SHALL CONDUCT FILLING, GRADING, AND COMPACTION PER
GEOTECHNICAL REPORT.
10.CONTRACTOR SHALL SUBMIT SHOP DRAWING OF PROPOSED GEOTEXTILE DETAIL
AT TOP AND TOE OF REVETMENT.
11.REMOVE ALL VEGETATION, TREES, EXISTING CONCRETE RIP-RAP AND OTHER
DEBRIS WITHIN REVETMENT CONSTRUCTION LIMITS TO DESIGN GRADE PRIOR TO
INSTALLATION OF GEOTEXTILE AND REVETMENT. DISPOSE OF ALL REMOVED
MATERIALS IN A LEGAL OFFSITE LOCATION UNLESS OTHERWISE DIRECTED BY
OWNER.
PRELIMINARY - 90% SUBMITTAL FOR REVIEW ONLY
