ALL PERSONAL PROPERTY WITHIN THE RIGHT-OF-WAY SHALL BE RELOCATED BY THE PROPERTY OWNER OR IT SHALL BE REMOVED BY THE CONTRACTOR AS NECESSARY TO CONSTRUCT THE PROJECT IN ACCORDANCE WITH THE PLANS. ALL PRIVATE AND PUBLIC PROPERTY AFFECTED BY CONSTRUCTION WORK SHALL BE RESTORED TO A CONDITION EQUAL TO OR BETTER THAN THE EXISTING PRE -CONSTRUCTION CONDITION, UNLESS SPECIFICALLY EXEMPT FROM THE PLANS. 5. SHRUBS THAT CONFLICT WITH THE PROPOSED DESIGN SHALL BE REMOVED. NO TREES ARE TO BE REMOVED WITHOUT APPROVAL FROM THE CITY OF SEBASTIAN PROJECT MANAGER. 6. CAUTION SHALL BE EXERCISED WHILE RELOCATING EXISTING SIGNS TO PREVENT UNNECESSARY DAMAGE TO SIGNS. IF THE SIGNS ARE DAMAGED BEYOND USE, AS DETERMINED BY THE CITY OF SEBASTIAN ENGINEERING INSPECTOR, SIGNS SHALL BE REPLACED BY THE CONTRACTOR AT HIS EXPENSE. ALL SIGNS SHALL REMAIN UNLESS OTHERWISE STATED. 7. EXISTING UTILITY LOCATIONS SHOWN IN THE PLANS ARE ONLY APPROXIMATE. ANY DRAINAGE PROBLEMS EXISTING BEFORE AND DURING CONSTRUCTION SHOULD BE BROUGHT TO THE ATTENTION OF THE CITY OF SEBASTIAN ENGINEERING INSPECTOR IMMEDIATELY. 12. ALL EXISTING DRAINAGE STRUCTURES ARE TO REMAIN UNLESS OTHERWISE SPECIFIED IN THE PLANS. 13. TEMPORARY DRAINAGE SHALL BE PROVIDED DURING CONSTRUCTION TO ELIMINATE ANY FLOODING OF PRIVATE PROPERTY. COST FOR TEMPORARY DRAINAGE SHALL BE INCLUDED UNDER PAY ITEM 102-1. 14. IRRIGATION LINES DAMAGED DURING CONSTRUCTION ARE TO BE CAPPED IMMEDIATELY AND REPORTED TO THE CITY OF SEBASTIAN ENGINEERING INSPECTOR. IT IS THE RESPONSIBILITY OF THE CONTRACTOR TO REPAIR AND/OR REPLACE ALL DAMAGED IRRIGATION COMPONENTS TO THE PRE -CONSTRUCTION CONDITION AT HIS/HERS EXPENSE PRIOR TO THE COMPLETION OF THE PROJECT. COST TO BE INCLUDED IN PAY ITEM 110-1-1. a 15. ANY NGVD-29 MONUMENT WITHIN THE LIMITS OF CONSTRUCTION IS TO BE PROTECTED. IF IN DANGER OF DAMAGE, THE CONTRACTOR SHOULD NOTIFY THE FLORIDA DEPARTMENT OF NATURAL RESOURCES, BUREAU OF SURVEY AND MAPPING, 3900 COMMONWEALTH BLVD., SUITE 309, TALLAHASSEE, FLORIDA 32399. TELEPHONE 904-488-2427 16. ANY PUBLIC LAND CORNER WITHIN THE LIMITS OF CONSTRUCTION IS TO BE PROTECTED. IF A CORNER MONUMENT IS IN DANGER OF BEING DESTROYED AND HAS NOT BEEN PROPERLY REFERENCED, THE CONTRACTOR SHOULD NOTIFY THE ENGINEERING INSPECTOR BY TELEPHONE WITHOUT DELAY. 17. ALL AREAS DISTURBED BY CONSTRUCTION ACTIVITIES SHALL BE SODDED UNLESS OTHERWISE SHOWN IN THE PLANS. 18. ELEVATIONS CALLED OUT ON THE PLAN SHEETS ARE FOR THE EDGE OF PAVEMENT, NOT THE TOP OF THE CURB. 19. SIGNING AND PAVEMENT MARKINGS ARE TO BE REPLACED IN ACCORDANCE WITH MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES, LATEST PUBLISHED EDITION. 20. ALL PROPOSED SIGNS SHALL BE DECORATIVE STYLE DESIGNATED BY THE CITY OF SEBASTIAN. 21. THERMOPLASTIC PAVEMENT MARKINGS SHALL NOT BE INSTALLED UNTIL THE FINAL ASPHALT SURFACE HAS CURED FOR 30 DAYS. TEMPORARY PAINT PAVEMENT MARKINGS SHALL BE INSTALLED DURING CURING PERIOD. 22. THE CONTRACTOR IS RESPONSIBLE FOR ALL COSTS OF ALL TESTINGS BY AN INDEPENDENT GEOTECHNICAL SOILS AND MATERIALS TESTING LABORATORY. THE COST OF ALL TESTINGS REQUIRED BY THESE PLANS AND SPECIFICATIONS SHALL BE INCLUDED WITH THE LUMP SUM BID AND NO ADDITIONAL COMPENSATION SHALL BE MADE BY THE CITY. 23. UNLESS OTHERWISE SHOWN, THE CONTRACTOR SHALL REPLACE ALL PAVING, SODDING, STABILIZED EARTH, LANDSCAPING, DRIVEWAYS, SIDEWALKS, IRRIGATION SYSTEMS, UNDERGROUND PIPING, ETC. LOCATED WITHIN AND OUTSIDE THE CONSTRUCTION LIMIT LINE DISTURBED OR DAMAGED BY THE CONSTRUCTION. THE CONTRACTOR SHALL REPLACE WITH EQUAL MATERIAL OR AS DIRECTED BY THE ENGINEER. 24. COMPLETE RECORD DRAWINGS, CONFORMING TO THE CITY OF SEBASTIAN STANDARDS, SHALL BE COMPILED DURING CONSTRUCTION FOR PERIODIC REVIEW. FINAL RECORD DRAWINGS, CERTIFIED BY A FLORIDA LICENSED SURVEYOR, SHALL BE SUBMITTED TO THE ENGINEER IN DIGITAL FORMAT PRIOR TO FINAL INSPECTION AND ACCEPTANCE ON ANY PART OF THE PROJECT. 25. LOCATIONS AND DIMENSIONS OF EXISTING RIGHT-OF-WAY AND EASEMENTS ARE BASED ON BEST AVAILABLE INFORMATION. THE CONTRACTOR SHALL VERIFY THE LIMITS OF THE RIGHT-OF-WAY AND EASEMENTS IN ORDER TO•AVOID ENCROACHMENTS. 26. THE CONTRACTOR SHALL NOTIFY FIBER OPTIC COMPANIES SEVEN (7) DAYS PRIOR TO ANY CONSTRUCTION. EXTREME CAUTION SHALL BE EXERCISED IN AREAS OF FIBER OPTICS. 27. THE CONTRACTOR SHALL BE RESPONSIBLE FOR REPLACING DAMAGED CURBS, ASPHALT, LANDSCAPING, DRIVEWAYS, AND ANY TEMPORARY REPAIRS OF OPEN CUTS. THESE ITEMS SHALL BE PAID UNDER GENERAL REQUIREMENTS. EROSION CONTROL/WATER QUALITY NOTES: 1. EROSION CONTROL AND SEDIMENTATION CONTROL DEVICES SHALL BE IN PLACE PRIOR TO BEGINNING ANY CONSTRUCTION. THEY SHALL BE INSTALLED, AT A MINIMUM, TO THE LIMITS SHOWN IN THE DRAWING, AS REQUIRED IN THE SPECIFICATIONS AND IN ACCORDANCE WITH ALL REGULATORY AGENCY REQUIREMENTS. THE COST TO THE CONTRACTOR FOR ALL EROSION AND SEDIMENTATION CONTROL IS INCLUDED IN THE LUMP SUM BID AND NO ADDITIONAL COMPENSATION SHALL BE MADE BY THE CITY. 2. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTENANCE OF ALL TEMPORARY EROSION CONTROL FEATURES. 3. THE CONTRACTOR IS TO PROVIDE EROSION/SEDIMENTATION CONTROL TO PREVENT SILTATION OF ADJACENT PROPERTY, STREETS, STORM SEWERS, AND WATER WAYS. IN ADDITION, THE CONTRACTOR SHALL PLACE STRAW, MULCH, OR OTHER SUITABLE MATERIAL ON GROUND ON AREAS WHERE CONSTRUCTION -RELATED TRAFFIC IS TO ENTER AND EXIT THE SITE. IF, IN THE OPINION OF THE ENGINEER AND/OR LOCAL AUTHORITIES, EXCESSIVE QUANTITIES OF EARTH ARE TRANSPORTED OFF-SITE EITHER BY NATURAL DRAINAGE OR VEHICULAR TRAFFIC, THE CONTRACTOR IS TO REMOVE AND CLEAN SAID EARTH TO THE SATISFACTION OF THE ENGINEER AND/OR AUTHORITIES. AN EROSION CONTROL BARRIER SHALL BE ESTABLISHED AS SITE CLEARING PROCEEDS AND MAINTAINED UNTIL SUCH TIME AS VEGETATION OR OTHER CONTROLS BECOME EFFECTIVE. 4. EXISTING DRAINAGE PATTERNS AND CAPACITIES ARE TO BE PROTECTED AND MAINTAINED DURING CONSTRUCTION TO ELIMINATE FLOODING AND PONDING WATER ON PRIVATE PROPERTY. THE CONTRACTOR IS TO PROVIDE TEMPORARY DRAINAGE MEASURES AS REQUIRED TO MEET THIS REQUIREMENT. 5. THE CONTRACTOR SHALL OBTAIN ALL REQUIRED NPDES PERMITS FOR CONSTRUCTION WORK AND SHALL COMPLY WITH THE CITY'S NPDES ORDINANCE ON ILLICIT DISCHARGE. MINIMUM STANDARDS: 1. SEDIMENT BASINS AND TRAPS, PERIMETER DIKES, SEDIMENT BARRIERS, AND OTHER MEASURES INTENDED TO TRAP SEDIMENT SHALL BE CONSTRUCTED AS A FIRST STEP IN LAND -DISTURBING ACTIVITY AND SHALL BE MADE FUNCTIONAL BEFORE UPSLOPE LAND DISTURBANCE TAKES PLACE. 2. SEDIMENT CONTROL MEASURES ARE TO BE ADJUSTED TO MEET FIELD CONDITIONS AT THE TIME OF CONSTRUCTION AND BE CONSTRUCTED PRIOR TO GRADING OR DISTURBANCE OF EXISTING SURFACE MATERIAL ON BALANCE OF SITE. PERIMETER SEDIMENT BARRIERS SHALL BE CONSTRUCTED TO PREVENT SEDIMENT OR TRASH FROM FLOWING OR FLOATING ONTO ADJACENT PROPERTIES. 3. PERMANENT OR TEMPORARY SOIL STABILIZATION SHALL BE APPLIED TO DENUDED AREAS WITHIN SEVEN DAYS AFTER FINAL GRADE IS REACHED ON ANY PORTION OF THE SITE. TEMPORARY SOIL STABILIZATION SHALL BE APPLIED WITHIN SEVEN DAYS TO DENUDED AREAS THAT MAY NOT BE AT FINAL GRADE BUT WILL REMAIN UNDISTURBED FOR LONGER THAN 30 DAYS. PERMANENT STABILIZATION SHALL BE APPLIED TO AREAS THAT ARE TO BE LEFT UNDISTURBED FOR MORE THAN ONE YEAR. 4. SURFACE RUNOFF FROM DISTURBED AREAS THAT IS COMPRISED OF FLOW FROM DRAINAGE AREAS GREATER THAN OR EQUAL TO THREE ACRES SHALL BE CONTROLLED BY A SEDIMENT BASIN. THE SEDIMENT BASIN SHALL BE DESIGNED AND CONSTRUCTED TO ACCOMODATE THE ANTICIPATED SEDIMENT LOADING FROM THE LAND -DISTURBING ACTIVITY. THE OUTFALL DEVICE OR SYSTEM DESIGN SHALL TAKE INTO ACCOUNT THE TOTAL DRAINAGE AREA FLOWING THROUGH THE DISTURBED AREA TO BE SERVED BY THE BASIN. 5. AFTER SIGNIFICANT RAINFALL, SEDIMENT CONTROL STRUCTURES WILL BE INSPECTED FOR INTEGRITY. DAMAGED DEVICES SHALL BE CORRECTED BY THE CONTRACTOR IMMEDIATELY. 6. CONCENTRATED RUNOFF SHALL NOT FLOW DOWN CUT OR FILL SLOPES UNLESS CONTAINED WITHIN AN ADEQUATE TEMPORARY OR PERMANENT CHANNEL, FLUME, OR SLOPE DRAIN STRUCTURE. 7. WHENEVER WATER SEEPS FROM A SLOPE FACE, ADEQUATE DRAINAGE OR OTHER PROTECTION SHALL BE PROVIDED. 8. SEDIMENT WILL BE PREVENTED FROM ENTERING ANY STORM DRAIN SYSTEM, DITCH, OR CHANNEL. ALL STORMWATER INLETS THAT ARE MADE OPERABLE DURING CONSTRUCTION SHALL BE PROTECTED SO THAT SEDIMENT -LADEN WATER CANNOT ENTER THE CONVEYANCE SYSTEM WITHOUT FIRST BEING FILTERED OR OTHERWISE TREATED TO REMOVE SEDIMENT. TRAFFIC CONTROL NOTES: 1. THE CONTRACTOR SHALL ADHERE TO THE REQUIREMENTS SET FORTH IN THE MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES (MUTCD) AND FDOT DESIGN STANDARDS, LATEST PUBLISHED EDITION, AT ALL TIMES. 2. ALL TRAFFIC CONTROL DEVICES (TEMPORARY SIGNS, PAVEMENT MARKINGS, BARRIER WALLS, AND ETC.) REQUIRED DURING CONSTRUCTION PHASE SHALL BE APPROVED BY THE PROJECT MANAGER PRIOR TO COMMENCEMENT OF CONSTRUCTION AND WILL BE MAINTAINED IN ACCORDANCE WITH MUTCD (DEFER TO FDOT DESIGN STANDARDS INDEX 600 IF CASE IS NOT COVERED IN MUTCD). FDOT SIGN STANDARDS INDEX 600 SHALL BE USED IN CONJUNCTION WITH ALL OTHER INDICES SPECIFICALLY MENTIONED. 3. ALL EXISTING SIGNS WHICH CONFLICT WITH THE TRAFFIC CONTROL PLAN SHALL BE REMOVED OR TEMPORARY RELOCATED IF NECESSARY. 4. THE COST OF REMOVING AND/OR REPLACING SIGNS SHALL BE PAID FOR UNDER THE CONTRACT UNIT PRICE OR PAY ITEM 102-1. 5. ALL SIGNS 48" X 48" OR LARGER SHALL BE MAINTAINED ON TWO (2) BREAKAWAY SUPPORTS OR YIELDING POSTS AS SHOWN IN FIGURE 6-1 OF MUTCD. THE CONTRACTOR SHALL USE THE MUTCD TO DETERMINE THE APPROPRIATE PLACEMENT AND ELEVATION OF CONSTRUCTION SIGNS. REFLECTIVITY SHALL ADHERE TO THE STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION (2010). 6. VEHICULAR ACCESS TO RESIDENCES AND BUSINESSES WILL BE MAINTAINED AT ALL TIMES. 7. MINIMUM LANE WIDTH WILL BE TEN (10) FEET. 8. PEDESTRIAN ROUTES MUST BE IDENTIFIED AND MAINTAINED THROUGHOUT CONSTRUCTION. 9. WORK HOURS ARE FROM 7:00 AM TO 6:00 PM MONDAY THRU FRIDAY. WORK WILL NOT BE ALLOWED ON WEEKENDS OR HOLIDAYS OBSERVED BY THE CITY OF SEBASTIAN. 10. ALL ACTIVITIES REQUIRING LANE CLOSURES SHALL BE BROUGHT TO THE ATTENTION OF THE CITY OF SEBASTIAN PROJECT MANAGER. LANE SHALL ONLY BE CLOSED DURING ACTIVE WORK PERIODS. 11. TRAFFIC CONDITIONS, ACCIDENTS, AND OTHER UNFORESEEN EMERGENCY CONDITIONS SHALL BE BROUGHT TO THE ATTENTION OF THE CITY OF SEBASTIAN PROJECT MANAGER. 12. THE EXISTING POSTED SPEED LIMIT WILL BE MAINTAINED DURING CONSTRUCTION. 13. THE CONTRACTOR SHALL SUBMIT A MAINTENANCE OF TRAFFIC PLAN SIGNED AND SEALED BY PROFESSIONAL ENGINEER FOR CONSTRUCTION WORK ON CITY STREETS. CONSTRUCTION IN STREETS. ROAD RIGHT-OF-WAY. AND PARKING AREAS: 1. OPEN ROAD CUTS REQUIRE PRIOR APPROVAL OF THE CITY, COUNTY, STATE, OR ANY OTHER AGENCY WHICH MAY HAVE JURISDICTION. 2. CONSTRUCTION, MATERIALS, AND WORKMANSHIP ARE TO BE IN ACCORDANCE WITH FLORIDA DEPARTMENT OF TRANSPORTATION SPECIFICATIONS AND STANDARDS. 3. AREAS IN EXISTING RIGHT-OF-WAYS DISTURBED BY CONSTRUCTION SHALL RECEIVE GRASS, MULCH, OR SOLID SOIL, AS STATED ON THE PLANS. 4.
