Pursuant to the above captioned contract and in compliance of same, we the undersigned do hereby certify
that this project is complete.
Issuance of this certificate releases the contractor from any further responsibility as stipulated in the contract
as it relates to bond performance, payment of subcontractors, release of liens, maintenance and indemnity. All
contractual obligations as to warranty, insurance, indemnification and patents, shall remain in full effect for the
contractually stipulated period of time. In no way does the issuing of this certificate release the Contractor from
all legal obligations as stipulated in Chapter 713 of the Florida Statutes.
Therefore, having reviewed the contract and inspected the work, the undersigned to hereby accept the
segment as being complete.

Project Identification: Tiki Bar Stormwater Improvements
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
As per our coordination, the City of Sebastian is looking to address existing drainage problems along
Indian River Drive within the vicinity of the Tiki Bar. To properly address the existing undersized, non -
permitted collection system and significantly reduce direct discharge to the Indian River Lagoon, it is
proposed to provide an inlet near the location of the existing inlet located east of the roadway, and add a
second stormwater inlet on the west side of the road, which will directly connect to the existing stormwater
collection system. This existing stormwater collection system is connected to an existing stormwater
pollution prevention baffle box thereby reducing pollutant impacts to the lagoon.
Based on the above, we are proposing to provide the necessary plans and hydraulic analysis for the existing
collection system required for FDOT permitting purposes. Plans will reflect grading the existing
surrounding areas to ensure positive drainage. Elevations will not be provided as it is assumed grading by
percent minimums will suffice the contractor's needs.
Deliverables: Design plans and details, hydraulic analysis, and FDOT permitting.
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA 16 will be performed for a fee as
described below:
