Issuance of this certificate releases the contractor from any further responsibility as stipulated in the contract
as it relates to bond performance, payment of subcontractors, release of liens, maintenance and indemnity. All
contractual obligations as to warranty, insurance, indemnification and patents, shall remain in full effect for the
contractually stipulated period of time. In no way does the issuing of this certificate release the Contractor from
all legal obligations as stipulated in Chapter 713 of the Florida Statutes.

General Description
Schulke, Bittle & Stoddard, LLC well provide conceptual p!anm:rg, and final eng:reenng desygn for the
new ADA accessible pier and dock at Sebastian Yacht Club's south pier.
Phase I - Canceatuat Planning and Desian Develooment
1.0 Preliminary Engineering
1.1 Boundary TopographOnformabonal Survey: Provide survey in ACAD format Survey will include
topographyibathymetry, MHWL, existing docks and piers, location of Intracoastal waterway, and
lies to south property line.
12 Geotechnical investigation
a. Perform one (1) portable Dynamic Cone Penetrometer test in the water, each to a depth of
10 feet if possible, near the proposed structure. If caprock is encountered, penetration
through it is not possible.
b. Perform one (1) soil boring to a depth of 20 feet below existing grade on the shore, close to
the proposed structure to determine the subsurface conditions at a deeper depth than the
portable Dynamic Cone Penetrometer tests, in accordance with ASTM Standards described
in ASTM 01586, known as the Standard Penetration Test (SPT). If caprock is encountered.

c. Provide a report covering the investigation including the boring logs illustrating the soil
classification, thickness of each layer, number of blows per foot (N value), groundwater
depth. and recommendations for site preparation with allowable bearing rapacity of the soil

16 Conceptual Plan Preparation:
a. Prepare a conceptual development plan of the new dock/pier, incorporating the survey
and existing site conditions. The plan will include existing and proposed dock/pier
locations, and other proposed on -site improvements.

additional infrastructure improvements and determine necessary construction required.

2.0 Final Engineering
2.1 Final Construction Plans. Based on the approved Conceptual plan, formal engineering plans and
site plan application will be prepared in accordance with the City of Sebastian's Lard
Development Regulations, FDEP, and ACOE requirements. This submittal will include the
following required elements.
a An engineering Me development plan depicting in more detail, features as developed on
the conceptual plan, incorporating all existing information readily available, including
surveys, record drawings, aerials, maps and documents, etc., and on -site observations.

dimensional lay -out, grading access, appropriate details and cross sections of structures
and the site, site data I area calculations, etc

b. Structural analysis and design of dock and walkway structure.
C. Prepare location plan, dock layout, site plan, structural framing plan, and cross sections
as required by FDEP, ACOE, and COS Building Department.
2.2 Surveyor. Prepare legal description/sketch (if required) for new and/or revised submerged land
lease.
2.3 Environmental Consultant: Prepare environmental impact assessment and public interest
statement in support of FDEP/ACOE submittals, including:
a. An Environmental Impact Report will be prepared to support the FDEP permitting and address
impacts to any regulated habitats. Avoidance and minimization of project impacts will be addressed.
The impact report will provide the quantification of the impacts to resources through the use of the
functional assessment methodology. Preparation of the ERP package including providing guidance
on dredge/fill sketches.
b. Prepare submittal to the USACOE to address the structure permitting in waters of the US. Submittal
includes the joint ERP, a justification statement for the activity, avoidance and minimization
statement and alternative action discussion
3.0 Permitting
3.1 Construction plans, engineering calculations and reports, and supporting documentation will be
submitted to the appropriate agencies for the following construction permits:
a. FDEP ERP
b. ACOE General Permit
C. City of Sebastian Building Department — Plan Review
d. FDEP NOI
3.2 Follow up with permitting agencies via telephone, e-mail or written correspondence. Review and
address and review comments and revise the plans accordingly. Resubmit as required.
3.3 Upon issuance of the above permits, the site plan shall be released and construction may
commence with site work.
Phase III — Construction Contract Procurement. Inspections and Construction Administration
4.0 Contract Administration and Inspections
4.1 Bid Documents: Upon site plan approval by Planning and Zoning, final engineering plans and
specifications will be prepared and provided to client for bidding purposes. Specifications will be
included on the construction plans.
4.2 Bidding and Procurement documents: assist COS in the preparation of bidding and
procurement information, including:
A. Description of time, place and conditions of bidding
B. Bidding and Proposal forms
D. Form of Agreement — between owner and contractor
E. Conditions of the Contract — standard, supplementary, and other special conditions
Note: City Standard bid doc forms will be used (provided by City). SBS will assist City with
development of project specific information provided on the forms.
4.3 Construction Procurement services: assist COS in construction procurement
A. Assist in preparation of bid sets for competitive bidding
B. Attend Pre -bid meeting, Respond to RFI's sent by bidders, and prepare
Addendums, if necessary
D. Bid validation
C. Prepare Notice of award and assist in finalizing construction contract
4.4 Contract Administration and Inspections: The engineer will coordinate the responsibility for
document review, meeting attendance, and inspections with civil engineer and other consultants.
1. Coordinate and attend pre -construction meetings
2. Attend progress meetings with contractor (bi-weekly)
3. Review shop drawings

4. Review contractors pay requests
5. Periodic inspections of the site work (2+-/ week)
6. Substantial completion inspection / punch list (2)
7. Review as-builts for adequacy and completeness
8. Final inspection / final completion determination
9. Prepare final certification submittals for site permits
10.Changes to Work: Oversee changes to work, field directives, minor plan changes,
assist with preparation of construction change orders.
4.5 Final certifications are required by condition of issuance of most permits, and will be prepared
based on satisfactory review of surveyed as-builts (by others) and construction inspection by the
engineer.
A. As -built plans will be prepared by the contractor and the contractor's surveyor in
accordance with the construction specifications, and/or additional information provided
by the engineer's inspector.
B. The field information provided by surveyor and contractor is not included in the
proposal and will be included in the construction specifications as a requirement of the
contractor to provide surveyed as-builts in accordance with applicable agency
requirements and permit conditions.

Invoices shall be submitted monthly, and the invoice amount shall be in proportion to the percentage of
the Scope of Work complete. All payments must be received by the Engineer within 30 days of the date
of an invoice. Any payments received after said date will be subject to a late interest fee of one and one
half percent (1- 1/2%) per month. The client shall be responsible for all attorney's fees, court costs, and

related expenses incurred in the event that payment is not timely made and proceedings are brought by
the Engineer to collect sums owed.

Agenda Item Title: Yacht Club Pier Engineering
Recommendation: Approve Work Authorization #CSA-5, a Continuing Services
Agreement, with Schulke, Bittle & Stoddard, LLC to design a pier
at the Yacht Club in the amount of $30,950. Authorize the City
Manager to execute appropriate documents.
Background: This project involves the design of a pier at the Yacht Club that
allows guests with mobility issues a more safe and convenient
access to boats. The envisioned 8'x20' pier will be built
approximately 20" above the average high -tide water level and
features a ramp that provides accessibility for those using walkers
and wheelchairs.
