For the above referenced project, take note of the following revisions, additions, deletions, clarifications, and
questions/answers etc. relative to the ITB, which in accordance with the Contract Documents shall become a
part of and have precedence over anything shown or described otherwise.
Revisions
The following responses are from questions posed during the mandatory site visit held on April 29, 2021.
ITB Addendum #1:
As it stands there was one scope of work addendum (see the attached and descriptions below):
-Line item 2a: shutter locations and count adjusted to actual affected openings.
Contractor Request #1:
Participating contractors made the request, in an attempt to provide the best bid and price development that they are allowed
to make follow-up contact with homeowners and additional site visits to each project.

All work to be performed in a workmanlike manner, in accordance with the Sebastian-CDBG Work Write Up Specifications, Florida Building Code, local
codes, and manufacturer’s specifications. The contractor shall be responsible for the repairs and/or reinstallation of mater ials/equipment/fixtures
damaged or removed due to any work item contained herein. Contractors shall properly dispose of all fixtures, materials and other items removed from
the dwelling unless otherwise specified herein. Bid will be accepted on the TOTAL BID submitted, and all items must be cost itemized in the space
provided or the bid will be rejected.
This house may or may not require the homeowner to vacate the premises during the construction period. The period for Construction shall be 90
Calendar days (Mon-Sun) from the date of contract acceptance, execution and Established Notice to Proceed date.
This document may not be altered in any manner. The scope of work set forth in this document may not be changed without the written consent of
Guardian CRM, Inc. under direct authorization from the City of Sebastian and/or its official representatives.
I hereby certify that I am licensed by the State of Florida, Department of Business and Professional Regulation, and that I am eligible to participate in the
City of Sebastian CDBG program. I also agree that change orders above the original contract amount shall only be paid for with CDBG funds to correct
documented code violations or to meet Section 8 Housing Quality Standards. Change orders must be approv ed by the homeowner or his/her
representative, the contractor, and local government CDBG Program Administrator prior to any initiation of work based on that change order.

SCOPE OF WORK
(“SOW”)

NOTICE TO BIDDING GENERAL CONTRACTORS
No contractor shall, without prior written approval from the Housing Specialist or CDBG Program Administrator, deviate from any
product recommendations listed within this Scope of Work (SOW). A notice of “or equal” exchange shall be provided to the Inspector
within forty-eight (48) hours of the proposed “or equal” substitution. This notice shall contain:
1) The names of both the SOW recommended product and proposed substitute product specification
2) Comparable manufacturer specifications list, included but not limited to price, warranty information, consumer review reports
3) Any additional information requested by the Sebastian CDBG Program Administrator or its agents.
4) All bids to include the repair/repaint to match existing, all damaged (interior/exterior surfaces, i.e., plaster, stucco, Tile, or any other
material), walls, ceilings, ECT… affected by construction.
5) The contractor will provide all material and shall be responsible for covering all homeowner belongings, including flooring that cannot
be moved during rehabilitation: this is not the owner’s responsibility: Daily clean-up within and around the home is required.
6) All measurements and material will be the responsibility of the contracting firm: substituting items to upgrade cabinet heights is
prohibited.
7) All change orders must comply with the project budget, included allotted contingency, and be approved by the Sebastian CDBG
Program Administrator or Project Manager prior to any change order work can begin.
8) Profit and overhead shall only be included in the specified line item. Other line items shall only include material, and labor cost.
9) Where the prime contractor of record elects to pull “combo” permits, i.e. where the roof, HVAC, or other such discipline permits are
attached to the primary construction permit and not pulled on an individual discipline basis, payment draws must be billed on two
separate draw requests: the first half at 50% project completion and the final 50% at the final closeout draw. The final 50% must be
accomplished with the final permit showing that all combo disciplines have passed final building department inspection.
Failure to provide any required notification will result in a violation of the terms and conditions of the SOW and Work Rehabilitation
Contract and a loss of payment on the substituted product if it is not replaced with the SOW specified product. Approval will NOT be
granted under any circumstance on or after the fact basis.

1. General Construction and Operational:
1a. Profit and Overhead:
➢ The contractor shall include all profit and overhead in this line item: Profit and overhead shall be inclusive of all costs not
directly related to material and labor (i.e. travel, supervision, sub/supplier meetings, etc.)
NOTE 1: The contractor shall include all profit and overhead on its specified line item under the Applicant (Owner) and
Contractor Contract page. Profit and overhead shall be inclusive for all costs not causally related to material and labor (i.e.,
travel, supervision, sub/supplier meetings, etc.). Do not include any profit or overhead costs in each individual item.
NOTE 2: The Profit and overhead can only be billed in two draw requests; the first half at 50% completion and the final 50%
at the final closeout draw.
1b. Manuals and Specifications:
➢ The contractor shall supply, at the time of the final CDBG Program inspection, all manufacturer manuals and specification
booklets/packets for all supplied and installed products listed within the Scope of Work. Failure to do so will result in a failed
final CDBG Program Inspection.
o To Include all applicable Plans, Drawings, and Permits, & approved building department docs $ N/A
1c. General Construction and Operational:
➢ The contractor shall be responsible for and provide all applications, surveys, permits, plans, engineering, or other required
federal, state, and local documentation.
NOTE 1: No profit and/or overhead shall be included in this line item.
o To Include all applicable Plans, Fees, Engineering, Drawings, Permitting, etc., $

2. Exterior Rehabilitation: Windows and Doors:
2a. Window and Door Shutters:
➢ Prepare the surface around all applicable exterior windows as listed below (Front-3 Openings; Right-3 Openings; Rear-2
Openings; Left-4 Openings) to accept new Miami Dade or Florida product approved or equal hurricane impact rated accordion,
panel, or roll-down shutter units as indicated below. All permanent set components including but not limited to screws,
embedded anchor bolts, threaded cones, metal shields, headers and sills (tracks) must be weatherized and/or protected
against corrosions and integrity contaminations.
New accordion shutter units shall be a minimum 22 gauge corrugated, galvanized aluminum, with a mill finish and include all
required attachments for proper and code compliant mounting. All products must meet or exceeds wind-borne debris impacts
and wind load tests of Miami-Dade Building Code standard and/or Florida Building Code Standard.
Shutters shall be mounted in a manner to ensure proper connection, coverage, and protection of each window opening and
be installed to meet the manufacturer’s specifications. Installation shall also include all required permits, additional anchoring,
or framing to allow for complete and compliant installation. Any damage caused by the installation process should be
prepared and patched to match exterior walls. (Unit size & dimensions to be determined by contracting firm).
NOTE 1: Any minor repairs and/or replacements to window encasements, exterior sills, etc…shall be included in this bid.
NOTE 2: Contractor shall submit Dade County or equal Florida product approval stamped by the building Department of
jurisdiction, with this projects close-out package.
NOTE 3: All shutter units must meet the FBC HVHZ for Indian River County and/or minimum ASTM 1986-1996.

2b. Entry/Exit Doors:
➢ Remove the existing primary entry/exit doors (1 unit at the front elevation and 1 unit at the rear elevation) and install new,
hurricane impact rated, energy efficient (for the southern region) door in the listed location(s). The installation shall include
jambs, preparation a sufficient door buck to accommodate the new door, wood casing, and install a new white pre-hung
fiberglass Hurricane Impact rated, energy efficient door. (Door size & dimensions to be determined by contracting firm);
Installation to include all required permits, additional anchoring, framing, hardware, components, and tap cons (TAP CONS
MUST BE WHITE) to ensure proper and complete installation to current Florida Building Code. Door installation shall include
keyed (alike) entry lockset with lever handle (both sides): “Schlage”, Flair F51 and matching deadbolt or approved equal,
deadbolt keyed one side to the lock set, preinstalled peephole, vinyl bubble weather-stripping, and aluminum threshold.
NOTE 1: Where a landing is required according to code. In absence of a landing, Contractor shall construct a concrete landing
in front of the door as required by code, (or where a landing exists, contractor shall re-construct/modify the existing landing in
order to meet code).
NOTE 2: Contractor shall submit Dade County or Florida Building Code hurricane impact product approval stamped by the
building Department of jurisdiction, with this projects close-out package. Door assembly shall meet Dade County or Florida
Building Code product approval without using interior slide bolts.
o Front-Primary Entry/Exit Door (1- 1/4 Light Unit with impact sidelight) $

3. Interior Rehabilitation: Flooring & Doors
3a. Interior Flooring (Vinyl):
➢ Remove and properly dispose of the existing floor covering and base molding in the areas of the home listed below. Make
all appropriate modifications to prepare the underlying floor (including the application of a surface prep primer) to accept
new vinyl plank flooring and baseboard.
Scrape and clean the floor to ensure that it is free of all debris and protrusions. Repair (with like code/compliant materials)
and patch all cracks and uneven sub-floor leveling in all affected areas to ensure a smooth uniform, surface.
Install new waterproof, low gloss, stain resistant, textured, straight edge, medium width, indoor vinyl plank flooring and new
3-1/4” white (pre-painted) baseboard in the areas listed below.
The vinyl flooring shall be a minimum 2.0mm in thickness, with a minimum 2.8mil wear layer or equivalent in quality. If not
self-adhering, where required, product should be installed with manufacturer approved mastic and/or underlayment. Vinyl
planks shall be “Style Selections”, “Peel & Stick” or equal, and contain a no-wax pre-finish.
Installation to include threshold strips at all exposed joints between rooms and/or at door openings. Provide shoe molding
and paint to match other finishes where applicable. Owner will select “standard solid and/or wood” color and pattern from
contractor’s supply samples.
