HomeMy WebLinkAboutCSA #8 Concha Water Control Structure InspectionDocusign Envelope ID: A6664113-A9BE4947-6502-D7737124F9C7 CITY OF SEBASTIAN, FLORIDA WORK AUTHORIZATION FORM CONSULTANT SERVICE AGREEMENT (CSA) Kimley-Horn and Associates, Inc. 200 Central Avenue, Suite 600 St. Petersburg, FL 33701 Work Authorization No: CSA - #8 Protect Title: City of Sebastian Concha Water Control Structure Inspection IT IS AGREED, on this 21" day of August, 2024, to undertake the following work in accordance with the provisions of the Master Agreement entitled NON-EXCLUSIVE ENGINEERING CONSULTING SERVICES AGREEMENT dated FEBRUARY 22, 2021 between Kimley-Horn and Associates, Inc. (Consultant) and the City of Sebastian (City). Description of Assignment The CITY has requested the CONSULTANT to conduct an Inspection (as defined below) and infrastructure assessment of the Concha Water Control Structure, owned and operated by the City of Sebastian (see Figure 1). The condition assessment report findings shall be used to assist the CITY in identifying and prioritizing repair:. The CONSULTANT shall research structure history including prior inspection reports, as -built plans, conduct Inspections, and prepare Inspection reports in support of the Inspection. These services shall be provided under professional supervision and in accordance with the provisions contained herein. The CONSULTANT Inspection team shall consist of a registered professional engineer, specialized inspectors, and certified divers, qualified by experience in the design, construction, and operation of structures comparable to those being inspected. Figure 1: Location Map The work/project will include Task 1— Kickoff Meeting: Description: Once a Notice to Proceed is issued, the CONSULTANT will schedule a kickoff meeting with the CITY. The focus of the meeting will be to outline the goals and objectives of the project, review and discuss condition rating definitions, review the schedule, flow of information, and review the nature of the deliverables. The meeting will take place as a web -based meeting. 'Deliverables: Attendance at Kickoff meeting and preparation of meeting highlights, agenda, sign in sheet and supporting presentation slides — submitted electronically via an Adoben4 portable document format file (PDF). Task 2 — Data Collection, Review, and Field Inspections: Description: The CITY shall provide the CONSULTANT with relevant as-builts, photos, any previous inspection studies, information Page 1 of 4 Docusign Envelope ID: A6664113-A9BEA947-6502-D7737124F9C7 CITY OF SEBASTIAN, FLORIDA WORK AUTHORIZATION FORM CONSULTANT SERVICE AGREEMENT (CSA) from maintenance crews, and surrey information for the Concha dam. The CONSULTANT will review the provided information and provide a summary of the data collected and identify requests for additional information (if so warranted). The results will be summarized in a technical memorandum. This task also includes effort the CONSULTANT to Iialse with CITY maintenance crews to discuss standard operations and any known deficiencies. The CONSULTANT is proposing to use a visual condition assessment similar to the one provided below: (Note that this is subject to revision via the kickoff meeting and/or direction from CITY staff prior to the CONSULTANT performing the Inspection): Condition Codes Description 1 — Excellent Fully operable, well maintained, and consistent with current standards. Little wear shown and no further action required. Sound and well maintained but may be showing slight signs of early wear. Delivering 2—Good full efficiency with little or no performance deterioration. Only minor renewal or rehabilitation may be needed In the near -term. Functionally sound and acceptable and showing normal signs ofwear. May have minor 3 - Moderate (within 3-5 years of failures or diminished efficiency with some performance deterioration and Increase in failure) maintenance cost. Moderate renewal or rehabilitation needed in near terms. Functions but requires a high level of maintenance to remain operational. Shows 4 - Poor (within 1-3 years of failure) abnormal wear and is likely to use significant performance deterioration in the near - term. Rq Jacement or major rehabilitation needed in the near -term. 5 - Very Poor (immediate to 1 year for I Effective 11% exceeded and/or excessive maintenance cost incurred. A high risk of failure) breakdown or Imminent failure with serious impact on performance. No additional life expectancy with immediate replacement needed The CONSULTANT will take photos and/or videos of all components Inspected associated with the Civil, Structural and Underwater assets of the Concha Water Control Structure. Subdiscipline Inspections are provided in the following. Civil Inspection: Consultant shall perform visual Inspections of each structure's assets including the fencing and barbed wire, structure signage, site accessibility, vegetation, and pipes. Erosion control measures along the embankment will be inspected as visible from the topside. Structural Inspection: Consultant shall perform visual Inspections of the structure's assets including concrete caps, steel sheet piling, protective coatings, and concrete weir. Visual access (this is understood to require no specialized equipment such as lifts, climbing harnesses, etc., and what can be accessed from the shore adjacent to the structure) will be limited to walking the structure. The structural Inspection will include an identification of signs of deterioration, such as corrosion, loss of section, missing or damaged components, concrete spelling, expansion joints, exposed reinforcement, structural damage, cracks, or other visual Indication of compromised structural integrity. Underwater Inspection: The CONSULTANT shall be responsible for conducting and reporting on the structure underwater Inspections for the Concha dam. The CONSULTANT shall provide all necessary equipment to perform the task in this statement of work at no additional charge to the CITY. The CONSULTANT shall perform a visual and tactile Inspection of the structure's submerged components that may include concrete surface, shoaling and scour areas, bulkheads, weirtweir crest, and embankment erosion protection. Deliverables: Technical memorandum summarizing data collection result — submitted electronically via an AdobeTM portable document format file (PDF). Task 3 - Draft Inspection Report: Description: The CONSULTANT shall prepare a Draft Inspection Report Including observations, visual condition ratings, and recommendations from each discipline (Civil, Structural, and Underwater). This report shall be based on a detailed, systematic technical Inspection and evaluation of the structure as it relates to its'safety, stability, and operational adequacy. Operational adequacy relates to the Inspection, operation, and evaluation of those components whose failure, or failure to operate property, would impair the operational capability and/or usability of the structure. The Inspection report shall contain the structure description, history, previous Inspection summary (if available), Inspection results, Inspection conclusions, recommendations and cost estimate (Class Level V) for deficiencies, photographs of findings, and supporting documentation. In the Inspection reports, the CONSULTANT shall Identify the location of deficiencies on a site sketch for the Page 2 of 4 Docusign Envelope ID: A6664113-A9BE-4947-8502-D7737124F9C7 CITY OF SEBASTIAN, FLORIDA WORK AUTHORIZATION FORM CONSULTANT SERVICE AGREEMENT (CSA) structure. The draft report will be submitted as individual electronic file in PDF format and include all supporting photographstvideos taken during the Inspection. The CONSULTANT shall disclose methodology, procedures, and results. The CITY shall review the draft report and provide comments for the CONSULTANT to address. These comments will be given at a web -based meeting between the CITY and the CONSULTANT. Deliverables: The CONSULTANT will provide the following: 1) Draft Inspection Report Including all supporting photosNideos. The draft report will be submitted electronically As an AdobeTM portable document format file (PDF). All supporting photosNideos will be submitted electronically segmented by sub -discipline. 2) Attendance at review meeting — including preparation of agenda sign -in sheet and meeting highlights — submitted electronically via an AdobeTM portable document format file (PDF). Task 4— Final Inspection Report: Description: The CONSULTANT will address CITY comments and prepare a Final Inspection Report The report will be signed and sealed by the respective Florida Professional Engineers that performed the Civil, Structural and Underwater Inspections. Dellverables: Final Inspection Report - submitted electronically via an AdobeTM portable document format file (PDF). All supporting photosNideos will be submitted electronically segmented by sub -discipline. Assumptions: • The CITY is responsible for making key staff available during the delivery of this project • Deliverables will be issued as a draft for review by the CITY. The CITY has an estimated 14-day time frame for comments. After receiving the comments from the CITY, the CONSULTANT will reconcile the comments/edits into a final version. The CONSULTANT will address one round of comments. • The CITY shall provide access to the site and accompany the CONSULTANT where access is limited. The CONSULTANT shall coordinate directly with CITY staff regarding scheduling logistics for access. • The CITY shall provide notes or verbal summary of any known deficiencies at the structure location that degrades the component's performance, alters operational procedures, and/or Increases its maintenance requirements. • All Inspections will be based on accessible field observations and do not Include any fortes of physical (intrusive or destructive) testing (inspections"). • The CONSULTANTS services shall be provided under the supervision of a registered professional engineer in the state of Florida. The CONSULTANT certifies compliance when signing and sealing the final Inspection report. It should be noted that there are elements in the schedule in which the CONSULTANT has no direct control such as review time by the CITY and quality of as-builts provided to CONSULTANT. The project schedule will be adjusted to reflect any delay in these items. 1. Preliminary Planning There is no preliminary planning required for this project. 2. Permitting There is no permitting required for this project 3. Contract Administration There is no contract administration required for this project. 4. Deliverables Task 1— KIck-off Meeting Kick-off Meeting Attendance, Agenda, and Presentation Slides. Task 2— Date Collection, Review and Field Inspection Data Collection Technical Memorandum. Schedule Following the Notice to Proceed the CONSULTANT shall commence and complete tasks in accordance with the following schedule. Task Weeks from NTP Task 1- IOdcoff Meeting 2 weeks Task 2- Data Collection, Review, and Field Inspections 2 Months Task 3- Draft Inspection Report 3 Months J Task 4-Final Inspection Report 4 Months S. Project Budget The CITY agrees to pay the CONSULTANT for work performed on a lump sum price basis in accordance with the following Project Budget. Task Cost Task 1- IGdcoff Meeting $1,675.00 Task 2- Collection, Review and Field Inspection $24,800.00 Task 3-Draft Inspection Report $23,015.00 Task 4- Final Inspection Report I $3,720.00 TOTAL $53,210.00 Basic of Compensation/Period of Services: The above referenced description for CSA #21-01-KHA-01 will be performed for a fee in the amount of $53,210.
