Mr. Sabo

Congratulations on a job well done. The Airport License will be forwarded to you by regular mail. A copy of
the following documents regarding your airport are attached:
1. Airport License Conditions;
2. Airport Inspection Record; and
3. Airport Sketch

any conditions shall be displayed at the office or place of business of the airport manager.
It is the responsibility of the airport owner/lessee to ensure that airport records maintained by the Federal

prior to operating at your airport.
Utilize the link Chart Supplement to access a copy of the current Chart Supplement for your airport. The
Airport Inspection Record may assist you in updating your information in the copy of the Chart Supplement
which should then be forwarded to the FAA at the following address:
FAA Aeronautical Information Services
National Flight Data Center

Record to update your Airport Master Record. Corrections to this form should be submitted to the FAA by
logging in and using the Airport Data and Information Portal at htti)s://adih.faa.,-,ov/azis/iiublic/#/I)ublic For
changes that cannot be made utilizing this website, please contact your representative with the FAA Orlando
