provided or the bid will be rejected.
This hous e may or may not require the homeowner to vac ate the premis es during the c onstruction period. The period for Construction shall be 90
Calendar days (Mon-Sun) from the date of c ontract ac c eptanc e, exec ution an d Established Notice to Proceed date.
This doc ument may not be alter ed in any manner. The sc op e of wor k s et f orth in this document may not be changed without the written consent of
Guardian CRM, Inc. under dir ect authorization from the City of Sebastian and/or its official representatives.

SCOPE OF WORK
(“SOW”)

NOTICE TO BIDDING GENERAL CONTRACTORS
No contractor shall, without prior written approval from the Housing Specialist or CDBG Pr ogram Administrator, deviate from a ny
product recommendations listed within this Scope of Work (SOW). A notice of “or equal” exchange shall be provided to the Inspector
within forty-eight (48) hours of the proposed “or equal” substitution. This notice shall contai n:
1) The nam es of both the SOW recommended product and proposed substitute product specification
2) Comparable manufacturer specifications list, included but not limited to price, warranty information, consumer review reports
3) Any additional information requested by the Sebastian CDBG Program Administrator or its agents.
4) All bids to include the repair/repaint to match existing, all damaged (interior/exterior surfaces, i.e., plaster, stucco, Til e, or any other
material), walls, ceilings, ECT… affected by construction.
5) The contractor will provide all material and shall be responsible for covering all hom eowner belongings, including flooring that cannot
be moved during rehabilitation: this is not the owner’s responsibility: Daily clean-up within and around the home is required.
6) All m easurements and material will be the responsibility of the contracting firm: substituting items to upgrade cabinet heights i s
prohibited.
7) All change orders must comply with the project budget, including allotted contingency, and be approved by the Sebastian
CDBG Program Administrator or Proj ect Manager prior to any change order work can begin.
8) Pr ofit and overhead shall only be included in the specified line item. Other line items shall onl y include material, and labor cost.
9) W here the prim e contractor of record elects to pull “combo” permits, i.e. where the roof, HVAC, or other such discipline permits are
attached to the primary construction permit and not pulled on an individual discipline basis, payment draws must be billed on two
separate draw requests: the first half at 50% project com pletion and the final 50% at the final closeout draw. The final 50% must be
accom plished wi th the final permit showing that all combo disciplines have passed final building department inspection.
Failure to provide any required notification will result in a violation of the terms and conditions of the SOW and Work Rehab ilitation
Contract and a loss of paym en t on the substituted product if it is not replaced with the SOW specified product. Approval will NOT be
granted under any circumstance on or after the fact basis.

1. General Construction and Operational:
1a. Profit and Overhead:
➢ The contractor shall include all profit and overhead in this line item: Profit and overhead shall be inclusive of all costs n ot
directly related to material and labor (i.e. travel, supervision, sub/supplier meetings, etc.)
NOTE 1: The contractor shal l include all profit and overhead on its specified line item under the Applicant (Owner) and
Contractor Contract page. Profit and overhead shall be incl usive for all costs not causally related to material and labor (i.e.,
travel, supervision, sub/supplier m eetings, etc.). Do not include any profit or overhead costs in each individual item .
NOTE 2: The Profit and overhead can only be billed in two draw requests; the first hal f at 50% completion and the final 50%
at the final closeout draw.
1b. Manuals and Specifications:
➢ The contractor shall supply, at the tim e of the final CDBG Program inspection, all manufacturer manuals and specification
booklets/packets for all supplied and installed products listed within the Scope of W ork. Failure to do so will result in a failed
final CDBG Program Inspection.

1c. General Construction and Operational:
➢ The contractor shall be responsible for and provide all applications, surveys, permits, pl ans, engineering, or other required
federal, state, and local docum entation.

2. Roofing and Attic:
2a. Windows: Where windows are installed, matching touch up paint is required for any disturbed siding, stucco and/or drywall.
➢ Remove all exi sting windows & hardware at the primary dwelling unit: Instal l (12) Twelve, new vinyl fram e, Hurricane Impact
Resistant wi ndows, with self-storing screens: must meet egress requirements, where/ if required. (W indow size & dimensions to
be det ermined by contracting firm); Installation to include all required permits, additional anchor framing, hardware, components,
and tap cons (TAPCONS M USTBE WHITE) to ensure proper and complete installation to current FBC.
All wi ndows should match existing window size, style and color (except where Code prohibits). All bath windows must be
obscured/safety glass. (Recommended Product & NOA: PGT Windows and Doors or equal in value and quality).
LINE-ITEM NOTES:
NOTE 1: Bid to include rei nstallation of existing windowsills, blinds, and awnings (where and if they exist). Test all a wnings ,
sills and blinds prior to rem oval and reinstallation as requi red to document working order.
NOTE 2: All window replacem ent shall match the exi sting styl e of the removed units, except the rear bay wi ndow. Bay window to
be replaced with standard vertical slidi ng window.
NOTE 3: Contractor shal l submit Dade County or Florida Building Code hurricane im pact product approval stamped by the building
Departm ent of jurisdiction, with this projects close-out package. W indow assembl y shall meet Dade County or Florida Building
Code product approval wi thout using interior slide bolts.

3. Exterior Rehabilitation: Entry/Exit Doors
3a. Sliding Glass Door Replacement:
➢ Remove exi sting rear sliding glass door (1 unit). Install one (1) new State of Florida Product Approved or Miami Dade NOA
approved Hurricane rated Impact double slide, vinyl frame, energy efficient, sliding glass door with sliding screen panel. The
door shall match in terms of style, the existing footprint and swing.
The sliding glass door assembly shall satisfy the Florida Building Code for Low E and pressure requirements. Provide all required
modifications to openings, if necessary, to accommodate the new sliding glass door assembly. (Door size & dimensions to be
det ermined by contracting firm); Installation to include all required permits, additional anchor framing, hardware, components,
and tap cons (TAP CONS M UST BE WHITE) to ensure proper and complete installation to current Florida Building Code. Sliding
glass door manufacturer shall be PGT Win Guard SGD730 or approved equal.
Replacem ent sliding glass doors must fit with minimum perimeter gap between the sliding glass door frame, door buck, and the
opening. All perim eter voids and openings shall be foam sealed and all sliding glass door assembly’s concealed frame and
exposed fram e seam s shall be sealed. W hen sliding glass door returns (inside and outside) are completed contractor shall apply
a continuous bead of caulking around window frame/return joint.

NOTE 1: The completed sliding glass door installation shall comply wi th all requirements of the building code of jurisdiction.
Note 2: Where a landing is required according to code. In absence of a landing, Contractor shall construct a concrete landing in
front of the door as required by code, (or where a landing exists, contractor shall re-construct/modify the existing landing in order
to m eet code).
NOTE 3: Contractor shall submit Dade County or equal Florida product approval stamped by the building Department of
jurisdiction, with this project’s close-out package.
o Sliding Glass Door (1 double0sliding unit) $
3b.Garage Door:
➢ Remove exi sting and install (in existing footprint) a new, White, steel or fiberglass, insulated/Low E (1) single car, hurricane
impact rated garage door. Installation to include weather stripping, trim, and al l required components to manufacturer’s
specifications. (Door size & dimensions to be determined by contracting firm). Installation to include new garage door opening
system (3/4 HP motor with cover and LED light, framing, tracks, two remotes, weather stripping, trim, and all required
com ponents to ensure a code compliant and fully operational system). (Door size & dimensions to be determinedby
contracting firm).
Installation to include all required permits, additional anchor framing, hardware, components, and tap cons (TAP CONS MUST
BE WHITE) to ensure proper and complete installation to current Florida Building Code. Any damage caused by theinstallation
process should be prepared and patched to match interior/exterior walls. Recommended Garage Door Product NOA#-Wayne
Dalton or equal in value and quality).
NOTE 1: Any repairs, up to including replacem ent (if required) to the existing garage door framing to ensure full impact
protection shall be included in this line item .
NOTE 2: Co ntractor shall submit Dade County or equal Florida product approval stamped by the building Department of
jurisdiction, with thi s projects close-out package.
NOTE 3: Contractor shall be responsible for removi ng and reinstalling the sliding screen panel. The unit shall be docum ented
for condition prior to rem oval and installation.

2. Roofing and Attic:
2a. Sloped Roofing:
➢ Remove existing roof covering (Total Roof Span includes Rear Flat Roof ) including all deteriorated surface material. Strip the roof
down to the sheathi ng/decking. Replace sheathi ng and/or decking not to exceed (10%): any replacement above 10% wi ll be
addressed by change order with appropriate back-up. The applicable roof span(s) are to be re-fastened using faster type and spacing
distances that are in accordance wi th the current Florida Building Code requirements. Any replaced roof sheathing and the new roof
covering shall be re-attached (if code allows) identically to the existing fastener and/or attachment method.
Install the following: a layer of ASTM modified sel f-adhered metal roof underlayment; 26ga. Galvalume drip edge, standard
color baked on paint finish, fastened to code; 26ga. galvanized valley metal fastened to code. The entire new roof system shall
conform to building code and jurisdictional requirements. No fasteners shall penetrate exposed surface areas, fascia or
drip edge.
Remove and replace exi sting roof covering wi th new, 26 Gauge, concealed fastener, metal roof covering to code. Additionally, all
boots and vent/stacks shal l be replaced with code com pliant low profile vent products and material. The new boots and vent/stacks
flashed per the most stringent requirements of the manufacturer’s specifications or FBC (5-year warranty required).
(Recomm ended Product: Fabral products or equal in value and quality).
LINE-ITEM NOTES:
NOTE 1: Prior to submitting thi s bid…all bidding contractors shall ensure with the building department and note the slope of
the roof to ensure that m etal roof installation can be utilized per this scope of work.
NOTE 2: Line item to include cost for sealing intersecting span of the main sloped roof and the rear flat roof.
NOTE 3: An additional 36” layer of self-adhering modi fied underlayment shall be placed above the seams in all valleys. The
underlayment shall extend a minimum of 2” over each seam.
NOTE 4: As applicable, where fastener type and requirements are not code compliant…the entire roof is to be re -fastened
using fastener type and spacing distances that in accordance with the current Florida Building Code requirements. The
contractor shall subm it an affidavit to the inspector to the effect that the sheathing/decking nail fastening specification
described in this line item has been met.
NOTE 5: Contractors are responsible for price consideration for removal and disposal of any areas of the applicable roof
spans where there is a double layer or more of exi sting roof covering present.

o Sloped Roofing- (Roof Covering and components________________________________________________$
2b. Gutters:
➢ Remove and install new, seam less, six-inch (6”), white aluminum gutters, including gutter guards, and extended downspouts in the following
roof spans:
Front and Rear Roof Span Elevations:
Installation to include any and all required hardware to ensure correct and proper operation and to allow proper water flow away from dwelling: in
such a manner to disburse water flow outward to the center of the yard grass and foliage as to limit erosion and water pooling at the base of the
walls. (Recommended Product: Amerimax Hom e Pr oducts or equal in value and quality).
LINE-ITEM NOTES:
NOTE 1: Gutter installation shall include the rear porch overhang span.
NOTE 2: W here satellite or other fascia/roof attachments will interfere with installation. The homeowner shall be notified of any suc h items that need
to be removed and later re-installed. Contractors without written permission and a liability waiver shall not remove or re-install such items.

All work to be performed in a workmanlike manner, in accordance with the Sebastian -CDBG Work Write Up Specifications, Florida Building Code, local
codes, and manufacturer’s specifications. The contractor shall be responsible for the repairs and/or reinstallation of materials/equipment/fixtures
damaged or removed due to any work item contained herein. Contractors shall properly dispose of all fixtures, materials and o ther items removed from
the dwelling unless otherwise specified herein. Bid will be accepted on the TOTAL BID submitted, and all items must be cost itemized in the space
provided or the bid will be rejected.
This house may or may not require the homeowner to vacate the premises during the construction period. The period for Constru ction shall be 90
Calendar days (Mon-Sun) from the date of contract acceptance, execution and Established Notice to Proceed date.
This document may not be altered in any manner. The scope of work set forth in this document may not be changed without the w ritten consent of
Guardian CRM, Inc. under direct authorization from the City of Sebastian and/or its official representatives.

SCOPE OF WORK
(“SOW”)

NOTICE TO BIDDING GENERAL CONTRACTORS
No contractor shall, without prior written approval from the Housing Specialist or CDBG Program Administrator, deviate from any
product recommendations listed within this Scope of Work (SOW). A notice of “or equal” exchange shall be provided to the Inspector
within forty-eight (48) hours of the proposed “or equal” substitution. This notice shall contain:
1) The names of both the SOW recommended product and proposed substitute product specification
2) Comparable manufacturer specifications list, included but not limited to price, warranty information, consumer review reports
3) Any additional information requested by the Sebastian CDBG Program Administrator or its agents.
4) All bids to include the repair/repaint to match existing, all damaged (interior/exterior surfaces, i.e., plaster, stucco, Til e, or any other
material), walls, ceilings, ECT… affected by construction.
5) The contractor will provide all material and shall be responsible for covering all homeowner belongings, including flooring that cannot
be moved during rehabilitation: this is not the owner’s responsibility: Daily clean-up within and around the home is required.
6) All measurements and material will be the responsibility of the contracting firm: substituting items to upgrade cabinet heights is
prohibited.
7) All change orders must comply with the project budget, included allotted contingency, and be approved by the Sebastian CDBG
Program Administrator or Project Manager prior to any change order work can begin.
8) Profit and overhead shall only be included in the specified line item. Other line items shall only include material, and labor cost.
9) Where the prime contractor of record elects to pull “combo” permits, i.e. where the roof, HVAC, or other such discipline permits are
attached to the primary construction permit and not pulled on an individual discipline basis, payment draws must be billed on two
separate draw requests: the first half at 50% project completion and the final 50% at the final closeout draw. The final 50% must be
accomplished with the final permit showing that all combo disciplines have passed final building department inspection.
Failure to provide any required notification will result in a violation of the terms and conditions of the SOW and Work Rehabilitation
Contract and a loss of payment on the substituted product if it is not replaced with the SOW specified product. Approval will NOT be
granted under any circumstance on or after the fact basis.

1a. Profit and Overhead:
The contractor shall include all profit and overhead in this line item: Profit and overhead shall be inclusive of all costs not
directly related to material and labor (i.e. travel, supervision, sub/supplier meetings, etc.)

2a. Driveway:
Break existing apron and driveway and dispose of the material. Fill and level the exposed ground in the footprint of the rem oved
driveway material and prepare to accept a new concrete apron and driveway. Once filled, compacted and leveled a new form shall be
placed, and a new concrete/cement apron and driveway shall be constructed.
The new driveway shall be installed in accordance with the building department width requirements. The driveway shall be
constructed to a minimum of 3,000psi and include all components (drain culvert) required to meet the current code and any req uired
local ordinances.
The contractor shall be responsible for replacing any damaged grass with sod (to match existing grass type) in the footpring of any
disturbed areas.
