Evans Environmental and Geological Science and Management, Inc., (EE&G) has completed a Phase
I Environmental Site Assessment of St. Sebastian PUD located in Sebastian, Florida. All work was
performed in accordance with our proposal of March 25, 1994.
The information presented in this report was obtained through indirect methods of assessing potential
for or occurrences of environmental harm affecting the subject property. No direct measurements
for the presence of water, soil or air contaminants were made, except where noted otherwise, and
lack of contaminants existing on or below the subject property should not be concluded without
proper testing. The findings of this investigation merely assess potential for property owner liabilities
arising from past and present usage of the property and should not be construed as conclusive
evidence that the property has or does not have conditions which are environmentally harmful.

1.0 EXECUTIVE SUMMARY
At the request of Mr. Bruce Cooper of City of Sebastian, Florida, Evans Environmental and Geological
Science and Management, Inc. (EE&G) has conducted a Phase I Environmental Site Assessment of St.
Sebastian PUD located in Sebastian, Florida.
FINDINGS:
Based upon the information sources, conditions, and the scope of work, EE&G submits the following
findings for the subject site regarding existing contamination or potential for contamination from the following

This Phase I Environmental Site Assessment has not revealed any indication of surface or subsurface
contamination and there is no indication that the surrounding properties have caused soil and/or
groundwater contamination.
According to the national wetlands inventory map prepared by the Department of the Interior - Fish and
Wildlife Services, Office of Biological Services, wetland areas are located on the subject property. Federal,
state, and local regulatory agencies should be contacted concerning specific wetland regulatory programs,
restrictions, and proprietary jurisdictions that may apply to the subject property.
The subject site is a known habitat of the endangered gopher tortoise and Florida scrub jay. Federal, state,
and local regulatory agencies should be contacted concerning specific regulatory programs, restrictions,
and proprietary jurisdictions that may apply to the subject property.

2.0 INTRODUCTION
EE&G was contracted to perform a Phase I Environmental Site Assessment of St. Sebastian PUD located
in Sebastian, Florida on behalf of Mr. Bruce Cooper for City of Sebastian, Florida. The purpose of our
+ services was to identify apparent environmental concerns from practices and activities that may have
occurred on/or adjacent to the site that could potentially contaminate the site. This assessment is a general
characterization of environmental concerns based on readily available public information and site
observations.
The site inspection was performed on April 13, 1994 by Robert Turner of EE&G. See section 10.0 for a
_ description of the information sources used to perform the assessment. The subject site, approximately 312
acres in size, is located in a residential area in Section 35, Township 30 South, Range 38 East; Section 1,
Township 31 South, Range 38 East; and Section 6, Township 31 South, and Range 39 East in Indian River
County, Florida.
3.0 SITE DESCRIPTION
The subject property is irregularly shaped and has an approximate area of 312 acres. Located on the
property are a large lake and several wetland areas. See Appendix A for Site Plan.
The subject site is bound on the east by Florida East Coast Railroad, across from which is US Highway 1;
+ on the west by Sebastian Municipal Airport and Golf Course; on the south by private homes; on the north
by private homes.
+ The subject site is located in an urban area. Municipal water is not currently supplied to the subject
property. The site is not currently serviced by the municipal sewer system. Electric utilities are not currently
provided to the subject property.

4.0 SITE BACKGROUND/HISTORY
4.1 Current Ownership
EE&G conducted a records review at the Indian River County Court House located in Vero Beach, Florida
to determine current ownership of the subject site.
As of April 18, 1994 the Indian River County Court House records showed the current ownership of the
subject site as being Carl Julian, Trustee.
4.2 Prior Ownership
In accordance with the proposal dated March 25 ,1994, a fifty year title search was beyond the scope of
+ work for this site assessment and was therefore not obtained.
4.3 Review of Aerial Photoaraohs
Aerial photographs dated 1973, 1978, 1981, 1984, 1988, and 1992 were available for review at the Indian
River County Tax Appraisers Office. Information gathered from the aerial photograph review is summarized

in the following table:

YEARS I COMMENTS
1992 Further residential development occurs to the southeast. Commercial development
again occurs to southeast; baseball field is replaced by commercial building.
1988 Residential development continues southeast of the subject property.
1984 Residential development continues to the southeast. Construction of the Sebastian
Municipal Golf Course is under way.
1981 Residential development continues surrounding the subject site.
1978 FPL electrical easement is cleared and paved across the subject site.
1973 The subject property is undeveloped, except for roads which have been constructed.
The Sebastian Municipal Airport appears to the west of the subject site.
All reviewed aerial photographs show no visible signs of industrial use, drums storage or landfill activities.
A copy of the 1992, 1984, and 1973 aerial photographs are provided in Appendix B.
4.4 Historical Cltv Directories
City Directories are reviewed in an attempt to discover the site history as well as the landuse pattern. No
City Directories for the City of Sebastian were available at the Indian River County Public Library In
Sebastian, Florida.
4.5 Prooertv Landuse History
.. Information obtained through a review of historical aerial photographs indicate that roads were constructed
on the subject site prior to 1973, however the property was never developed. The roads have not been
maintained and are presently in poor condition. During the site inspection, the road surfaces were confirmed
_ to be paved but in poor condition.
5.0 ENVIRONMENTAL SETTINGS
5.1 Site Drainaae and T000araohv
Surface and subsurface drainage is of interest during environmental assessment because this information
Indicates the general direction that contamination, if present, could be transported and settled. The site
topography was observed to be relatively flat. There is a large lake and several wetland areas located on
the subject property; see Section 5.3 for further discussion of wetland areas. A USGS Topographic Map
of the area indicated that the ground surface elevation of the site to be approximately twenty (20) feet above
sea mean level (MSL).
5.2 Reaional Geoloav and Hvdroaeoloov
Indian River County is located in east -central Florida southeast of the crest of the Ocala Uplift. It is underlain
by a thick sequence (nearly 10,000 feet) of sedimentary rocks (carbonates, sands, and clays) overlying a
basement complex of igneous and metamorphic rocks. These rocks range in age from approximately 90
million years old for the basement rocks to recent sediment at the surface.

The upper 1,000 feet of sediments in Indian River County is limestone, dolomite, sand, clay, and shell beds
ranging in age from middle Eocene to recent. The sediments of the middle Eocene age (52 million years)
through the lower Oligocene age are porous carbonates that make up the Floridan aquifer. The overlying
sediments are predominantly clastics (sand and clay) ranging in age from 25 million years (Miocene age)
to the recent sediments found along the present coastline. This section of younger sediments contains the
shallow aquifer that is of great importance to the county.
In Indian River County, a groundwater protection ordinance has been adopted to preserve to the
groundwater quality. The Ordinance has restrictions which apply to certain types of industries operating
within the protection areas. The protection areas are usually located within the boundary of the public well
_ field cones of influence. The subject property is not located within a well field protection area.
5.3 Wetlands
Wetland is the collective term used to describe a variety of ecosystems which include swamps, bogs and
other transitional zones that are located between water bodies and dry land. Wetlands are defined by
regulation as areas that are inundated or saturated by surface or groundwater at a frequency and duration
sufficient to support, and that under normal circumstances do support, a prevalence of vegetation typically
adapted for life in saturated soil conditions. Under federal law, the "no net loss" wetlands regulation,
promulgated in 1988, wetland development is restricted. Wetland issues are a significant factor in land
assessment. Land owners, developers, and lending institutions should be aware of the financial burden by
development of wetlands.
A national wetlands inventory map was prepared by the Department of the Interior - Fish and Wildlife
— Service, Office of Biological Services. This map was reviewed to identify the presence of wetlands or
wetland vegetation located on or near the subject property. According to this map there are designated
wetlands on the subject property.
5.4 Coastal Zone
The Coastal Zone Management Act (CZMA) enacts the State to develop management plans for areas
— identified as Coastal Zone areas. Undeveloped coastal dunes and beaches are generally fall in the scope.
In Florida, 80% of the coastal areas fall in the CZMA definition. Special land management planning and
development restrictions have been implemented by the State. The subject property is located in a CZMA
according to the Indian River County Comprehensive Plan, 1988.
5.5 Endanaered Soecies
The subject property is a known habitat of the endangered gopher tortoise and Florida scrub jay. Federal,
State, and local regulatory agencies should be contacted concerning specific regulatory programs,
restrictions, and proprietary jurisdictions that may apply to the subject property.

6.0 SITE RECONNAISSANCE AND FINDINGS
6.1 Soil Tvoe & Descrintion
Black sandy top soils were observed on the subject property. Soil excavation or pile filling was not
observed within the subject property. The Soil Survey of Indian River County, prepared by the USDA Soil
Conservation Service was reviewed to determine the general soil types of the subject property and adjacent

areas. According to this document, the soil on the subject site belongs to the Immokalee-Myakka-Satellite
series. This series can be described as nearly level, poorly drained, sandy soil.
6.2 Potable Wells/Monitoring Wells
A water well is considered a significant factor in environmental assessment because water wells can channel
contamination from the surface to the groundwater. It can also be a contamination -dispersing tool when
contaminated groundwater is retrieved for consumption. It affects the direction and the transmissivity of a
contamination plume as well. Potable or monitoring wells were not observed on the subject property during
the site inspection.
6.3 Storage Tanks and Drums
No evidence of Underground Storage Tanks (USTs), including fill ports and vent pipes, were observed during
the site inspection. No drums were observed during the site inspection.
6.4 PCB Hazards in Transformers and PCB -Containing Eauioment
The inspector conducted a site survey for the presence of PCBs or PCB -containing equipment, limited to
a visual inspection, such as transformers, capacitors, and hydraulic equipment. PCBs were widely used in
such equipment until 1979 when the U.S. EPA strictly regulated their use. Many utilities have since acted
to replace PCB -containing transformers and capacitors. There are no transformers located on the subject
property.
6.5 On -Site Hazardous Substance Identification/Inventory
The site inspector did not observe any regulated substances on the subject site during the inspection.
6.6 Asbestos-Containina Material (ACM)
The subject site is undeveloped. Other potential sources of ACM were not observed on the subject
property.
6.7 Radon
The U.S. Environmental Protection Agency (EPA) Radon Zone Map was reviewed to determine average
predicted indoor radon screening levels in Indian River County. According to this map, Indian River County
is classified as Zone 3, having a predicted average indoor radon screening level of less than two (2)
picoCuries per liter. Four (4) picoCuries per liter has been established by the Environmental Protection
Agency as the minimum level that requires further testing, but does not require immediate remediation.
However, radon levels can vary significantly from property to property and even from room to room in the
same building. No conclusive comment can be made regarding the presence of radon gas without proper
screening measurements. In this case, the subject site is undeveloped land, the radon gas hazard is
therefore, not relevant.
6.8 Lead -Based Paint and Lead Content in Drinkina Water
The most common health hazards from lead originate from Lead -Based Paint (LBP) and lead content in
drinking water. The health hazards are especially significant in older residential buildings. These

environmental concerns can only be determined through testing. No conclusive comment can be made
regarding the presence of lead without proper screening measurements. The subject site is undeveloped.
Historical aerial photographs do not reveal the presence of landfills on the subject site. Lead contamination
of groundwater should not be a concern.
6.9 Indoor Air Qualitv (IAQ)
The subject site is undeveloped, therefore potential sources of IAQ were not observed on the subject
property.
6.10 Site Observations and Inauiries
The site inspection was performed on April 13, 1994 by Robert Turner of EE&G. Access to the subject
property was provided through the client.
The site is approximately 312 acres in size. The entire property was traversed by the site inspector. The
following information are site observations:
o The site is not currently connected to the municipal water supply, municipal sewer system, or
electrical utilities. No potable wells or cisterns were observed by the site inspector at the time of
the site visit.
o A large lake is located on the subject site.
o Drainage pathways on the subject site appeared to the lake and wetland areas. Storm drains were
not observed on the subject site. No evidence of significant erosion was observed on the site.
c An embankment was observed on the west border of the subject site.
o Wooded areas were observed on the site.
o No evidence of structural ruins was observed by the site inspector during the investigation.
o No foul odors were noted by the site inspector in the area of the subject site.
o Evidence of dumping of household wastes and construction debris was observed by the site
inspector.
o No evidence of air emissions was observed on the subject site.
o The site inspector did not observe any waste water discharges at the time of the site investigation.
o No monitoring wells were observed by the site inspector on the subject site. No evidence of
petroleum product contamination was observed on the subject site at the time of the site inspection.
o No evidence of remedial activity was observed during the site inspection.
o No stained or discolored soils were observed by the site inspector during the site visit.

o No evidence of leachate or seeps was observed during the site investigation.
o No areas of dead, distressed, discolored, or stained vegetation were observed during the site
investigation.
o No evidence of chemical spillage was observed on the subject site during the site inspection.
o No evidence of groundwater or surface water contamination was observed during the site
investigation.
o No evidence of extensive use of pesticides, herbicides, soil conditioners, or fertilizers was observed
by the site inspector.
o No evidence of major electromagnetic radiation sources was observed on the subject site at the
time of the site Investigation.
o The potential off -site contamination sources are discussed in Section 7.0, Surrounding Areas.
Apparent evidences of contamination sources were not observed to exist from adjacent properties.
o No evidence of discharges, leachate, migration, or run-off from off -site contaminant sources was
observed during the site investigation.
7.0 SURROUNDING AREAS RECONNAISSANCE
7.1 Neiahborina Properties
North: Residential area
East: The east side of the subject property is bound by Florida East Coast Railroad, across from
which Is US Highway 1.
South: Residential area
West: Sebastian Municipal Airport and Golf Course
7.2 Adiacent Prooerties Overview
The subject site, adjacent properties and surrounding area are characterized by a combination of light
commercial and residential developments. According to the underground facility list from Florida
Department of Environmental Protection (DEP), the subject property does not have any underground storage
tanks. Three (3) facilities within a 'k-mile radius of the subject site appeared on federal and state databases
concerning the handling of hazardous waste, and/or registration of USTs and ASTs. Descriptions of these
facilities, as well as a discussion of their potential to cause negative impact to the subject site are provided
in Section 8.0 of this report.

8.0 ENVIRONMENTAL/REGULATORY AGENCY INQUIRIES
8.1 Federal Reoulatory Aaencies
The federal environmental regulatory agencies having jurisdiction over the subject site, adjacent properties
and those within a'h-mile radius were contacted and available public records were reviewed by EE&G for
information regarding registrations, investigations, violations, or enforcement actions against the subject site
or adjacent properties.
8.1.1 NPL Sites
EPA's National Priorities List (NPL) was reviewed for any sites on the subject property, adjoining
properties and those within ',h-mile. The NPL is a national list of abandoned or uncontrolled
hazardous waste sites which have been scored by EPA according to a hazard ranking system that
assesses the health and environmental threat posed by the site. The NPL is used by the EPA to
prioritize sites scheduled for cleanup action.
The NPL list shows no sites on the property or within a 'h-mile radius of the subject property.
8.1.2 CERCLIS Listing
The CERCLIS (Comprehensive Environmental Response Compensation and Liability Information
System) is a list of potential hazardous waste sites that EPA is investigating for an existing or
threatened release of a hazardous substance. However, EPA advises that the list contains only
.. those sites that have been brought to the attention of EPA, and therefore it cannot claim that the
list contains all potential hazardous waste sites that exist.
M The CERCLIS list shows no sites on the property or within a 'h-mile radius of the subject site.
8.1.3 ERNS Listing
The Emergency Response Notification System (ERNS) Listing is a federal data base containing
information regarding reported releases of oil and hazardous substances. The ERNS database
integrates both initial notification information of releases of oil and hazardous substances and
additional follow-up information for those incidents.
The ERNS database revealed no reported spills or incidents on the subject site or within a 'h-mile
_ radius of the subject site.
8.1.4 RCRIS Listing
EPA's Resource Conservation and Recovery Information System (RCRIS) Listing identifies those
facilities or sites that have notified the EPA of their activities regarding handling of hazardous wastes.
Inclusion of a site on the RCRIS listing does not necessarily indicate environmental problems on the
site, but rather that the facility is (or was) engaged in hazardous waste handling activities and
therefore may have the potential to cause environmental degradation if hazardous wastes have been
mishandled or otherwise released in an uncontrolled manner.

The RCRIS list reveals no facilities on the subject site, however there are three sites listed within a
h-mile radius of the subject site:
Sebastian Aero Services, Facility ID # FLD177763471, 300 West Airport
Drive, Sebastian, Florida is listed as a small quantity hazardous waste
generator (of 100 to 1000 Kg of hazardous waste during any given month).
This site is not considered to pose significant environmental threats to the
.. subject site because of its distance from the subject site.
Freds Auto, Facility ID # FLD984250381, 1104 Louisiana Avenue,
Sebastian, Florida is listed as a small quantity hazardous waste generator
(of 100 to 1000 Kg of hazardous waste during any given month). This site
is not considered to pose significant environmental threats to the subject
site because of its distance from the subject site.
Mel Fisher's Cobb Coin, Facility ID # FLD984197038, 1201 Indian River
Drive, Sebastian, Florida is listed as a small quantity hazardous waste
generator (of 100 to 1000 Kg of hazardous waste during any given month).
This site is not considered to pose significant environmental threats to the
subject site because of its distance from the subject site.
8.1.5 FINDS Listing
The Facility Index System (FINDS) Listing is a list of facilities and/or locations subject to regulation
.. under certain EPA programs, due to operations conducted at these sites. Inclusion of a site on the
FINDS listing does not necessarily indicate environmental problems on the site, but rather that the
site is (or was) engaged in activities that may have potential to cause environmental degradation
_ if hazardous compounds have been released in an uncontrolled manner.
The FINDS list reveals no facilities on the subject site, however there are three sites listed within'h
mile radius of the subject site. These are the same three sites listed under the Resource
Conservation and Recovery Act (RCRA) section.
8.2 State and Local Reaulatory Agencies
The state environmental regulatory agency having jurisdiction over the subject site, adjacent properties and
those within a �h-mile radius were contacted and available public records were reviewed by EE&G for
Information regarding registrations, investigations, violations, or enforcement actions against the subject site
or adjacent properties.

8.2.1 Florida Sites List

landfilling activities or other related waste handling activities such as those conducted at transfer
stations. The appearance of a site on this list does not necessarily indicate environmental problems
at the site, but rather that the site handles solid wastes that could pose an environmental concern
'rf, as a result of an uncontrolled release, hazardous compounds were able to impact the
environment and possibly migrate from the site.

to the notification requirements found in applicable chapters of the Florida Administrative Code. The
appearance of a site on this list does not necessarily indicate environmental problems at the site,
but rather that potential for environmental degradation to the site or adjacent sites is present if
leakage from the storage tank system occurs.

According to the national wetlands inventory map prepared by the Department of the Interior - Fish and
Wildlife Services, Office of Biological Services, wetland areas are located on the subject property. Federal,
state, and local regulatory agencies should be contacted concerning specific wetland regulatory programs,
restrictions, and proprietary jurisdictions that may apply to the subject property.
The subject site is a known habitat of the endangered gopher tortoise and Florida scrub jay. Federal, state,

10.4 Definitions
Asbestos -Containing Materials (ACM) means those materials that were determined to contain
more than one percent asbestos as determined by the Polarized Light Microscope (PLM) method.
Hazardous Substance means those substances as defined by the Comprehensive Environmental
Response, Compensation and Liability Act (CERCLA), 42 U.S.C. 9601 (14), and includes any material
that is toxic, flammable, explosive, or corrosive as these terms are defined by CERCLA. Excluded
from this definition are petroleum substances or products as defined below.
Petroleum Substance or Product means any material containing refined or crude oil, or any
fraction thereof, and includes natural gas, natural gas liquids, liquified natural gas, or synthetic gas
usable for fuel, or mixtures of natural gas and such synthetic gas. Hazardous substances as defined
above are excluded.
Release means such occurrences as defined by CERCLA, 42 U.S.C. 9601 (10), and includes any
intentional or accidental discharging, spilling, leaking, pumping, pouring, emitting, injecting,
escaping, leaching, dumping or disposing Into the environment.
- Subject Property means the land and all structure located in/at as describe in the legal description,
also known as St. Sebastian PUD.
Not Observed means not visually observed during the site inspection.

11.0 LIMITATIONS
This Phase I Environmental Site Assessment was performed in a manner consistent with generally accepted
professional consulting principles and practices. The conclusions and information presented in this report
were obtained through indirect methods of assessing potential for occurrences of environmental harm
affecting the subject property. Direct measurements, tests, or analyses for the presence of contaminants
were not made, except where noted otherwise. The presence of contaminants existing on or below the
subject property cannot be concluded without proper testing. The results presented in this report should
not be constructed as conclusive evidence that the property has or does not have environmentally harmful
conditions.
Opinions and recommendations presented in this report apply to conditions existing at the time of this
assessment and are intended only for the purposes and site -specific conditions indicated. EE&G shall not
be held responsible for the impact of any changes in environmental standards, practices, or regulations
subsequent to this assessment. EE&G does not warrant the accuracy of information obtained or supplied
by others, nor does it approve the use of isolated, out -of -context portions of this report for any purpose.

proposed referenced project. Upon field investigation of the
site, Masteller, Moler & Reed, Inc. proposes to furnish the
following:
An updated boundary survey of lands now or formly owned
by Carl Julian, Trustee - A.K.A. "San Sebastian P.U.D.",
said lands being located adjacent to the Sebastian City
Hall. Client will need to provide an exact legal
description 'of said lands together with the
certifications as required by the City of Sebastian.
All surveying services performed on this project will be in
complete accord with the surveying minimum technical standards as
set forth under Section 472 of the Florida Statutes.
We propose to perform the above described scope of services
for the lump sum fee of $ 1,580.00. The fee for our services does
not include any recording or filing fees.
For any and all subcontract services invoiced through our
office, there will be a 10% surcharge of the face amount of said
subcontractor's invoice, in order to cover the costs associated
with administration and coordination of the subcontractor's
services.
If at any time information must be forwarded to you by
express mail, these charges will be invoiced to you monthly.
Items not specifically outlined in this proposal are not to
be considered a part of this contract.

Other provisions are as follows:
a) Pavment of Fees
Payment of all fees for services rendered shall be paid
within thirty (30) days of billing. Commencing on the thirty-first
day, interest shall be added to the unpaid balance at the rate of
one and one-half percent (1-1/2%) per month or a total of eighteen
percent (18%) per annum. In the event that the matter of
delinquent payments shall be turned over to legal counsel for
collection, there shall be added the amounts due for reasonable
legal fees.
The surveys prepared by Masteller, Moler & Reed, Inc. shall
be the sole property of that firm until payment on any unpaid
balance is made in full. Owner/Client agrees that until payment
is made in full, he, she, or it shall have no proprietary interest
in the surveys prepared by Masteller, Moler & Reed, Inc.
Masteller, Moler & Reed, Inc. shall have the absolute right to
request return of any and all drawings submitted to governmental
bodies or other parties on behalf of the Owner/Client in the event
of non-payment of outstanding fees by Owner/Client.

application submitted by Owner/client based in whole or in part
upon the surveys prepared by Masteller, Moler & Reed, Inc.

Backcharges will not be accepted by Masteller, Moler & Reed,
Inc. unless we provide written agreement covering all corrective
action and the total amount of the backcharge necessary to
accomplish the corrective action.
d) Print Costs
Print costs include five (5) copies of each phase of each
survey as part of the surveying fee. All other prints and Xerox
copies requested by the client shall be billed to the client based
on a charge of $0.666/SF for blue line prints, $1.50/SF for
sepias, $8.80/SF for mylars, $0.50 for each 8 1/2 x 11 copy, $0.70
for each 8 1/2 x 14" copy, and $1.00 for each oversized copy.
e) Price Guarantee
Prices quoted are firm for sixty (60) days from the date of
this proposal.
f) Contract Assignment
While binding upon the parties, their successors or assigns,
this Contract may not be transferred or assigned without the
consent of both parties. The Owner/client reserves the right to
terminate this Contract within ten (10) days written notice.
Masteller, Moler & Reed, Inc. shall be compensated for services
performed to termination date.
If this proposal meets with your approval, please sign and
return one (1) copy of this letter as our formal Authorization to
Proceed.

PHASE I ENVIRONMENTAL SITE ASSESSMENT
I. The Base Services are as follows: Cost: $1,500.00
Perform a Phase I Environmental Site Assessment of the above referenced property. The assessment will
include the following tasks:
A. Conduct an on -site visual inspection of all accessible areas and existing structures, noting current
usage, signs of spills, contamination and storage or use of hazardous materials including asbestos.
B. Perform surrounding area reconnaissance, noting adjacent property uses and any hazardous
facilities or storage tank sites within a half -mile of the subject property.
C. Perform regulatory review of all publicly available files held by Federal, State and Local agencies
which pertain to the use and handling of hazardous waste, hazardous substances and storage tanks.
D. Obtain information indicating recognized environmental conditions in connection with the property
through interviews with owners, occupants and appropriate local regulatory authorities.
E. Perform historical review using standard historical sources (e.g. aerial photographs, city
directories). The review will develop a history of the previous uses or occupancies of the subject
property and surrounding area to identify any potential recognized environmental conditions. The
period of review will span the previous fifty years and/or until the initial development of the
property from virgin land.
F. Prepare a final report which outlines the findings of the Phase I Environmental Site Assessment
and includes recommendations for further investigation, if necessary. Documentation of any
pertinent resources, references and key exhibits will be included to support the findings of the
report, when appropriate.

AUTHORIZATION FOR SITE ACCESS TO PERFORM
ENVIRONMENTAL ASSESSMENT SERVICES
SITE LOCATION: St. Sebastian PUD

The undersigned hereby authorizes EE&G complete access to the undersigned's property described above
and to all structures and rooms within said structures as is reasonably necessary to perform the Services
as stated in paragraph 2 below, including, without limitation, ingress and egress.
2. Services. EE&G plans to conduct the following assessment services on said property:
