Description of Assignment: Klmley-Horn and Associates, Inc. (CONSULTANT) understands the City of Sebastian (CLIENT) has observed deficiencies in the form of roof leaks at the City's police department building, located at 1201 Main St, Sebastian, FL 32958. CONSULTANT understands the CLIENT has performed multiple replacements of localized locations where leaks have previously been Identified, but the root cause of the leaks still exists. CONSULTANT also understands the roof consist of a Bat roof in the center, with sloping barrel tiles around the perimeter. The flat portion of the roof measures approximately 11,000 square feet, with the total roof area measuring approximately 21,500 square feet. CLIENT Is requesting CONSULTANT perform an assessment of the existing roof system to determine the root cause of the leaks and provide a summary memorandum of findings. The memorandum will Include recommendations for repair or replacement with associated construction costs. This assignment is limited to the assessment and summary memorandum and development of any repair or replacement construction documents will occur under a separate work order after the assessment has been completed and repair replacement recommendations have been made. The servicestproject will consist of: Roof Condition Assessment and Summary Memorandum Description: CONSULTANT will perform a limited visual (non-destructive) assessment of the existing roof at the referenced property. The goal of the assessment is to understand, to the extent feasible, the cause of the roof leaks and determine if repair of the system Is feasible or If replacement Is required. it is anticipated the assessment will take up to two (2) days with two (2) engineers. As part of this task, CONSULTANT will also review any as-builts, previous repair plans, or condition assessment reports as supplied by CLIENT. After completion of the assessment, CONSULTANT will prepare a summary memorandum based on the observed conditions. The memorandum is anticipated to include the following: • Summary of observations and deficiencies observed on the existing roof system • Representative photographs from CONSULTANTS assessment • Locations of recommended repair or replacement • Opinion of Probable Cost (OPC) for the recommended repairs/replacement o The CONSULTANT has no control over the cost of labor, materials, equipment, or over the Contractor's methods of determining prices or over competitive bidding or market conditions. Opinions of probable costs provided will be based on the Information known to CONSULTANT at the time and represent only the CONSULTANT'sjudgment as a design professional familiar with the construction industry. The CONSULTANT cannot and does not guarantee that proposals, bids, or actual construction costs will not vary from its opinions of probable costs. CLIENT will need to coordinate access to the roof on the dates of the scheduled assessment. Deliverables: Summary memorandum with repair or replacement recommendations and opinion of probable cost 1. Preliminary Planning There is no preliminary planning required for this project. 2. Permitting &Inspections There are no permitting or Inspections required for this project. 3. Contract Administration There is no contract administration required for this project. 4.
