O'Halloran City Clerk MINUTES SEBASTIAN CEMETERY COMMITTEE JANUARY 28, 1988 MAYOR'S CONFERENCE ROOM CITY HALL - 1225 MAIN STREET SEBASTIAN, FLORIDA The meeting was called to order at 10:40 A.M. by Elizabeth Reid, the Secretary of the Cemetery Committee, who explained that the Chairman of the Committee was unable to be present and that the Committee had no vice chairman due to the fact'that the former vice chairman had resigned and left town. it was, therefore, agreed that the first order of business was to elect a vice chairman, and nominations were called for. Mr..Schofield nominated Mrs. Phelan; Mrs. Payant nominated Mr. Schofield, which was seconded by Mrs. Phelan, who declined to accept her nomination. Mrs. Phelan moved that nominations be closed and Mr. Schofield was appointed as Vice Chairman. Meeting was then turned over to Mr. Schofield. ROLL CALL: Mrs. Phelan, Mrs. Plyler, Mrs. Payant, Vice Chairman Schofield, Ex-Officio Member Kip Kelso 2. PLEDGE OF ALLEGIANCE - Not done APPROVAL OF MINUTES - Motion by Mrs. Phelan, seconded by Vice Chairman Schofield, to approve the Minutes of the Meeting of April 23, 1987, as printed. Carried. 5. OPERATING STATEMENT - PERIOD ENDING 11/30/87 -.No discussion IRRIGATION SYSTEM - UNIT 4 - Mr. Kelso presented a diagram of a proposed irrigation system for Unit 4, which would involve the purchase of a 3 h.p. pump to be hooked up'to the existing 3" P.V.C. mainline. It was the consensus of opinion that this is a good idea for the present, but plans will have to be made also for Unit:3'sometime in the future. There was a brief discussion re the fence on the north boundary of the Cemetery, and the steps to be taken to get it completed. Mr. Kelso will talk to his supervisor and also find out how the property owner abutting the northern boundary feels about sand being shifted on to his property. If that person is .willing, Mr. Kelso was requested to obtain something from him in writing. 9. NEW BUSINESS: A. REQUEST TO PLACE 3 FOOT CROSS ON GRAVE - MRS. YOUNG After discussing, the request of Mrs. Young to place a 3 foot cross on the grave of her father, Arthur .Zimmermacher, .the follow- ing decision was made: To deny the request since it would not be in conformity with the following Cemetery Rule: Ail Cemetery plat markers shall be either bronze, marble or granite. No permanent attachments shall be above the marker. B. CREMAINS LOTS - UNIT 4 Mr. Kelso mentioned tha~ he would like to tal~ to Councilman Vallone about obtaining Council approval, to use all the areas marked on the map with a "P" for cremains rather than planters and the Committee had no objection. The Secretary informed the Committee that David Hincemon had been appointed by the City COuncil on January 27, 198'8, as-a member of the Committee, to fill the vacancy left by former Vice Cha'irman Daniel Pottinger. Please make copies locally and distribute to the cemeteries and crematorles in your local county. Coler, Secretary OF FLORIDA OF HF_.ALTH AND REHABILITATIVE SF__J:IVlCF~q February 10, 1989 TO, Cemetery Sexton/Crematory Manager SUBJECT, Burial Transit Permit (BTP) Procedures Florida Statute Chapter 382.007 requires that whoeYer is in charge of a place o~ ~lnal disposition (cemetery/crematory) must ~ign the original Burial Transit Permit, HR$ Form 326, enter the date of interment, and forward to the local HRS county public health-unit, vital statistics office, within ~.en calendar dsys. The local vital statistics office is required to match all local registrar copies of the BTP with the original BTP, which you must submit, and report any discrepancies to the HRS State Office of Vital Statistics. The HRS Office of Vital Statistics reports these cases to the Department of Professional Regulation who in turn issues complaints against the funeral home if an original 8TP haj not been received in the county of death. Your cooperation in this procedure is necessary for the proper tracking of disposition of deceased remains.
