COORDINATION AGREEMENT
This is an agreement entered into on August 15, 2005 between the Sebastian Police
Department and the Indian River County Council on Aging, Inc.
For over thirty (30) years, the Indian River County Council on Aging has been providing care
and support which enables elders to remain independently in their homes and live with dignity.
However, due to the multiple needs of the elderly clients, it is agreed that:
The Indian River County Council on Aging and the Sebastian Police Department will maintain a
mutual, cooperative relationship in which each will:
I. Make clients aware of the services available from the other
2. Make appropriate referrals to the other, based on clients' needs and wishes
3. Maintain ongoing communication concerning clients being served by both
agencies, simultaneously
4. Working together to the betterment ofthe individuals we serve
