Dear Mr. Kelso,
It has been 4 years (don't seem that long) that I contacted you about a marker for
Donna J. Perkins. Has there been any change in the situation, has there been a marker
placed for her?
I have contacted Jamie Hicks at Everlasting Stoneworks for prices and inscription
information. I will be in a position in about 4 weeks to make final arrangements with him
regarding a marker for Donna.
I hope this meets with your approval. In reviewing the paperwork that you sent me 4
years ago, it has been way past the 120 days of placing a marker for her. As I stated in
previous correspondence I assumed that the marker would be taken care of by her
husband at that time.
This is just to let you know that Donna had not been forgotten and that I want to do
something for her. If you need any further information please let me know.

I am in receipt of your March le, 2009 letter pertaining to Mrs. Donna J. Perkins.
You stated in your February 174', 2009 letter that Brownlie Maxwell Funeral Home in Melbourne
has record of burying Mrs. Perkins in the Sebastian Municipal Cemetery on December 13, 1971
in block 32, lot 9. I have found a vault in that location and I am satisfied with the 1971
Brownlie Maxwell Funeral Home record. We can do no more concerning Mrs. Perkins.
Please find enclosed business cards of local monument companies if you would like to purchase
a monument for Mrs. Perkins.

Dear Ms. Keb:
In response to your February 17th, 2009 letter in which Brownley Maxwell said there were
records indicating that Donna Perkins was in lot #9 in block 32 of unit 2 in the Cemetery, I
probed and did locate a vault. The grave is unmarked and right next to the grave you
mentioned in your January 6th, 2009 letter. (photo enclosed)
The City of Sebastian's ordinances require a marker be installed within 120 days after burial.
If this is in fact your aunt, would you be the surviving relative responsible for compliance to the
City's ordinance? If not, please advise us who to contact to mark your aunt's site as soon as
possible.
In addition, please advise us if you feel compelled to change the spelling of "Jerry" to "Gerry" on
the marker next to your aunt's site at your own expense. The City does not provide for
mistakes on markers but we would like to be as accurate as possible.
If you do not want to change the spelling, would you know which monument company installed
the marker? I might be able to get the next -of -kin's name from the monument company to see
about the correct spelling.

After I received your letter of January 14, 20091 wrote -to the Maxwell Brownlie Funeral
Home in Mclbcnvw.
They have contacted me with the following information: Donna Titus Perkins was buried
on December 13, 1971 in BIock # 32, Lot # 9 at your location according to their records.
Would you, therefore, do additional clerking and let nw know if this is correct' If it is,
what is the correlation of her location to the one of Gerry A. Perkins?

To dispute the funeral Homes records I would have to assume that it is Donna Perkins but there is
no marker
And as we were discussing yesterday you need to contact Ms. Keb and let her know as per her

City Clerk( Sally A. Maio ) what correlation was Donna perkins in relation to Gerry A. Perkins, In
the schematic
I gave you it shows that Donna Perkins is in lot # 9 and Gerry perkins is in # 10 side by side.
As we discussed you also need to tell Ms. Keb she need to get a marker for donna , and when you
do that
Send a copy of the ordinances hi -lite marker installation period and tell Ms. Keb that if she would
like find
Out what monument co. did Gerry,s and get a new one with the correct name and dates.

Dear Ms. Keb:
The sexton and I have done a thorough search of the City's records and do not find any
evidence of your aunt, Donna Jean Perkins, being buried in the municipal cemetery.
It is my understanding that Florida death certificates list the funeral home and cemetery of the
deceased; however, copies of these must be applied for by family relatives accompanied by a
fee and photo identification.
The Brevard County Health Department did indicate they have a file #7971071739 for Donna
Jean Perkins. I am enclosing a copy of the application process if you would like to pursue this.

INFORMATION AND INSTRUCTIONS FOR BREVARD COUNTY DEATH RECORD APPLICATION
Our office issues certified copies of death records once the required fee is received and the eligibility
requirements listed below have been met.
ELIGIBILITY:
WITHOUT CAUSE OF DEATH: Any person of legal age (18) may be issued a certified copy of a death record without
the cause of death.
WITH CAUSE OF DEATH INFORMATION: Death records with the cause of death information may only be issued to
the following individuals: 1) the decedent's spouse or parent; 2) to the decedent's child, grandchild or sibling, if of legal age;
3) to any person who provides a will, insurance policy or other document that demonstrates his or her interest in the estate of
the decedent or 4) to any person who provides documentation that he or she is acting on behalf of any of the above named
persons.
All requests for certification of a death certificate that includes the cause of death information must include the signature of
the applicant, state his or her qualifying eligibility, or a notarized Affidavit to Release Cause of Death Information (DH Form
1959), which is available upon request.
The funeral director associated with the funeral home listed on the death record, or attorney representing an eligible person
listed above must include their signature, professional license number, and the name and relationship of the person they are
representing. If you are a funeral director not associated with the funeral home listed on the death record, or an attorney not
representing someone listed above as eligible to receive cause of death information, then a completed Affidavit to Release
Cause of Death Information (DH Form 1959) must accompany this request.
NOTE: Florida Clerks of Court will not accept a death record with the cause of death information when filing probate.
APPLICANT'S SIGNATURE: Applicant's signature is required, as well as his/her name, valid residence address and telephone

Dear Cl .rk,
Earlier i 12008, I had been to the cemetery and could not find a marker of Donna Jean
(Titus) Perkins, In November, (this is 2nd letter) I wrote to you regarding the location of
the gray, and marker. She died December of 1971. I attended her funeral and pretty sure
she was buried there. Her D. O. B. was June 26, 1933 in Ohio.
I could i ind a marker for Jerry A. Perkins, Jan 3, 1933, Dec. 12, 1965.
Gerry ar d Donna were married to the same man — William J. Perkins. At the time of
Donna's death they were living in Malabar.
Donna a as my aunt. I am trying to verify the location of here burial. Can you please fill
in the bb inks as to why 1 could not find a marker or burial location?
