HomeMy WebLinkAboutSkate Facility Waiver Instructions SKATE FACILITY WAIVER INSTRUCTIONS Be sure all skaters’ information is completed at top of waiver, and all appropriate signatures are on back of waiver. This waiver needs only to be completed on the first visit. Each time the skater visits the facility, they must supply proper ALL SKATERS identification so that the attendee can verify a waiver is on file. MUST HAVE A SIGNED WAIVER ON FILE. When accepting the waivers the following items must be completed. 1.If the skater is over the age of 18, the attendee must check their identification and birth date, be sure they checked the first box and complete the statement on the front of the waiver, and have them sign the back of the waiver. NOT 2.If the skater is under the age of 18, and accompanied by a parent or guardian, a photograph of the skater must be attached to the waiver, the parent or legal guardian must check the second box and complete the statement on the front of the waiver, and the signature of the parent or guardian must have been notarized on the back of the waiver. Be sure parent or legal guardian’s signature is on back of waiver. 3.If the skater is under the age of 18, and accompanied by a parent or guardian, a photograph of the skater must be attached to the waiver, the parent or legal guardian must check the second box and complete the statement on the front of the waiver, and the attendee must check the identification of the parent or legal guardian. Be sure parent or legal guardian’s signature is on back of waiver. IF WAIVER IS NOT FULLY COMPLETED AND SIGNED, THE SKATER WILL NOT BE ALLOWED TO SKATE.
