The applicant shall prepare and present at the pre-construction conference an emergency restoration plan to address eventualities such as hurricanes. The applicant shall provide a 24- hour telephone number by which the Department may contact the person responsible for and who has authority to act on the emergency restoration plan and maintenance of traffic. 11. In the event the applicant fails to meet any of the provisions of this permit, all work in pursuit of the permit objective must cease and the applicant must immediately restore affected public property to a condition as good as or better than before permit work began. The applicant will continue to indemnify the Department as stated above until restoration is accepted by the Department. 12. The applicant is required by Florida Statute to notify local law enforcement agencies prior to closing one or more lanes of the State highway for periods exceeding two hours. 13. At the completion of the construction, and prior to Department acceptance of certification, the engineer of record shall certify with original signature that the work and materials: (a) Comply with the Department Standard Specifications for Road and Bridge Construction dated and Roadway and Traffic Design Standards dated (b7 is in accord with all plans, special..provisions, policies, regulations, and safety standards pertaining to the permitted work. (c)Is in accord with all applicable local, county, State, and national ordinances, rules, codes, and laws. 15. The Department may accept certification of the permitted work upon the restoration of the public facility and satisfactory completion of the permitted work. The Department shall not accept certification prior to the applicant's submission of two complete sets of as-built plans and three sets if a bridge structure is involved. Unless the applicant can show good cause for delay and unless the Department approves an extension, all permitted construction work shall be completed within one year of permit approval. 16. If permission is granted to temporarily close a sidewalk,it is done so with the express condition that it will be re-opened as quickly as possible. If any Portable Traffic Monitoring Site(PTMS)or a Telemetry Traffic Monitoring Site (TTMS)is in conflict,The PL&EM Office must be called to establish location.If any PTMS and or TTMS sites are damaged during construction,the complete restoration and repairs will be the responsibility of the applicant.The Planning and Environmental Management Office(PL&EM)must be notified immediately at(954)777-4601 to schedule a location or Inspection. The D.O.T.right-of-way cannot be utilized for on-site construction. The D.O.T. roadway,sidewalk,etc.must not be disturbed until the off-site improvements shown on the permit are ready to be constructed. This construction should be completed as soon as possible so as to minimize disruption within the R/W. This permit is valid only for work proposed within the D.O.T. right-of-way. Certification acceptance and final approval is contingent upon conformity of all work done according to this permit. A copy of this permit and plan will be on the job site at all times during the construction of this facility. Permittee is cautioned that utilities may be located within the construction area. Validity of this permit is contingent upon permittee obtaining necessary permits from all other agencies involved. Give Special attention to MOT note instructing that the local media shall be contacted a week prior to any lane closures on the state road system if it will occur during peak hours or over the span of more than one day.
