Castille Secretary CERTIFIED MAIL NO.: 70040750000359835042 RETURN RECEIPT REQUESTED NSW-FLR10R164-HW An inspection of Sebastian Administration/Police Building was performed on Thursday, July 15, 2004 at 3:00 PM to assure compliance with the state's federally approved National Pollutant Discharge Elimination System (NPDES) stormwater program. Representatives from Science Applications International Corporation (SAIC) conducted the inspection at the above referenced location, on behalf of the Department of Environmental Protection (DEP), and noted potential violations of Section 403.0885, Florida Statutes, and the rules promulgated thereunder. The enclosed inspection report identifies areas that received a marginal or unsatisfactory rating during the inspection. The ratings are found on the first page under the "Summary Evaluation" section. Details about the ratings follow the first page and may include checklists, narrative discussion and photographs. At the time of the inspection, potential violations identified at Sebastian Administration/Police Building included the following: 1. The Storm Water Pollution Prevention Plan (SWPPP) did not contain the latitude/longitude of the outfalls or a contractor's certification form. You may avoid a determination of actual violations and formal enforcement by the Department by correcting any potential violations you may have at your project/site. The Department requests that you provide the following information about all corrective actions implemented on site: 1. A revised SWPPP containing the information noted as missing. If the Department does not receive a response regarding this "Non -Compliance Letter" with evidence that all corrective actions have been completed within 30 days of receipt of this letter, the Department will pursue formal enforcement for the violations and will seek to impose civil/administrative penalties. The Department may schedule an inspection to determine whether the corrective actions implemented on site satisfy applicable permit requirements.
