Dear Mr. Milewski:
This is to acknowledge receipt of your application, plans and related material for the above
project. Your application has been reviewed and the following items require your attention and
response in accordance with Rules 62-4.055 and 62-4.070 and 62-604, Florida Administrative
Code (F.A.C.):
1. Please revise the engineering drawing sheet to include a note on the minimum vertical and
horizontal separation between potable water and sanitary sewer mains to comply with Rules
62-604.400(2)(g) — (i), F.A.C. and 62-604.400 (3), F.A.C., as noted below.
New or relocated, underground water mains shall be laid to provide a horizontal distance of
at least six feet, and preferably ten feet, between the outside of the water main and the
outside of any existing or proposed gravity- or pressure-type sanitary sewer, wastewater
force main, or pipeline conveying reclaimed water not regulated under Part III of Chapter
62-610, F.A.C. The minimum horizontal separation distance between water mains and
gravity -type sanitary sewers shall be reduced to three feet where the bottom of the water
main is laid at least six inches above the top of the sewer. New or relocated, underground
water mains crossing any existing or proposed gravity- or vacuum -type sanitary sewer shall
be laid so the outside of the water main is at least six inches, and preferably 12 inches, above
or at least 12 inches below the outside of the other pipeline. However, it is preferable to lay
the water main above the other pipeline.
At the utility crossings described above, one full length of water main pipe shall be centered
above or below the other pipeline so the water main joints will be as far as possible from the
other pipeline. Alternatively, at such crossings, the pipes shall be arranged so that all water
main joints are at least three feet from all joints in vacuum -type sanitary sewers, or pipelines
conveying reclaimed water regulated under Part III of Chapter 62-610, F.A.C., and at least
six feet from all joints in gravity- or pressure-type sanitary sewers, wastewater force mains,
or pipelines conveying reclaimed water not regulated under Part III of Chapter 62-610,
F.A.C.

2. There appears to be a discrepancy between Part II, Subpart (5)(A)(26) of the permit
application and sheets C5, C6 of 14 of the engineering plans. The item is initialed, however,
the engineering plans show a design different from the standard criteria. Please provide a
brief, but specific explanation why this item does not apply.
3. The operating conditions for the proposed lift station in the engineering drawings and the
permit application do not correspond. Please revise to ensure all permit documentation is in
concurrence.
4. It is recommended to install an isolation valve every 1000 LF of proposed force main
constructed.
5. Please label the type of material for the proposed forcemain in the engineering plans and Part
II item 2 in the permit application and resubmit.
Construction of the proposed collection/transmission system without a valid permit issued by the
Department is a violation of Section 403.087(1), Florida Statutes (F.S.) and subject to penalties
under Section 403.161, F.S.
You are requested to respond, in writing, to the above items within thirty (30) days of the date of
this letter. Pursuant to Section 120.60, Florida Statutes, the Department may deny a permit
application if the applicant, after receiving notice, fails to correct errors, omissions, or supply
additional information within a reasonable period of time.
The processing of this application will resume following receipt of the information requested
above. Please refer to this letter in your response. Should you wish to discuss the above
