constructions of the administration building and police evidence compound at the Sebastian Municipal Airport.
This job as we understand is budgeted at $2,000,000. Should the contract be awarded, and accepted by
Holland Builders of South Florida and acceptable to surety, the performance and payment bond will be Issued
to guarantee this project through an A rated T -listed surety company.

Marc Parent joined Holland Builders, as Sr. Vice President of
Construction Services in early 1996. His responsibilities include the overall
management and implementation of all construction projects as well as
overseeing Holland's construction team of project managers, estimators, and
field superintendents.
Highly knowledgeable in all phases of construction and site development he
has personally been involved in the construction of more than $500 million
dollars of commercial, institutional, aviation and multifamily projects. He has
more than 30 years experience in the construction industry.

VICE PRESIDENT of CONSTRUCTION
Employed by Holland Builders since 1988, Ron Parent is both a Florida
Licensed State General Contractor as well as a Licensed Roofing Contractor.
At Holland Builders he serves as both the Chief Estimator for the company
as well as Construction Vice President. He has been involved in the majority
of aviation projects constructed by Holland Builders both as the senior project
manager and on the design development team. His more than 20 years of
commercial construction experience includes, in addition to aviation facilities,
the construction of supermarkets, shopping centers, warehouses and
distribution centers.
JAMES E. KELLY
PROJECT MANAGER
Jim joined Holland Builders in 2000 as a Project Manager and Estimator.
He is a Florida Licensed General Building Contractor. His primary focus at
Holland Builders has been aviation and medical projects. Prior to joining the
Holland team, he spent 20 years working as project manager, estimator and
independent contractor focusing on commercial and high end residential
projects.

Randy L. Mosby, P.E., the president of Mosby and Associates, Inc. has extensive
experience in governmental and private development, drainage and pavement design,
land planning, site development and structural design and coastal protection. Mr.
Mosby also has experience in bridge design, construction, master planning, waste water
supply and treatment, and construction management, including testing and inspection.
His professional registrations include Professional Engineer in Florida (PE22326)
Certified General Contractor in Florida (CG22927), Forensic Engineer (616), and Special
Inspector (826).
A graduate of the University of Central Florida in Civil/Environmental Engineering, a
graduate of the University of Missouri - Rolla, with a Master of Engineering in Structural
Engineering, he has experience representative of his fields of specialization. As a project
engineer for Sverdrup & Parcel and Associates, Inc., he supervised projects including
the preliminary design for the Dames Point Bridge in Jacksonville, Florida, construction
inspection and material testing for the J. Turner Butler Boulevard Expressway,
Jacksonville, Florida and the structural design of four low-level trestle bridges in the
Florida Keys. These projects consisted of roadway and drainage designs, along with the
preparation of construction specifications and cost estimations. Mr. Mosby has
experience in the design of several highway improvements, large acre developments,
airports and industrial parks.
Currently as the owner, the president, and senior engineer for Mosby and Associates,
Inc., along with his newly acquired partner, Mr. Bruce A. Moia, P.E. they supervise one
civil engineer, one structural engineer, four civil designers and a staff of technical
support professionals for the civil and structural branches of Mosby and Associates, Inc.

Vice President/ Senior Engineer
Bruce Moia, P.E. the Vice President of Mosby & Associates, Inc. has over 20
years experience in the design and permitting of commercial, industrial, multi-
family and single family subdivisions, and. site plans, as well as roadway and
master utility designs. He received his Florida Professional Engineering license in
1993, is a member of Chi Epsilon Engineering Honor Fraternity, the Florida
Engineering Society, and serves as vice-chairman of the City of Palm Bay Board
of Adjustments.
A Florida native and a graduate of Florida Tech, with a Bachelor of Science in
Civil Engineering, Mr. Moia started his engineering career at BRPH, a full service
A/E firm. specializing in large government contracts with the Armed Forces,
including base improvements at Guantanamo Bay, Cuba and Cosimo, Italy, as
well as a key civil personnel for the construction of six elementary schools for the
Brevard County School Board.
Bruce joined Mosby and Associates, Inc. in 1987 as a design technician
fesponsible for drafting of numerous development plans including the Main Street
realignment in Sebastian, Barber Street Park improvements, Fischer Lake Island,
and Garden Grove subdivisions.
In 1992, as Vice'President of Civil Engineering with MAI Architects Engineers in
Melbourne, he was responsible for design, permitting, and construction
administration of numerous development projects such as Intracoastal
Marina/Healthsouth Office Tower, Nissan of Melbourne Eye Associates.
In 1996, Mr. Moia joined the City of Melbourne as the development engineering
responsible for review, approval, and construction administration of all
development projects in the City limits. In 1998, he joined Brevard County as the
County Development Engineer responsible for review and approval of all
development projects in the unincorporated areas of Brevard County, as will as
management of the Land Development Department, which included project
management, engineering review and engineering inspection.
Currently, as owner, vice president, and senior engineer for Mosby and
Associates, Inc., Mr. Moia supervises two civil engineers, a civil designer, and
staff of technical support professionals for the civil branch of Mosby and
Associates, Inc.
JEFFREY M. SMITH
CIVIL ENGINEER
Mr. Smith is a civil engineer for Mosby & Associates, Inc. with his area of expertise
being in permit drawings for the Florida Department of Transportation, the Department
of Natural Resources and the Department of Environmental Regulation. He is also
experienced in civil engineering drawings for roadways and bridge design. A graduate
of Pennsylvania State University with a Bachelor of Science in Civil Engineering, he has
worked on civil engineering projects from conceptual ideas to final design permits. He
has performed civil engineering work for the Cities of Sebastian, Vero Beach, Fellsmere,
Ft. Pierce and the Counties of Brevard, Indian River, Martin and St. Lucie, Florida.
Mr. Smith has project experience for the Florida Department of Transportation permit
drawings for right-of-ways, parking lot layout and design. He has prepared structural
drawings for steel, concrete and masonry structures with additional emphasis on bridge
design. Past projects include drawings for roadway design, large subdivisions, industrial
parks, coastal projects and site plans.
GEOFFREY K. BARKETT
CIVIL ENGINEER
Mr. Barkett is a civil engineer for Mosby & Associates, Inc. with his area of
expertise being obtaining project approval from various local, state, and federal
jurisdictional agencies. He is also experienced in civil engineering drawings for
subdivision, roadways, plats, stormwater, and site plan designs. A graduate of
Appalachian State University with a bachelor's degree in finance and University of
Central Florida with a bachelor's degree in civil engineering, he has worked on
civil engineering projects from conceptual ideas to final design. He has
performed civil engineering work for the Cities of Sebastian, VeroBeach, Ft.
Pierce and the Counties of Indian River and St. Lucie, Florida.
Mr. Barkett has project experience for the Florida Department of Transportation
permit drawings for right-of-ways, vehicular access, and drainage design. Past
projects include drawings for roadway design, large subdivisions, industrial
parks, coastal projects and site plans.
RODOLFO VILLAMIZAR
PROJECT MANAGER
Rodolfo Villamizar has experience in design and supervision of buildings and house
construction mainly on reinforced concrete. Mr. Villamizar also has experience in
construction and maintenance of roads and bridges, with vast experience in bridges and
geotechnical studies. A graduate from the University Fransisco de Paula Santander in
Cucuta, Colombia, with a Master of Civil Engineering in Structural Engineering from the
University of Los Andes in Bogota, Colombia. Mr. Villamizar is extremely well versed in
his field of specialization, he has been preparing structural designs of bridges and
commercial buildings over the past ten years. Mr. Villamizar also possesses an
Advanced Degree in Environmental Management Engineering in the field of geotechnics
and has acted as Professor on structures for the Universities of Fransisco de Paula
Santander and Antonio Nadno in Cucuta, Colombia.
Mr. Villamizar has worked as Technical Director of the Cucuta's Metropolitan Office,
where duties included being responsible for the projects developed by the office within
that time which such as bridges, roads, sports fields and schools. As a specialized
professional for the Colombian National Road Department, he was in charge of
overseeing the maintenance of over one hundred miles of national roads, responsible
for structural inspection of more than five hundred bridges and supervisor of several
road construction and design projects.
Mr. Villamizar is working for Mosby and Associates, Inc. as a Structural Engineer for the
past two years, his responsibilities include but are not limited to, analysis and design of
various residential and commercial/industrial projects. He is also responsible for
structural inspections for concrete restoration and upgrade of structures. Mr. Villamizar
possesses widespread knowledge_ of the New Building Codes, particularly hurricane
conditions.
WILLIAM J. ARTHUR
SENIOR ENGINEERING TECHNICIAN
Mr. Arthur is a Senior engineering Technician for Mosby and Associates, Inc. with
expertise in permit drawings for the Florida Department of Transportation and the
Florida Department of Environmental Protection. He is also experienced in civil
engineering drawings for roadways and waste water treatment facilities. A graduate of
Dean Institute of Technology with an Associates Degree in Construction Engineering, he
has worked on civil engineering projects from conceptual ideas to final design permits.
He has preformed civil engineering work for the City of Sebastian and the City of Vero
Beach, Florida.
He also has field inspection experience in sewer and water main, drainage, roadway
and site grading. Mr. Arthur is experienced in landfill liner inspection and is Troxier
certified for field compaction tests.
Mr. Arthur has project experience for the Florida Department of Transportation permit
drawings for rights -of -ways, and parking lot layout and design. Past projects include
drawings for roadway design, large and small subdivisions, industrial parks and coastal
projects.
ANGEL MIRANDA
SENIOR ENGINEER TECHNICIAN
Mr. Miranda has worked with Mosby & Associates, Inc. since 1997. He has been
involved in a wide variety of large and small-scale projects. He currently works as our
computer/AUTOCAD workstation manager. Mr. Miranda is proficient in overseeing all
operations relating to structural design and construction documents. He holds an
Associates Degree in Architectural Design. He currently works in our structural
department as a structural technician. He is also one of our field inspectors for
structural and concrete restoration projects. He also oversees all construction draw
requests for such projects. He has prepared structural drawings for steel, concrete and
masonry structures. Mr. Miranda is well versed in the current changes to the Florida
Building Code.
ROBERT W. BLANKS
SENIOR ENGINEER TECHNICIAN
Robert W. Blanks has extensive experience with AutoCAD drafting and Design in Civil
Engineering, Surveying and Architectural applications. Mr. Blanks has experience in
Subdivision construction, Roadway construction, Water & Sewer systems construction,
Site Planning, and Surveying & Mapping.
Mr. Blanks is a graduate of Brevard Community College with both an Associates of
Science in Drafting Technology and an Associates of Arts in General Studies. As an
Engineering Tech. with the City of Palm Bay, Mr. Blanks has worked on projects
including Emerson Road N.W. Roadway Improvements, I-95 & C-1 Canal Bike Path and
other Capital Improvement projects for the City of Palm Bay. These projects consisted
of roadway and drainage designs along with preparations of construction documents
and quantities estimations. Mr. Blanks also has experience in producing survey maps,
residential site plans and structural plans.
Currently working for Mosby and Associates, Mr. Blanks is in his seventeenth year in
working with AutoCAD, and other various programs to produce clear and accurate plans
for various new subdivisions and site plans.

Any person submitting a quote, bid, or proposal in response to this invitation or a
contract, must execute the enclosed form PUR 7069, sworn statement under section
287.133(3)(a), FLORIDA STATUTES ON PUBLIC ENTITY CRIMES, including proper
check(s), in the space(s) provided, and enclose it with his quote, bid, or proposal. If you are
submitting a quote, bid or proposal on behalf of dealers or suppliers who will ship
commodities and receive payment from the resulting contract, it is your responsibility to see
that copy(ies) of the form are executed by them and are included with your quote, bid, or
proposal. Corrections to the form will not be allowed after the quote, bid, or proposal

The 1989 Florida Legislature passed Senate Bill 458 creating Sections 287.132 - 133,
Florida Statutes, effective July 1, 1989. Section 287.132(3)(d), Florida Statutes, requires the
Florida Department of General Services to maintain and make available to other political
entities a "convicted vendor" list consisting of persons and affiliates who are disqualified
from public contracting and purchasing process because they have been found guilty of a
public entity crime. A public entity crime is described by Section 287.133, Florida Statutes,
as a violation of any State or Federal law by a person with respect to and directly related to
the transaction of business with any public entity in Florida or with an agency or political
subdivision of any other state or with the United States, including, but not limited to, any bid
or contract for goods or services to be provided to any public entity or with an agency or
political subdivision and involving antitrust, fraud, theft, bribery, collusion, racketeering,
conspiracy, or material misrepresentation.
By law no public entity shall accept any bid from, award any contract to, or transact
any business in excess of the threshold amount provided in Section 287.017, Florida Statutes,
for category two with any person or affiliate on the convicted vendor list for a period of 36
months from the date that person or affiliate was placed on the convicted vendor list unless
that person or affiliate has been removed from the list pursuant to Section 287.133(3)(f),
Florida Statutes.
Therefore, effective October 1, 1990, prior to entering into a contract (formal contract
or purchase order) in excess of the threshold amount for category two, to provide goods or
services to THE CITY OF SEBASTIAN, a person shall file a sworn statement with the
contracting officer or Purchasing Director, as applicable. The attached statement or affidavit
will be the form to be utilized and must be properly signed in the presence of a notary public
or other officer authorized to administer oaths and properly executed.

DRUG-FREE WORKPLACE FORM
The undersigned Contractor, in accordance with Florida Statute 287.087 hereby
certifies thatHolland Bldrs.of S.Fla.does:
1. Publish a statement notifying employees that the unlawful manufacture,
distribution, dispensing, possession, or use of a controlled substance is
prohibited in the workplace and specifying the actions that will be taken
against employees for violations of such prohibition.
2. Inform employees about the dangers of drug abuse in the workplace, the
business' policy of maintaining a drug-free workplace, any available drug
counseling, rehabilitation, employee assistance programs and the penalties
that may be imposed upon employees for drug abuse violations.
