Robert, the attached quote specified 61 phone numbers for PinPoint. I understand from Karen Clawson that you have
253 numbers in total that you would like included in the service. Given a rate of $0.05 per TN, this will increase the
previously quoted rate by $9.60 per month, for a total of $107.60/month.
With you approval, Karen can proceed with adding all 253 TN's to your Pinpoint account.

Customer agrees to purchase the Service in the quantities and according to the prices and terms and conditions set forth in
this Pricing Schedule/Letter of Election and in the applicable Service Publication. In jurisdictions that require the Service to be
provided pursuant to tariff, the relevant Service Publication is the applicable Tariff; in jurisdictions that do not require the

1. Prices in this Pricing Schedule are exclusive of, and Customer will pay, all current or future taxes, regulatory surcharges,
recovery fees, shipping charges, and other similar charges specified or allowed by any governmental entity relating to the
sale, use, or provision of the Service.
2. AT&T will furnish, install, maintain and provide maintenance of the Service in accordance with AT&T's applicable Tariff or
Guidebook. The service period shall begin on the date Service is installed and billing begins ("Service Period"), unless
specified otherwise in the tariff.
3. State Specific Terms:
Tennessee
If Services are provided under this Pricing Schedule in the State of Tennessee, the following provision shall apply;
otherwise, it shall not:
Customer and AT&T acknowledge and agree that to the extent the services to which Customer subscribes under this
Pdcing Schedule constitute a "bundle or combination of products or services" under Tennessee Senate Bill
182/House Bill 593, effective June 1, 2005, all references to "AT&T's General Subscriber Services Tariff," "AT&T
tariffs," "AT&T's lawfully filed tariffs" or any other reference to AT&Ts tariffs on file with the Tennessee Regulatory
Authority shall be deemed references to agreed contract terms and conditions identical to those set forth in the
applicable tariff(s) for the services included within the Customer's bundle or combination, as such tariffs existed on
May 31, 2005, and which are on file with the TRA. Such tariffs are incorporated herein by reference as if included
fully herein and can be reviewed at www.att.com/servioepublications.
4. If Customer terminates the Service prior to the date Customer's obligation to pay for the Service begins, Customer will
reimburse AT&T for time and materials incurred prior to the effective date of termination, plus any third party charges
resulting from such termination.
5. Customer shall return all security tokens associated with the Service to AT&T or AT&T's third -party provider upon
termination of Service.
6. Customer agrees to pay any added costs incurred by AT&T due to a Customer initiated change in the location of the
requested Service prior to the time the Service is installed.
7. Should the Customer desire to add or change Service, the Company agrees to do so under the terms and conditions of
the Tariff or Guidebook and the Customer agrees to pay any additional charges resulting from the change or addition.
8. Suspension of Service is not permitted for the requested Service.
9. Service may be transferred to another Customer at the same location upon prior written concurrence from the Company.
The new Customer to whom the Service is transferred will be subject to all tariff provisions and equipment configurations
currently in effect for the present Customer.

10. The Subscriber is responsible for assuring that its authorized users comply with the provisions of these terms and the
tariffs and that unauthorized persons do not gain access to or use the Services through user names, passwords, or other
identifiers assigned to Subscriber pursuant to these terms. Specifically, Subscriber must keep User IDs, passwords, and
any security token(s) that may be provided secure from use by any unauthorized individual. Subscriber shall also not use
the Services in any way that would be or would assist any third party to be in violation of any law or these terms.
Subscriber shall comply with all applicable laws, rules, and regulations in connection with the Services. Subscriber shall
not infringe upon any intellectual property rights contained in the product, including any associated programs, designs,
materials, and trademarks. Subscriber shall provide such information and assistance as are reasonably requested by
AT&T for purposes of facilitating AT&T's provision of Services to Subscriber. Finally, Subscriber shall be liable to AT&T
and its suppliers for any damages related to violations of this agreement.
11. The Service Attachment describes the service period, monthly recurring charges, and non-recurring charges for the
quantity of Service the Customer has ordered. In the event that any extended or total amounts in this Pricing Schedule or
the Service Attachment conflict with any per -component rales in the Service Attachment, the per -component rates shall
control.
9-1-1 Pinpoints Service Technical Service Requirements
1. 9-1-1 PInPoine service is available only with Private Branch Exchange (PBX) service employing AT&T ILEC DID
station numbers; it is not available with any Centrex or "Centrex -like" telephone numbers.
2. a) 9-1-1 PinPoin? BellSouth Direct Inward Dialing (DID) service is configured on a "per account" basis. All
telephone numbers on an AT&T ILEC DID account equipped with 9-1-1 PinPoine service must be provisioned with
9-1-1 PinPoine service. The Subscriber cannot randomly select which telephone numbers on an account will have
the service. If multiple billing accounts are configured into one 9-1-1 Pinpoint® service arrangement, all telephone
numbers for all billing accounts will be aggregated into one charge and billed on one primary billing account.
b) 9-1-1 Pinpoints per PRI station number (per TN) is configured on a "per TN" basis. If multiple billing accounts
are configured into one 9-1-1 Pin Point® service arrangement, all telephone numbers for all billing accounts will be
aggregated into one charge and billed on one primary billing account.
3. The PBX customer is responsible for providing telephone number and address data to the service for all telephone
numbers assigned to each billing account, even if the telephone number is in an unassigned status.
4. Users of 9-1-1 PinPoine service are prohibited from provisioning PBX station numbers outside the boundary of the
E9-1-1 tandem serving the physical address of the main PBX location. 9-1-1 calls from any telephone numbers
provisioned outside the boundary of the serving E9-1-1 tandem would be routed to an incorrect Public Safet�
Answering Point (PSAP) with no location information. The subscriber is responsible for ensuring that 9-1-1 PinPomt
service is provisioned in a compliant configuration that will ensure that 9-1-1 calls are routed correctly with the correct
location information.
5. It is the Customer's responsibility to comply with hardware, software, and operating system requirements, including
the appropriate Internet browser to access and maintain the Service.
