Dear Sir/Madam,
Our records indicate GREG KISEDA was issued the above
Consumptive Use Permit by St. Johns River Water Management
District for PUBLIC SUPPLY on June 9, 1987. The expiration
date on this permit is June 9, 1994. For your convenience, we
are enclosing an application form for your use in submitting an
application for renewal. In addition, if you would like
assistance in completing this application form you may contact
this office and we will be happy to assist you. A meeting can be
arranged on-site or in one of our four offices.
Please submit the enclosed application, along with the
appropriate fee as listed on the enclosed attachment, so that
your permit will continue and you will not be in violation of
Chapter 40C-2. Your application for renewal must be received by
this agency no later than 5:00 pm on the date of expiration. If
your submittal is not received by June 9, 1994, you will then
have to re -apply to the District as a new applicant for a new
permit in order to resume use.
