Bin Services of Vero, Inc. will start performing their services on Harch 1,1995. There is no termination date. Monthly MOF: and analytical reports will be certified and submitted to the respective DEP District Office in Orlando as prescribed in FAC Chapters relating to Certified Operation of Community Public Drinking Water- Facilities. 2- Daily supervision of the wastewater treatment to include but not be limited to, operational surveillance and recommendation to maximize plant efficiency while maintaining compliance requirements. Monthly MOR and analytical reports will be certified and submitted to the respective DEP District office in Orlando as prescribed in the FAC relating to Certified Operation of a Community Public: Wastewater Facility. The total monthly costs for the above services will be $1,060. Emergency service will be billed at $20/hour during normal business hours with charges for weekend and holiday emergency hours being $30 and `f+40, 'respec'tively. Chemical and general maintenance costs are not included. Repair=s and maintenance requirements will be brought to your attention as the need arises for discussion and determination of corrective action. Additional services not mentioned above which are preformed by Bio --Services at your request will be billed at prevailing rates as agreed upon prior to initiation. This agreement will remain in effect until cancelled by either party by submission of written notification 14 days prier to termination date. Finally, it is understood that this proposal is sufficient to comply with the present certified operator- requirements as set forth in the FAC as reflected in your current operational permits. FROM: Richard B. Votapka, Utilities Director SUBJECT: Proposed Improvements at Water and Wastewater Treatment Plant as a result of Tropical Storm Gordon. Based on our observations during last week's intense rainfall from Tropical Storm Gordon, I recommend that the following improvements be made as soon as possible: 1. Water Treatment Plant - Construct small circular earth berms approximately 3" high around the wet and dry wells for the septic tank lift station. This will prevent stormwater runoff from flooding the system causing it to become saturated to the point where the toilet in the water plant office/lab is rendered inoperable. 2. Wastewater Treatment Plant - a. Grade a swale from the blower/control panel area north towards the chlorine shed and then east towards Lake Schumann to adequately drain the area. b. Construct a wooden platform (pallet type) or raise the existing slab with 3" of concrete overlay. Both A and B will ensure that the area in the immediate vicinity of the control panel and blowers will not be submerged during intense rainfall. As was the case with the blower motor that shorted out because of improper grounding and conduit connections, a short to the conduit could transmit electricity to the water which would electrocute anyone who stepped into the water. Votapka, Utilities Director )x/ SUBJECT: Waterborne Diseases After attending the AWWA Teleconference on April 18, 1994 entitled "Is Your System At Risk" regarding waterborne diseases, I called Glenn Schuessler, Assistant Director at the Indian River County Environmental Health Department to discuss outbreaks of cryptosporidium and giardiasis. Glenn told me that he know of two (2) cases of crypto and seven (7) of giardiasis which were found locally in private wells. He said that the test is a long, laborious process which utilizes a diamataceous earth filter. He knew of now local laboratories that would check for this. Glenn said that the Health Department normally relies on testing for E. Coli bacteria, which is a good organism indicator for pathogenic bacteria which might be in the water supply. They crypto and giardiasis organisms were protozoan cysts, not bacteria. My concern was the water in the backwash holding lagoon that we reuse in our potable water treatment process. Glenn suggested that we check the coliform bacteria level as soon as possible. We may find that it is too risky using this water in the finished water process. I called Ron Briskie, Plant Supervisor, and asked him to test the water in the backwash lagoon as soon as possible and let me know the results. AIN D :J Pj DATE: October 11, 1993 TO: GDU Acquisition File FROM: Richard Votapka, Utilities Director SUBJECT: Environmental Assessment of Underground Fuel Tanks at the GDU - Sebastian Highlands Water and Sewer Plants Charles Vogt of the Environmental Health Department returned my call from October 6th in regard to the underground diesel fuel tanks for the emergency generators at GDU's water and wastewater plants. Charles said he made a visual inspection of the sites only. He did not take any borings. He didn't see any evidence of diesel fuel contamination at the 6000 gallon tank at the water treatment plant. He didn't find the tank at the wastewater plant. There were no vent pipes, filler pipes, or pipes to the generator room at the plant. Charles said that the DHRS required GDU to install monitoring wells at the GDU Filbert Street water plant by January 1, 1994. He recommended four (4) monitoring wells which cost between $400.00 - $500.00 each. Charles suggested installing a temporary tank (4,000 gallon) above the ground. By doing so, an extra 180 days is afforded to the owner to have something done. In 1998, the installation of which should be tank "law" requires an upgrade. Charles suggested a double wall, above ground tank, the volume of enough capacity for a 10 day run of the generator. 2 HJ"Ir?-A'AD'114 DATE: November 2, 1993 TO: Kay O'Halloran, Acting City Manager/City Clerk op FROM: Richard Votapka, Utilities Director /,,P V SUBJECT: Water Treatment Plant Operator for the GDU Water Treatment Plant on Filbert Street, Sebastian, Florida Unfortunately, the three GDU - Sebastian Highlands Division employees to whom the City has extended offers of employment do not have water treatment plant operator licenses. Two of them have their Class "C" wastewater treatment plant operator licenses. Once employee, Tom Stirtzinger, will be taking the water treatment plant operator license test on November 8. I contacted Ronald in the Bureau of Testing at the Florida Department of Professional Regulation (904-487-9771) to learn when the test results will be made public. He informed me that he will release the test results by mail on December 9, 1993. I explained my dilemma to him concerning the GDU water treatment plant takeover on December 17, 1993 and not knowing whether or not Tom will have his license by then. Information concerning whether or not an applicant has passed or failed the exam will not be given to anyone who calls the DPR regardless of circumstances. As a matter of assurance to guarantee a certified "C" licensed operator at the water treatment plant, I have contacted Bio Services of Vero Beach, Inc. and met with Jane Burton today. She proposes to charge the City on a time and material basis at a rate of $20.00 per hour. I have scheduled a meeting with her and the GDU employees on November 9, 1993 to review the plant operation. A 5' by 9 ' ramp for the shed housing the small tractor at the wastewater treatment plant. 2. A 2' by 7' slab for the chlorine room at the wastewater treatment plant. 3. A 2' by 3-1/2' ramp for the chlorine room at the water treatment plant. Since your department did such a nice job on the slab at the Park Place Water Treatment Plant, 2'd like them to do these three small jobs.
