Box 780127 Sebastian, Florida 32978-0127 ATTN: Larry Napier, Assistant Finance Director Re: Water Treatment Plant Operation cj4kqt% As verification of our conversation. of August 22, 1988, this is a written statement summerizing the verbal agreement under which Bio -Services has been providing certified operator services for the water treatment plant at City Hall since 1984: Operation of one Category V Class D Water Treatment Plant will be on a two (2) day per week non-consecutive on-site inspection basis in accordance with the directives as outlined in Florida Administrative Code Chapters 17:16, 19, and 22. Plant visists will include, but not be limited to, chlorine (free) residual testing on plant effluent and remote tap, operational surveillance and adjustment to maximize plant efficiency. Also, monthly Total Coliform Bacteria testing on the well (raw) and remote taps (2) being included. Monthly Operating Reports(see attached copy) as well as Bacteriological reports( see attahced copy) will be completed, certified, and forwarded to the local DHRS Environmental Health Unit as well as the DER --Orlando District Office as prescribed by the FAC Chapter 17:16 and 17:22, with copies to the client. Chemicals (liquid chlorine) and general repairs and maintenance costs are not included above. Bio -Services will be responsible for ordering the chlorine on an as needed basis. General repairs and maintenance requirements will be brought to the attention of you and/or your designated representative as the need arises for discussion and determination of corrective action. Additionally, Bio -Services will be responsible for flushing the hydropneumatic tank periodically but no less than three times yearly. The monthly costs for the bi-weekly visits and tank flushings plus the bacteriological sampling , analysis and reports submission will be $150.00;month. The costs for the liquid chlorine will be invoiced monthly when applicable. It is also understood that this agreement is sufficient to comply with the present requirements set forth by the DER and the DHRS--Environmental Health Unit in Indian River County. Our employees and owner are local residents and our reputation is build on the local utilities we operate and the local utility owners we serve. The construction division manager reports his operations and reprioritizes for unscheduled system maintenance or engineering repairs. Every morning the president is on the radio with company maintenance crews and sludge truck drivers to assure overnight outages have been corrected and operations are on schedule. Schedule - HUS will assure the proper operation of the three named plants 24 hours per day. Actual licensed operator time will be 7.5 hours/day total for all three installations. Our maintenance and repair crews are available on a round the clock basis for emergency repairs. We would propose that the operating agreement be for 3 years with a price index escalation clause effective annually. (HUS will supply a standard contract form for the County's use, if desired). October 1, 1995, has been designated as the takeover date by the County Utilities Department to begin operation of the systems. Therefore, please be advised that your services in reference to the above based on the February 22, 1995 mutual agreement between your firm and the City are to be terminated on the close of September 30, 1995. The City of Sebastian Utilities Department thanks you for your operational services since March 1 of this year when our last licensed water and sewer operator resigned pending the sale of the utility systems to Indian River County.
