THE COST OF ALL TESTINGS REQUIRED BY THESE PLANS & SPECIFICATIONS SHALL BE INCLUDED WITH THE LUMP SUM BID AND NO ADDITIONAL COMPENSATION SHALL BE MADE BY THE OWNER. 9. UNLESS OTHERWISE SHOWN, THE CONTRACTOR SHALL REPLACE ALL PAVING, SODDING, STABILIZED EARTH, LANDSCAPING, DRIVEWAYS, SIDEWALKS, IRRIGATION SYSTEMS, UNDERGROUND PIPING, ETC. LOCATED WITHIN AND OUTSIDE THE CONSTRUCTION LIMIT LINE DISTURBED OR DAMAGED BY THE CONSTRUCTION. THE CONTRACTOR SHALL REPLACE WITH EQUAL MATERIAL OR AS DIRECTED BY THE ENGINEER. 10. WHERE A CONNECTION IS TO BE MADE IN THE FIELD TO AN EXISTING CONDUIT, THE CONTRACTOR SHALL EXCAVATE THE AREA TO VERIFY THE TYPE OF CONDUIT, CONDUIT SIZE, AND REQUIRED CONNECTION AND FITTINGS AND OBTAIN THE OWNERS APPROVAL OF THE PROPOSED METHOD OF CONNECTION PRIOR TO INSTALLING MATERIALS. 11. THE CONTRACTOR SHALL COORDINATE AND VERIFY ALL DIMENSIONS OF ALL EXISTING AND PROPOSED FACILITIES PRIOR TO FABRICATION OF PIPING AND EQUIPMENT TO ENSURE PROPER ASSEMBLY OF ALL ITEMS. MAINTENANCE OF TRAFFIC SHALL BE IN ACCORDANCE WITH THE MANUAL OF UNIFORM TRAFFIC CONTROL DEVICES PART VI, WORK ZONE TRAFFIC CONTROL. THE CONTRACTOR WILL PROVIDE A MAINTENANCE OF TRAFFIC PLAN TO THE ENGINEER PRIOR TO CONSTRUCTION. TRAFFIC CIRCULATION WITHIN EXISTING PARKING LOT AREA MUST BE MAINTAINED DURING CONSTRUCTION. 15. COMPLETE RECORD DRAWINGS, CONFORMING TO THE CITY OF SEBASTIAN STANDARDS SHALL BE COMPILED DURING CONSTRUCTION FOR PERIODIC REVIEW. FINAL RECORD DRAWINGS, CERTIFIED BY A FLORIDA LICENSED REGISTERED LAND SURVEYOR SHALL BE SUBMITTED TO THE ENGINEER IN DIGITAL FORMAT PRIOR TO FINAL INSPECTION AND ACCEPTANCE ON ANY PART OF THE PROJECT. 16. LOCATIONS AND DIMENSIONS OF EXISTING RIGHT-OF-WAY AND EASEMENTS ARE BASED ON BEST AVAILABLE INFORMATION. THE CONTRACTOR SHALL VERIFY THE LIMITS OF THE RIGHT-OF-WAY AND EASEMENTS IN ORDER TO AVOID ENCROACHMENTS. 17. THE CONTRACTOR SHALL MAKE EXPLORATORY EXCAVATIONS AT ALL INTERSECTIONS OF PROPOSED WORK AND EXISTING UTILITIES. THE EXPLORATORY EXCAVATIONS SHALL BE MADE SEVENTY-TWO (72) HOURS IN ADVANCE OF THE WORK. IF THERE IS A POTENTIAL CONFLICT, THE CONTRACTOR IS TO NOTIFY THE CITY OF ANY OBSTRUCTION. 18. ELEVATION DATUM IS LOCAL DATUM. 19. IN AREAS WHERE CONSTRUCTION ACTIVITIES RESTRICT NORMAL ACCESS TO PROPERTIES, THE CONTRACTOR SHALL PROVIDE AND MAINTAIN ALTERNATIVE ACCESS ROUTES, WHICH ARE SUBJECT TO APPROVAL BY THE ENGINEER. 20. THE CONTRACTOR SHALL NOTIFY THE CITY AND IRC UTILITIES 72 HOURS IN ADVANCE OF ANY WATER MAIN OR SANITARY FORCE MAIN SHUT -DOWN. 21. ALL EXCAVATIONS SHALL BE BACKFILLED AT THE END OF EACH WORK DAY. 22. THE CONTRACTOR SHALL NOTIFY FIBER OPTIC COMPANIES SEVEN (7) DAYS PRIOR TO .ANY CONSTRUCTION. EXTREME CAUTION SHALL BE EXERCISED IN AREAS OF FIBER OPTICS. 23. THE CONTRACTOR SHALL BE RESPONSIBLE FOR REPLACING DAMAGED CURBS, ASPHALT, LANDSCAPING, DRIVEWAYS AND ANY TEMPORARY REPAIRS OF OPEN CUTS. THESE ITEMS SHALL BE PAID UNDER GENERAL REQUIREMENTS. 24. THE CONTRACTOR SHALL MAINTAIN PEDESTRIAN TRAFFIC, I.E. TEMPORARY SIDEWALKS AROUND WORK AREAS. THE CONTRACTOR MAY HAVE TO RESTRICT WORK HOURS ON SIDEWALKS, WITH TEMPORARY FENCING BETWEEN CONSTRUCTION AND TEMPORARY SIDEWALKS, ETC. 25. ALL CONSTRUCTION CONTROL SHALL BE REFERENCED AND REPLACED BY A FLORIDA REGISTERED LAND SURVEYOR AFTER CONSTRUCTION. NOTICE TO DRAWING HOLDER DO NOT SCALE THESE DRAWINGS. IF ANY DISCREPANCIES OCCUR WITHIN THESE DOCUMENTS, THE CONTRACTOR SHALL CONTACT THE CITY OF SEBASTIAN FOR CLARIFICATIONS PRIOR TO CONSTRUCTION. REQUIRED DURING A CONSTRUCTION PHASE SHALL BE INSTALLED AND APPROVED BY THE CITY OF SEBASTIAN TRAFFIC OPERATIONS ENGINEER PRIOR TO THE COMMENCEMENT OF CONSTRUCTION AND WILL BE MAINTAINED IN ACCORDANCE WITH MUTCD, REFER TO INDEX 600. IF CASE IS NOT COVERED IN MUTCD, INDEX 600 SHALL BE USED IN CONJUNCTION WITH ALL OTHER INDEXES SPECIFICALLY MENTIONED IN EACH TRAFFIC CONTROL PHASE. 3. ALL DEVICES WILL BE IN GOOD CONDITION. MINIMUM REFLECTIVE INTENSITY SHALL MEET THE FDOT STANDARD SPECIFICATIONS FOR ROADS AND BRIDGE CONSTRUCTIONPER THE LATEST PUBLISHED EDITION. 4. INGRESS/EGRESS TO COMPLEX, MUST BE MAINTAINED THROUGHOUT CONSTRUCTION SCHEDULE. 5. THE CONTRACTOR SHALL SUBMIT A MOT PLAN SIGNED AND SEALED BY PE FOR CONSTRUCTION WORK ON CITY STREETS. EROSION CONTROL/WATER QUALITY NOTES 1. EROSION CONTROL AND SEDIMENTATION CONTROL DEVICES SHALL BE IN PLACE PRIOR TO BEGINNING ANY CONSTRUCTION. THEY SHALL BE INSTALLED, AT A MINIMUM, TO THE LIMITS SHOWN IN THE DRAWING, AS REQUIRED IN THE SPECIFICATIONS AND IN ACCORDANCE WITH ALL REGULATORY AGENCY REQUIREMENTS. THE COST TO THE CONTRACTOR FOR ALL EROSION AND SEDIMENTATION CONTROL IS INCLUDED IN THE LUMP SUM BID AND NO ADDITIONAL COMPENSATION SHALL BE MADE BY THE OWNER. 2• THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTENANCE OF ALL TEMPORARY EROSION CONTROL FEATURES. 3. THE CONTRACTOR IS TO PROVIDE EROSION/SEDIMENTATION CONTROL TO PREVENT SILTATION OF ADJACENT PROPERTY, STREETS, STORM SEWERS AND WATER WAYS. IN ADDITION, THE CONTRACTOR SHALL PLACE MULCH OR OTHER SUITABLE MATERIAL ON GROUND ON AREAS WHERE CONSTRUCTION -RELATED TRAFFIC IS TO ENTER AND EXIT SITE. IF, IN THE OPINION OF THE ENGINEER AND/OR LOCAL AUTHORITIES, EXCESSIVE QUANTITIES OF EARTH ARE TRANSPORTED OFF-SITE EITHER BY NATURAL DRAINAGE OR BY VEHICULAR TRAFFIC, THE CONTRACTOR IS TO REMOVE AND CLEAN SAID EARTH TO THE SATISFACTION OF THE ENGINEER AND/OR AUTHORITIES. AN EROSION CONTROL BARRIER SHALL BE ESTABLISHED AS SITE CLEARING PROCEEDS AND MAINTAINED UNTIL SUCH TIME AS VEGETATION OR OTHER CONTROLS BECOME EFFECTIVE_ 4. EXISTING DRAINAGE PATTERNS AND CAPACITIES ARE TO BE PROTECTED AND MAINTAINED DURING CONSTRUCTION TO ELIMINATE FLOODING AND PONDING WATER ON PRIVATE PROPERTY. THE CONTRACTOR TO PROVIDE TEMPORARY DRAINAGE MEASURES AS REQUIRED TO MEET THIS REQUIREMENT. 5. MINIMUM STANDARDS 1- SEDIMENT BASINS AND TRAPS, PERIMETER DIKES, SEDIMENT BARRIERS AND OTHER MEASURES INTENDED TO TRAP SEDIMENT SHALL BE CONSTRUCTED AS A FIRST STEP IN LAND -DISTRIBUTING ACTIVITY AND SHALL BE MADE FUNCTIONAL BEFORE UPSLOPE LAND DISTURBANCE TAKES PLACE. 2. SEDIMENT CONTROL MEASURES ARE TO BE ADJUSTED TO MEET FIELD CONDITIONS AT THE TIME OF CONSTRUCTION AND BE CONSTRUCTED PRIOR TO GRADING OR DISTURBANCE OF EXISTING SURFACE MATERIAL ON BALANCE OF SITE. PERIMETER SEIDMENT BARRIES SHALL BE CONSTRUCTED TO PREVENT SEDIMENT OR TRASH FROM FLOWING OR FLOATING ONTO ADJACENT PROPERTIES. 3. PERMANENT OR TEMPORARY SOIL STABILIZATION SHALL BE APPLIED TO DENUDED AREAS WITHIN SEVEN DAYS AFTER FINAL GRADE IS REACHED ON ANY PORTION OF THE SITE. TEMPORARY SOIL STABILIZATION SHALL BE APPLIED WITHIN SEVEN DAYS TO DENUDED AREAS THAT MAY NOT BE AT FINAL GRADE BUT WILL REMAIN UNDISTURBED FOR LONGER THAN 30 DAYS. PERMANENT STABILIZATION SHALL BE APPLIED TO AREAS THAT ARE TO BE LEFT UNDISTURBED FOR MORE THAN ONE YEAR. 4. SURFACE RUNOFF FROM DISTURBED AREAS THAT IS COMPRISED OF FLOW FROM DRAINAGE AREAS GREATER THAN OR EQUAL TO THREE ACRES SHALL BE CONTROLLED BY A SEDIMENT BASIN. THE SEDIMENT BASIN SHALL BE DESIGNED AND CONSTRUCTED TO ACCOMODATE THE ANTICIPATED SEDIMENT LOADING FROM THE LAND -DISTURBING ACTIVITY. THE OUTFALL DEVICE OR SYSTEM DESIGN SHALL TAKE INTO ACCOUNT THE TOTAL DRAINAGE AREA FLOWING THROUGH THE DISTURBED AREA TO BE SERVED BY THE BASIN. 5. AFTER SIGNIFICANT RAINFALL, SEDIMENT CONTROL STRUCTURES WILL BE INSPECTED FOR INTEGRITY. DAMAGED DEVICES SHALL BE CORRECTED BY THE CONTRACTOR IMMEDIATELY. 6. CONCENTRATED RUNOFF SHALL NOT FLOW DOWN CUT OR FILL SLOPES UNLESS CONTAINED WITHIN AN ADEQUATE TEMPORARY OR PERMANENT CHANNEL, FLUME OR SLOPE DRAIN STRUCTURE. 7. WHENEVER WATER SEEPS FROM A SLOPE FACE, ADEQUATE DRAINAGE OR OTHER PROTECTION SHALL BE PROVIDED. 8. SEDIMENT WILL BE PREVENTED FROM ENTERING ANY STORM DRAIN SYSTEM, DITCH OR CHANNEL. ALL STORMWATER INLETS THAT ARE MADE OPERABLE DURING CONSTRUCTION SHALL BE PROTECTED SO THAT SEDIMENT -LADEN WATER CANNOT ENTER THE CONVEYANCE SYSTEM WITHOUT FIRST BEING FILTERED OR OTHER WISE TREATED TO REMOVE SEDIMENT 9. UNDERGROUND UTILITY POWER LINES SHALL BE INSTALLED IN ACCORDANCE WITH THE FOLLOWING STANDARDS IN ADDITION TO OTHER APPLICABLE CRITERIA: (a) NO MORE THAN 500' LINEAR FEET OF TRENCH MAY BE OPENED AT ONE TIME. (b) EXCAVATED MATERIAL SHALL BE PLACED ON THE UPHILL SIDE OF TRENCHES. (c) EFFLUENT FROM DEWATERING OPERATIONS SHALL BE FILTERED OR PASSED THROUGH AN APPROVED SEDIMENT TRAPPING DEVICE, OR BOTH, AND DISCHARGED IN A MANNER THAT DOES NOT ADVERSELY AFFECT FLOWING STREAMS OR OFF-SITE PROPERTY. (d) RESTABLIIZATION SHALL BE ACCOMPLISHED IN ACCORDANCE WITH THESE REGULATIONS. CONSTRUCTION IN STREETS, ROAD RIGHT-OF-WAY & PARKING AREAS 1. OPEN ROAD CUTS REQUIRE PRIOR APPROVAL OF THE CITY, COUNTY, STATE OR ANY OTHER AGENCY WHICH MAY HAVE JURISDICTION. 2. CONSTRUCTION, MATERIALS AND WORKMANSHIP ARE TO BE IN ACCORDANCE WITH FLORIDA DEPARTMENT OF TRANSPORTATION SPECIFICATIONS AND STANDARDS. 3. AREAS IN EXISTING RIGHT-OF-WAYS DISTURBED BY CONSTRUCTION SHALL RECEIVE GRASS, MULCH OR SOLID SOD, AS STATED ON THE PLANS. 4. IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO VERIFY AND DOCUMENT THE ACTUAL LOCATION AND EXTENT OF EXISTING UTILITIES WHICH MAY AFFECT CONSTRUCTION. EXISTING UTILITIES SHOWN ARE BASED UPON THE BEST AVAILABLE INFORMATION SUPPLIED TO THE OWNER/ENGINEER AND IS SHOWN FOR INFORMATIONAL PURPOSE ONLY. 5.
