Pursuant to the above captioned contract and in compliance of same, we the undersigned do hereby certify
that this project is complete.
Issuance of this certificate releases the contractor from any further responsibility as stipulated in the contract
as it relates to bond performance, payment of subcontractors, release of liens, maintenance and indemnity. All
contractual obligations as to warranty, insurance, indemnification and patents, shall remain in full effect for the
contractually stipulated period of time. In no way does the issuing of this certificate release the Contractor from
all legal obligations as stipulated in Chapter 713 of the Florida Statutes.
Therefore, having reviewed the contract and inspected the work, the undersigned to hereby accept the
segment as being complete.

CONSULTANT SERVICE AGREEMENT (CSA)

Project Identification: City of Sebastian Community Center Bidding Assistance & Construction
Administration
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled NON-EXCLUSIVE CONTINUING
PROFESSIONAL SERVICES AGREEMENT FOR ENGINEERING dated July 12.2017.
Description of Assignment:
MBV Engineering, Inc. will provide bidding assistance and construction administration services
for the City of Sebastian's Community Center. The project will include:
I. Bidding Assistance will include:
a. Review bidder's questions and prepare required plan modifications.
b. Bid evaluation
c. Provide the City with bid selection recommendation
2. Construction Administration Services will include:
a. Pay Request and Change Order review and recommendation
b. Project inspection during construction
C. Project certification for Certificate of Occupancy
Basis of Compensation / Period of Services:
The above referenced Professional Engineering Services for CSA-1 will be performed on an hourly
rate basis, with a not to exceed fee amount of $12,500.
