Pursuant to the above captioned contract and in compliance of same, we the undersigned do hereby certify
that this project is complete.
Issuance of this certificate releases the contractor from any further responsibility as stipulated in the contract
as it relates to bond performance, payment of subcontractors, release of liens, maintenance and indemnity. All
contractual obligations as to warranty, insurance, indemnification and patents, shall remain in full effect for the
contractually stipulated period of time. In no way does the issuing of this certificate release the Contractor from
all legal obligations as stipulated in Chapter 713 of the Florida Statutes.
Therefore, having reviewed the contract and inspected the work, the undersigned to hereby accept the
segment as being complete.

Project Identification: Oyster Pointe Drainage Connection Survey and Design
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
It is our understanding that you wish to install a new baffle box to combine multiple stormwater pipes
currently outfalling into an open ditch located along Indian River Drive, just north of Capt. Hiram's. As
such, please see below for our proposed scope outlining the proposed survey, civil design, and permitting
services for the proposed improvements.
Work Scope:
Survey
• Establish East and West right of way lines for Indian River Drive in area of site.
• Establish a minimum of (2) horizontal and vertical control points at or near site.
• Depict right of way lines on survey.
• Locate pavement abutting site with elevations at both edges and center.
• Provide sufficient existing ground elevations to establish current drainage patterns.
• Locate all drainage structures and culverts with size, material and invert, lying within or serving site.
• Locate all aboveground utilities within or serving site which may be an obstruction or cause a
conflict.
• Locate significant desirable trees within right of way limits and 10 feet beyond.
• Provide 6 certified copies of the topographic survey.
• Provide PDF file or CAD file if requested.
Civil Design / Permitting
• Verify flows from existing pipe network.
• Size proposed baffle box for maximum flows with minimal head loss.
• Provide drainage connection plans and details.
• Submit to City of Sebastian and FDOT for drainage permits.
• Assist City with bidding process.
• Construction Administration.
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA 3 will be performed for a fee not to
exceed the amount as follows, including direct costs:
