Project Identification: Sebastian Cemetery Expansion - Additional Services
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
MBV Engineering, Inc. will provide additional survey and engineering services for the expansion of the
City of Sebastian's cemetery. The additional project scope will include:
1. Obtain additional topography, to include existing retaining wall data, as necessary to complete
review.
2. Structural analysis of the existing retaining wall to include an on -site inspection of the wall and
preparation of a findings report.
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA 15 ADD will be performed for a fee not
to exceed the amount of $2,550.
