CONSULTANT SERVICE AGREEMENT (CSA)

Project Identification: Sebastian Cemetery Expansion Additional Services - Revision of ADD 2 & 3
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
MBV Engineering, Inc. will provide additional engineering services for the northern expansion of the City
of Sebastian's cemetery. The purpose of this CSA is to provide revisions to ADD 2 and 3, and shall have
precedence. The additional project scope will include:
1.0 Preliminary Engineering
1.1 Surveying
Scope: Update survey with topography after sand pile has been removed. (Previously
completed under ADDI)
2.0 Final Engineering
2.1 Civil Design
Scope:
• Site plan design services and preliminary plat plan based on the original design scope.
• Combination of multiple electronic files to create a master electronic file for the
cemetery.
• Site plan design services for the modified development of the required cemetery
roadways for Area 5 of the City's Cemetery, including pavement requirements and
details.
2.2 Landscape and Irrigation Design
Scope:
• Landscape design services in accordance with the City's Land Development Code for
Area 5 of the City's Cemetery. Design shall include landscape buffer design for the
following areas:
• West property line
• North property line
• Fast property line
• Irrigation design for Area 5 of the City's Cemetery, including landscape buffers along
the west, north, and east property lines.
2.3 Structural Design
Scope:
• Structural design of the proposed memorial monument wall's foundation.
• Structural analysis of the existing retaining wall to include an on-site inspection of the
wall and preparation of a findings report. (Task has already been completed)
3.0 Permitting
Per City of Sebastian, this project only requires administrative review and approval. No external
agency review required.
4.0 Contract Administration and Inspections
4.1 As -Built Survey
Scope: Perform an as -built survey of Area 5 of the City's Cemetery to include all roadways,
memorial walls, plot delineations, as specified by the City.
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA -15 ADD 2/3 will be performed for the
following breakdown of fees:
1.0 Preliminary Engineering
1.1 Surveying
2.0 Final Engineering
2.1 Civil Design
2.2 Landscape and Irrigation Design
2.3 Structural Design
3.0 Permitting
4.0 Contract Administration and Inspections
4.1 As -built Survey
