that this project is complete.
Issuance of this certificate releases the contractor from any further responsibility as stipulated in the contract
as it relates to bond performance, payment of subcontractors, release of liens, maintenance and indemnity. All
contractual obligations as to warranty, insurance, indemnification and patents, shall remain in full effect for the
contractually stipulated period of time. In no way does the issuing of this certificate release the Contractor from
all legal obligations as stipulated in Chapter 713 of the Florida Statutes.
Therefore, having reviewed the contract and inspected the work, the undersigned to hereby accept the
segment as being complete.

Project Identification: Stormwater Management Plan Update - Stonecrop
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
As per our coordination, the City of Sebastian is looking to have the Stormwater Management Plan
updated for the Stonecrop basin area in conjunction with the FEMA grant application requirements.
Work Scope:
Task l: Preparation of an H&H (Hydrology & Hydraulics) study of approximately 120 nodes for
the Stonecrop basin, to include:
• Assistance with estimating the Finished Floor Elevations within the basin
• Pre and Post Developed length of flood duration determination
• Estimate of repair / replacement cost for roadways per FDOT Guidelines
• Model the following storm events:
0 25 year- 24 hour and 72 hour
0 100 year - I hour, 24 hour, and 72 hour
Task 2: Preparation of a response to both the FEMA Environmental & Engineering comments
Task 3: Preparation of an aerial map showing all drainage improvement activities, including the
beginning/end coordinates in decimal degree format
Task 4: Preparation of a ground disturbance map showing the total area of ground disturbance in
square feet and vertical depth in feet. Include the coordinates in decimal degree format of
the exact location of all areas of ground disturbance and staging areas
Task 5: Contingency for additional responses
Deliverables: H & H Study, Response to FEMA comments, Aerial Map, and Ground Disturbance Map
- Response to FEMA engineering comments to be no later than 30 days upon authorization
- Response to FEMA environmental comments to be by 2/22/19.
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA 18 will be performed for a fee not to
exceed the below breakdown of fees:
