Project Identification: Sebastian Cemetery Expansion Additional Services
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
MBV Engineering, Inc. will provide additional engineering services in order to address the comments
provided by the reviewers for the City of Sebastian's cemetery. The purpose of this CSA is to provide the
additional services necessary to address comments from the City dated May 12, 2020. The additional
project scope will include:
1.0 Civil Design
Scope:
• Update plans regarding the existing roadway widths, as well as providing emergency vehicle
access of 14'.
• Address working hours within notes on sheet C2.
• Coordination with surveyor for tree locations, add existing trees to be saved to the site plan,
and provide both tree protection and tree protection detail on plans.

• Provide park bench details.
• Provide cross -sections for the East and West property lines.
• Re -work drive aisle to provide wider turning radius at the first left turn within the new area.
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA-15 ADD 5 will be performed for the
following breakdown of fees:
