CONSULTANT SERVICE AGREEMENT (CSA)

Project Identification: Stonecrop Drainage Improvements - Wood Storks
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
MB V Engineering, Inc., in conjunction with Florida Environmental Consulting, Inc., shall provide Wood
Stork consulting according to the FWS guidelines as requested by the City and SJRWMD project review
comment letters.
Work Scope:
This would include coordination with the jurisdictions, a site meeting, Wood Stork regulation
coordination, construction plan review with the FWS, negotiation of a 'not likely to affect' determination
and written documentation including an FWS nexus results letter and/or an updated EIR.
Deliverables:
A 'not likely to affect' determination and written documentation including an FWS nexus results letter
and/or an updated EIR.
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA 14 ADD2 will be performed for a fee
not to exceed the amount of:
Environmental Impact Report $3,800
