You are hereby notified of the following for the contract referenced above:
• Effective Date: January 15, 2021
-Complete work ror January and commence work under new contract on February 1, 2021.
• Expiration Date: January 14, 2024
• Renewal Option: Two (2)— one year terms
•City's sole discretion, contingent upon budget availability and Contractor's performance.
Services shall be provided per the terms and conditions of the executed proposal, dated December 9, 2020, in
the annual cost of $171,684.00. Monthly invoicing shall be consistent with Bid Form.
The Project Manager is Lisa Frazier, Community Development Director. All substantial communication
pertaining to this project should be directed to him via e-mail Ifrazier@cityofsebastian.org or by telephone at
772-388-8228.
