The above -mentioned Agreement for Professional Janitorial Services between the City of Sebastian and
Nash Janitorial Services expired on March 11, 2021. At the City's sole discretion, a request is being set forth to extend
the agreement, with expiration on May 31, 2021. All prices, terms and conditions of the previous agreement shall
remain in full force and effect.
Please sign and return this Agreement Extension Notification with an updated Certificate of Insurance to the City
via email by Wednesday, March 17, 2021 to afraser@cityofsebastian.org.
