The extension to the above -mentioned Agreement for Professional Janitorial Services between the City of Sebastian and
Nast) Janitorial Services expires on May 31, 2021. At the Cityrs sole discretion, a request was made to extend the
agreement until September 30, 2021. The City agrees to a monthly rate increase of seven percent (7%) for the four
additional months. All other terms and conditions of the previous agreement shall remain in full force and effect.
Please sign and return this Agreement Extension Notification with an updated Certificate of Insurance to the City
via email before May 31, 2021 to kkillgore@cityofsebastian.org.
