HomeMy WebLinkAboutCSA #5 - Housing Rehabilitation SpecialistCITY OF SEBASTIAN, FLORIDA WORK AUTHORIZATION FORM CONSULTANT SERVICE AGREEMENT (CSA) GUARDIAN COMMUNITY RESOURCE MANAGEMENT, INC. 15000 Citrus Country Drive #331 Dade City, FL 33523 Work Authorization No: CSA-# 5 (RFP#10.12) Prolect Title: Housing Rehabilitation Specialist IT IS AGREED to undertake the following work in accordance with the provisions of the Guardian CRM Inc., Master Agreement entitled CDBG Administragon Services Dated January g, 2020. Description of Assignment: Program delivery In the forth of regular housing specialist services, including Initial observation for program appropriateness, writing a scope of work, technical assistance as needed during the income qualification and readiness process, developing bid documents, monitoring the work for payment, reviewing change orders, and reviewing any homeowner complaints or contractorlhomeowner disputes and recommending appropriate responses or actions for City approval. The Consultant shall follow City Housing procedures and forms, as well implement the Ciy's CDBG housing rehab program. The Consultant shall report directly to the City staff Program Administrator (PA) and support the PA for any City decisions or agenda items that area In addition to normal evaluation, bidding, and observation of the housing rehab work for quality and program goals. This WO is in no way a substitute for City permitting or code compliance requirements and regular City staff performing those duties. Green improvements and hardening will be considered where feasible and program funds are available after all code, safety and housing quality matters are addressed. The Consultant shall recommend bid tabulations, change orders and payments, but the PA shall make the final decisions. 1. Payment shell be by the task and upon completion of the task and any deliverables, Including email answers to questions or reports. Consultant CITY OF SEBASTIAN, FLORIDA WORK AUTHORIZATION FORM CONSULTANT SERVICE AGREEMENT (CSA) payments are not conditioned on whether a case or unit is finished, e.g., if the homeowner withdraws or the case falls out for other unforeseen reasons, the homeowner and contractor cannot agree to contract and proceed, fraud is discovered and work stops and the project is cancelled by the City, etc. The number of units is unknown at this time but will not exceed 6 under this order. Travel and reimbursable expenses are governed by the Agreement. Any changes made after the final approval will be considered additional and charged according to the City approved base Agreement between the City and the Consultant. The work shall be by the task and billable monthly as tasks are completed and deliverables provided. Compensation: Payment for said services is based on the Contract and deliverables outlined above and shall not exceed $2,000/finished housing unit, or the amount for each task completed above for cases that do not get completed for reasons beyond the consultant's control.
