HomeMy WebLinkAboutWork AuthorizationCITY OF SEBASTIAN, FLORIDA WORK AUTHORIZATION FORM CONSULTANT SERVICE AGREEMENT (CSA) CARTER ASSOCIATES, INC. 1708 2151 Street Vero Beach, FL 32960 Work Authorization No: CSA - #2 Proiect Title: Stonecrop Drainage Improvement Project — Phase 2 IT IS AGREED to undertake the following work in accordance with the provisions of Carter Associates, Inc. Master Agreement entitled Stonecrop Ditch — Phase 2 Construction Administration, Dated June 9, 2022. Description of Assignment: Carter Associates, Inc. scope of work/project will include the following: 1. Administrative Tasks • Review and familiarize with phase 1 permit plans. • Send notification to phase 1 consulting engineer taking over design responsibility of construction plans. • Assemble phase 1 CAD files into working drawings necessary to create 3D surfaces for computation of quantities and x- sections necessary for construction detail. • Modify phase 1 permit plans as necessary to provide sufficient details for construction purposes. • Meet with City staff(s) to review modified construction plans in detail and prepare for bidding services. 2. Bid Document Preparation/Process • Attend meetings with City staff(s) to develop outline for bid documents utilizing City of Sebastian front end documents. • Prepare a bid item list which will include a "quantity takeoff' to be utilized in the contract documents for the purpose of obtaining unit bid prices during bidding. • Coordinate with the CLIENT on bid dates and provide a pdf file of bid documents and plans for web site posting. CLIENT shall be responsible for advertisement for bids. • Attend a pre -bid meeting. • Provide technical assistance to address contractor questions during the bid process and assist with issuance of addendums if required. • Attend bid opening. • Meet with CLIENT to review and evaluate bids and alternate bid items. • Provide a spreadsheet identifying the bid items along with a side -by -side comparison of each bidder and their bid item prices. • Assist CLIENT with contractor selection and recommendation for award of contract. • Prepare final contract construction documents for execution. 3. Site Inspections • Attend pre -construction meeting with the contractor and CLIENT representative(s). • Review contractor's preliminary schedule, monitor on -going construction progress, and coordinate with contractor to _provide scheduled updates for duration of construction. Provide schedule updates to CLIENT. • Respond to _cohtractor Request for Information (RFI's) and provide construction clarifications as needed. • Review contractor's submittals and shop drawings for approval. • Provide silt fence and erosion control device inspection. • Review density tests and geotechnical reports. • Coordinate with contractor for MOT plan during proposed roadway closing(s). • Conduct regular periodic construction observations/inspections to verify that the construction is proceeding in accordance with the plans and specifications. • Consult with CLIENT for any change order requests during construction. • Review Pay Requests from the contractor and submit to the CLIENT following approval. 4. Project Closeout • Conduct final inspection(s) and prepare punch list of work deficiencies. • Review as -built survey (to be provided by contractor) for compliance with approved plans. • Coordinate with contractor to address deficiency punch list items and re -inspect. • Prepare final certifications to governmental permitting agencies. 5. We are pleased to present this proposed agreement to provide civil engineering services for the below described tasks for the City of Sebastian — Stone Crop Ditch Phase 2 project. The following describes our proposed scope of work and terms of agreement between Carter Associates, Inc. (CAI) and the City of Sebastian (CLIENT): SECTION 1: SCOPE OF SERVICES The following scope of services is based on a construction duration ending February 2024. Construction beyond this period may require additional fees. This proposal specifically excludes all application and/or impact fees, which shall be the responsibility of the CLIENT. 2. It is our understanding that all permitting/jurisdictional approvals have been obtained with phase 1. Therefore, this agreement excludes all engineering services for permitting approvals. Services in this agreement are limited only to construction administration services. 3. Architectural, landscape, electrical, mechanical, and structural services are not included with this fee proposal. 4. Construction stakeout and as-builts are not included with this proposal. SECTION 3: COMPENSATION The proposed scope of services shown below will be invoiced monthly based on work performed to date for each task.
