You are hereby authorized to commence work on November 8, 2017 in accordance with CAS #7,
dated November 7, 2017 for the Indian River Drive Milling and Resurfacing evaluation, beginning at the
Sportsman's Marina & Lodge to Captain Butcher's.
The Project Manager is Joseph Griffin, City Manager. All substantial communication pertaining

Project Identification: Indian River Drive Milling & Resurfacing from Sportsman's Marina & Lodge (412
Indian River Drive) north approximately 1.7 miles to Captain Butchers (1730 Indian River Drive).
IT IS AGREED to undertake the following work in accordance with the provisions of the MBV
Engineering, Inc. Master Agreement entitled Non -Exclusive Continuing Professional Services Agreement
for Engineering Dated July 12, 2017.
Description of Assignment:
Per our coordination, it is our understanding that the City of Sebastian is requesting an evaluation be
performed on Indian River Drive roadway within the City limits from approximately Sportman's Marina
& Lodge north to Captain Butchers. This evaluation will be used to determine if milling and re -surfacing
the roadway is a viable alternative to full roadway replacement.
Work Scope:
Evaluation
• Perform an on-site evaluation of the existing conditions for the specified portion of Indian River Drive.
• Preparation of a findings report with photographs.
Civil Design and Permitting
• Preparation of construction plans for the roadway improvements via existing aerial photography (no
survey included).
• Preparation and submittal for FDEP and SJRWMD permits, if required.
Bidding Assistance
• Coordination with City personnel for the preparation of the bid documents
• Assist City with bid evaluation and contract award
Construction Administration
• Construction Inspection / Certification (including pre -construction meeting, construction progress
inspections, final as -built walk-through, and final overall site inspections / certifications)
• Prepare and submit all final project certifications for project clearance
Basis of Compensation/Period of Services:
The above referenced Professional Engineering Services for CSA 7 will be performed for a fee not to
exceed the amount as follows, including direct costs:
